Durante la evaluación ginecológica se evidenció una lesión cervical, por lo que se realizó colposcopia, identificándose una lesión acetoblanca sugestiva de lesión intraepitelial de alto grado. Posteriormente, la biopsia cervical confirmó el diagnóstico de carcinoma in situ. La paciente fue evaluada por el servicio de ginecología y recibió indicación de manejo quirúrgico conservador mediante conización cervical con bisturí frío. El procedimiento se realizó sin complicaciones, con resección adecuada de la lesión y evolución postoperatoria favorable. El caso evidenció la importancia del tamizaje oportuno mediante citología cervical, así como el valor de la colposcopia y la biopsia dirigida para la confirmación diagnóstica. Asimismo, resaltó el papel de la conización cervical como alternativa terapéutica conservadora en lesiones precursoras de alto grado. En conclusión, el diagnóstico precoz y el tratamiento oportuno del carcinoma in situ de cuello uterino constituyen estrategias esenciales para reducir la morbimortalidad, especialmente en contextos con acceso limitado a servicios preventivos oncológicos especializados.A case was reported of a woman from Cajamarca who presented with pelvic pain, dyspareunia, and postcoital bleeding, with a cytological history of carcinoma in situ. During the gynecological evaluation, a cervical lesion was observed, and a colposcopy was performed, identifying an acetowhite lesion suggestive of high-grade intraepithelial lesion. Subsequently, a cervical biopsy confirmed the diagnosis of carcinoma in situ. The patient was evaluated by the gynecology service and received an indication for conservative surgical management via cervical conization with a cold scalpel. The procedure was performed without complications, with adequate resection of the lesion and a favorable postoperative course. This case highlighted the importance of timely screening through cervical cytology, as well as the value of colposcopy and targeted biopsy for diagnostic confirmation. Furthermore, it highlighted the role of cervical conization as a conservative therapeutic alternative for high-grade precursor lesions.
