    Factors associated with abandonment of contraceptives hormones in puerpera between 2017 – 2022

    The research aimed to identify the factors associated with the discontinuation of hormonal contraceptives in postpartum women between 2017 and 2022. The methodology developed was a qualitative, narrative systematic review. It was conducted through a collection of scientific publications from databases such as Google Scholar, Scielo, PubMed, Redalyc, and Elsevier. A total of 30 publications were selected as scientific evidence, where the associated factors (sociodemographic, clinical, gynecological-obstetric, and institutional) were the independent variable, and the discontinuation of hormonal contraceptives was the dependent variable. According to the 30 reviewed scientific publications, the factors associated with contraceptive discontinuation in postpartum women were: sociodemographic factors (36.6%), such as partner pressure, desire for pregnancy, and low education level; clinical factors (30%), such as bleeding, headaches, and weight gain; gynecological-obstetric factors (23.4%), such as primiparity and lack of counseling; and institutional factors (10%), such as dissatisfaction with the service and poor knowledge level among postpartum women. In summary, 100% of the reviewed scientific publications identified the existence of factors associated with the discontinuation of hormonal contraceptives in postpartum women between 2017 and 2022.

    factors associated with the abandonment of exclusive breastfeeding in adolescents

    Design: This study was a systematic narrative non-experimental, retrospective review based on an electronic search in virtual libraries. Study scope: This systematic review study selected original scientific articles and thesis research from the Renati SUNEDU database, with a maximum age of 7 years. Study subjects: A total of 30 scientific articles were considered. Determinants: Maternal, socioeconomic, and cultural factors are the independent variables, while the abandonment of exclusive breastfeeding represents the dependent variable. Results: The most prominent maternal factors were insufficient milk and self-efficacy; the socioeconomic factors included working during the first six months of the child's life and income level; while cultural factors highlighted the fear of damaging the breasts and the husband's support seen through a gender perspective. Conclusions: The factors significantly associated with the abandonment of exclusive breastfeeding in adolescents were grouped into three categories: maternal, socioeconomic, and cultural, with maternal factors showing the highest odds ratio values.

    Knowledge and use of emergency oral contraceptives in women of fertile age from 2017 to 2022

    Objective: To determine the scientific evidence on the knowledge and use of emergency oral contraception in women of reproductive age. Design: This study was conducted through a narrative review of various scientific articles, selecting those with descriptive, cross-sectional, correlational, case, and cohort methodologies. Articles published between 2017 and 2022 were considered, and 30 articles were reviewed. Results: According to the obtained results, the methodology used was 64% descriptive, 21% correlational, 10% cohort, and 2% cases and reviews. Regarding knowledge, 53% had low knowledge, 30% had medium knowledge, and 17% had high knowledge. Regarding use, 47% had poor use of emergency oral contraception, 37% had regular use, and 17% had good use. Conclusions: The reviewed scientific evidence confirms that the lack of knowledge and, consequently, the improper use of emergency oral contraception arise due to a lack of information and insufficient health promotion.

    Laparoscopia en apendicitis aguda con peritonitis complicada en gestante de primer trimestre en clínica particular de Lima marzo 2022

    Objetivo: Describir el manejo de apendicitis aguda complicada mediante tecnica laparoscopica en gestante de primer trimestre en Clínica privada de Lima en marzo 2022. Material y métodos: Estudio observacional, descriptivo, retrospectivo y revisión de caso clínico sobre el manejo de Laparoscopia en apendicitis aguda con peritonitis complicada en gestante de primer trimestre en clínica privada de Lima en marzo, 2022. Se realizó revisión de literatura para discusión de caso clínico. Descripción del caso clínico: Se presenta el caso de un paciente primigesta 7ss, quien ingreso por un cuadro de 2 días de evolución, caracterizado por dolor abdominal difuso de moderada intensidad asociado a nauseas, vómitos y deposiciones liquidas, mismas que fueron aumentando de intensidad paulatinamente. Mediante la correlación clínico-patológica se llegó al diagnóstico de Abdomen agudo - Apendicitis aguda. Al paciente se le realizaron exámenes para descartar etiologías infecciosas, fue tratado mediante antibióticos y analgesia, continuando el pase a sala de operaciones donde confirman el diagnostico de apendicitis con peritonitis generalizada. Paciente continua hospitalización por 5 días con monitoreo continuo antibiótico terapia analgesia horaria, control de drenaje Jackson pratt y BHE; al encontrarla estable al PO5 se le procede a retiro de drenaje y alta con indicaciones. Paciente continúa su gestación llegando a término en octubre 2022 vía parto por cesárea con recién nacido vivo sin complicaciones Conclusión: El diagnostico de apendicitis aguda en gestante del primer trimestre de una clínica en Lima se basa principalmente en el cuadro clínico correlacionados con exámenes e imágenes diagnósticas. En este caso en específico se utiliza laparoscopia como técnica menos invasiva donde no evidenciamos complicaciones a corto o largo plazo sobre la madre, el embrión o nacido vivo. A continuación se presenta el caso, junto con una revisión sobre el tema de apendicitis aguda; abarcando aspectos relevantes sobre su incidencia en la gestación, cuadro clínico y tratamiento.

    Maternal and perinatal complications in pregnant women with Covid-19.

    Objective: To identify the scientific evidence regarding maternal and perinatal complications in pregnant women with COVID-19. Design: This study was a non-experimental narrative systematic review, involving an exhaustive electronic search of articles related to maternal and perinatal complications in pregnant women with COVID-19. Study setting: The systematic review selected various articles from academic databases, including systematic reviews, retrospective, prospective, cohort, and case-control studies, published between 2019 and 2021. Subjects: A total of 35 scientific articles were included in this study. Findings: Maternal and perinatal complications are the dependent variables, and COVID-19 is the independent variable. Pregnant women with pre-existing conditions, risk factors, or over the age of 35 are more likely to be hospitalized, often resulting in pregnancy termination, which can be harmful before 37 weeks of gestation, causing prematurity, low birth weight, intrauterine growth restriction (IUGR), and neonatal pneumonia. Additionally, spontaneous abortions were observed in pregnancies during the first trimester. Pregnant women are more vulnerable to SARS-CoV-2 infection, leading to morbidity and mortality. Conclusions: Maternal complications include hypertensive disorders, placental abruption, disseminated intravascular coagulation, placenta previa, premature rupture of membranes, and spontaneous abortions. Similarly, perinatal complications include intrauterine growth restriction, prematurity, low birth weight, respiratory issues, tachycardia, bradycardia, and neonatal pneumonia.

    Nivel de conocimientos y actitudes sobre los riesgos de un embarazo en la adolescencia en estudiantes de 4° y 5° de secundaria de la Institución Educativa 5093 Ventanilla

    Material y método: Metodología descriptiva, cuantitativa, correlacional y de corte transversal. La muestra estuvo conformada por 205 estudiantes (varones y mujeres) de la institución 5093 Ventanilla. Resultados: De los 205 estudiantes que formaron parte del estudio, 15 (7%) de estos presentaban un nivel bajo en cuanto a los conocimientos que tenían sobre los riesgos de un embarazo en la adolescencia; 30 (15%) de estos presentaban un nivel medio; y 160 (78%) de estos presentaban un nivel alto, en cuanto a la actitud frente a los riesgos de un embarazo adolescente, de los 205 estudiantes que formaron parte del estudio, 27 (13%) de estos presentaban actitudes negativas sobre los riesgos de un embarazo en la adolescencia; 30 (15%) de estos presentaban actitudes regulares; y 148 (72%) de estos presentaban actitudes positivas. Se halló una relación significativa entre el nivel de conocimientos y las actitudes sobre los riesgos del embarazo adolescente (p valor igual a 0.000 y Rho igual a 0.859). Conclusión: Existe una relación significativa, elevada y directamente proporcional entre los constructos considerados dentro de la investigación.

    Positions of the second stage of labor and their association with outcomes maternal.

    The objective of the research is to determine the scientific evidence regarding the association of positions during the second stage of labor with maternal outcomes. The study design was a systematic review. Information was collected by searching articles in electronic databases such as PubMed, Dialnet, Cochrane, ScienceDirect, Latin American and Caribbean Literature in Health Sciences (LILACS), Google Scholar, and repositories from international and national universities. A total of 30 studies published between 2017 and 2021 were selected. After analyzing the articles, the results showed that the vertical position during the second stage of labor provides a positive perception among women, reduces pain, the duration of the expulsive stage, and the use of medications compared to the horizontal position. It was concluded that there is an association between positions during the second stage of labor and maternal outcomes.

    Relationship between the level of knowledge and attitude towards vertical birth in pregnant women treated at the José Carlos Mariátegui maternal and child center in Villa María del Triunfo, 2023

    Peru is considered a multicultural country, with varied and different customs rooted in each community, having a relationship with each other. If necessary, the Ministry of Health, among other organizations, has been working together with health personnel to respect and put into practice, at the rural and urban level, their customs, beliefs and rights. Today, there are guidelines to facilitate intercultural adaptation and relevance, as well as assertive transmission through the constant training of Doctors and Obstetricians, to provide appropriate care and information to pregnant women in our country. OBJECTIVE: The objective of this study is to determine the relationship that exists between the level of knowledge and attitudes of vertical birth in pregnant women treated at the José Carlos Mariátegui Maternal and Child Center, 2023. MATERIAL AND METHODS: Correlational, quantitative study, non-experimental design of cross section. The population was made up of 120 pregnant women. The information was obtained through surveys that were divided into 4 parts: knowledge of vertical birth, attitude of pregnant women towards vertical birth, obstetric and sociodemographic data in 21 open questions adapted to the Estanones scale. RESULTS: Data from 120 pregnant women between 18 and 37 years of age from the José Carlos Mariátegui Maternal and Child Center were analyzed. In this study, it was found that 64 (53%) of these. They presented a medium level of knowledge, 72 (60%) of them obtained a regular attitude, which certified a characteristic relationship according to the Rho equal to 0.864 between knowledge and the attitude of the pregnant women. CONCLUSION: Consequently, a high and directly proportional relationship was observed between the level of knowledge and attitudes towards vertical birth in the women attended at the José Carlos Mariátegui Maternal and Child Center.

    Risk factors for urinary tract infection and maternal complications during pregnancy

    Objective: To determine the scientific evidence on the risk factors for urinary tract infection and maternal complications during pregnancy. Material and Methods: The methodology for developing the study was based on a systematic qualitative review and consisted of 28 articles published in the following virtual databases: Scielo, Elsevier, Science Direct, and Redalyc. The GRADE system was used to evaluate the strength of recommendation for each reviewed article. Results: 25% of the studies were case-control studies. Regarding the quality level of the evidence, 64% of the studies were of high quality. Concerning risk factors for urinary tract infection, 23% of the studies reported anemia, 20% adolescence, 18% incomplete prenatal care, 15% a history of urinary infections, recurrent vaginal infections, and the use of intimate protectors, each with 6%. Regarding maternal complications, 31% had preterm labor, 24% premature rupture of membranes, 15% preterm labor threat, 12% abortion, and also found hypertensive disease of pregnancy, abortion threat, and intrauterine growth retardation, each with 6%.

    Sexual Behavior of Adolescents from 15 to 19 years old Associated with the use of Contraceptive Methods

    Objective: To determine and analyze the scientific evidence on the sexual behavior of adolescents aged 15 to 19 years associated with the use of contraceptive methods. Methodology: A systematic review was conducted using virtual libraries such as Google Scholar, Redib, Redalyc, and Scielo. Results: A total of 40 articles were selected based on inclusion and exclusion criteria. Most of the articles (37) were from Latin America and the Caribbean, while only 3 were from Spain. It was evident that the main factor affecting adolescents' sexual behavior is misinformation. Additionally, many studies agree that adolescents start their sexual lives at a very early age due to social pressure, curiosity, parental issues, and more. Conclusion: Most of the analyzed articles do not show a significant relationship between the sexual behavior of adolescents aged 15 to 19 years and the use of contraceptive methods. It is concluded that adolescents lack knowledge or hold incorrect information about contraceptive methods, putting their sexual health at risk, including exposure to sexually transmitted diseases or unintended pregnancies.
