Materials and Methods: Systematic review, observational, and retrospective study. The research was restricted to full-text articles, and the selected articles were critically reviewed using Jover's evaluation to determine their level of evidence. Results: A variety of related studies were reviewed. In the final selection, 10 articles were chosen, with 3 articles each from Brazil and Iran, 2 from Turkey, and 1 from Australia and the Netherlands. Conclusions: The studies analyzed provide evidence on the efficacy of a closed suction system compared to an open suction system in hemodynamic changes in patients on mechanical ventilation. 60% (6/10) of the studies demonstrated an impact on hemodynamic changes, while 40% (4/10) did not report significant changes in either system. Consensus recommendations and specialist opinions emphasize the importance of monitoring hemodynamic changes during the suction procedure, regardless of whether a closed or open system is used, to prevent complications. The closed suction system is recommended as safer, and it is clarified that nursing staff should not perform suction as part of a routine due to potential adverse effects.
