Ítem Acceso abierto Effectiveness of sternal closure with absorbable materials in cardiac surgeries through median sternotomy in the prevention of complications(Universidad Privada Norbert Wiener, 2019-08-18) Condor Rios, Ruth Esperanza; Sánchez Linares, Cyntia Margot; Calsin Pacompia, WilmerObjective: To analyze the evidence on the effectiveness of sternal closure with absorbable materials in cardiac surgeries through median sternotomy in the prevention of complications. Materials and Methods: Systematic, quantitative review; Grade system. Population of 19 articles, sample: 10 scientific articles published and indexed in databases: PubMed, EBSCO and Academic Google, Grade system. Results: The studies correspond to 40% control cases, 40% randomized clinical trials and 20% analytical cross-sectional, absorbable materials are fibrin-coated sheet, collagen, bone cement, biological adhesive; They report lower rates of instability compared to conventional sternal closure; use of absorbable L-Poly-Lactide acid pins with or without hydroxyapatite, reports lower rates of sternal displacement, greater bone density and tension force overload to achieve anteroposterior displacement; one study used polyglyconate suture in high-risk patients without finding sternal dehiscence; frozen bone graft has lower rates of sternal instability and infection compared to the control group. Conclusions: 90% of the studies reviewed (n= 9/10) show effectiveness of sternal closure with absorbable materials in cardiac surgeries through median sternotomy, obtaining better rates of instability and sternal dehiscence, sternal displacement, favoring osteogenesis, sternal fusion. 10% of studies show ineffectiveness in sternal closure
