    Seguimiento farmacoterapéutico a pacientes que usan anticoagulantes atendidos en el Hospital Nacional PNP “Luis N. Sáenz” – 2016

    The main objective of this study was to perform pharmacotherapeutic follow-up in patients on anticoagulant treatment who visited the PNP “Luis N. Sáenz” National Hospital from September to November 2016. The study was prospective, observational, and descriptive. The data collection technique involved reviewing clinical records with the pharmacological anamnesis format and identifying Drug-Related Problems (DRPs) according to the Granada consensus. The results showed that of the 109 patients, 86% (n=94) had DRPs. A total of 185 DRPs were found, with 39% (n=73) being potential and 61% (n=73) being actual. Additionally, it was identified that of the 109 patients, 74% used Warfarin, and only 26% used Rivaroxaban, due to its high cost. We conclude that the most prevalent DRPs were Necessity DRPs (38%), followed by Safety DRPs (34%), and finally, Effectiveness DRPs (28%). The pharmacotherapeutic follow-up applied to patients using oral anticoagulants was successfully designed and implemented in daily consultations at the PNP Luis N. Sáenz National Hospital.
