Ítem Acceso abierto Effectiveness of the tetravalent human papillomavirus vaccine administered to women under 45 years old in the prevention of cervical cancer(Universidad Privada Norbert Wiener, 2017-11-12) Arámbulo Figueroa, Luis Enrique; Cruz Zapata, Ligia Mavel; Bonilla Asalde, César AntonioObjective: To systematize the evidence regarding the effectiveness of the quadrivalent human papillomavirus vaccine administered to women under 45 years of age in the prevention of cervical cancer. Methodology: This is a systematic observational and retrospective review that synthesizes the results of multiple studies. The search was restricted to full-text articles, and the selected articles were subjected to a critical reading and analyzed based on the evidence and strength of recommendation according to the GRADE system. Ten articles were chosen, with 20% of them corresponding to studies from Spain, while the others came from Argentina (20%), Brazil (10%), Japan (10%), Colombia (10%), Canada (10%), Australia (10%), and Mexico (10%). Considering the research designs, the included studies consisted of four randomized controlled trials, two systematic reviews, two meta-analyses, and two case-control studies. Results: The quadrivalent human papillomavirus vaccine is effective in reducing the occurrence of cervical intraepithelial neoplasia lesions of grade 2 and 3 by 80-100%, also demonstrating high immunogenicity and acceptable safety. Following vaccination, the incidence of human papillomavirus infection decreased by 87.6%, and the effectiveness of two doses for preventing high-grade lesions was 76%, while with three doses it was 84%. Conclusions: The quadrivalent human papillomavirus vaccine is highly effective in preventing cervical cancer in women under 45 years of age.
