To analyze the factors associated with medication acquisition in older adults with chronic comorbidities (hypertension and diabetes mellitus) in Peru, using data from the Demographic and Family Health Survey (ENDES 2023). Methodology. An observational, cross-sectional and analytical study was conducted. Microdata from ENDES 2023 were used to identify adults aged 60 years or older with a diagnosis of hypertension and/or diabetes mellitus. A descriptive analysis of sociodemographic, health insurance, and treatment adherence variables was performed, followed by bivariate analysis using Pearson’s chi² test and multivariate analysis through binary logistic regression to estimate prevalence ratios (PR) and 95% confidence intervals. A significant level of 0.05 was established. Results. Most older adults acquired medications with a medical prescription, with no statistically significant differences according to sex, age, educational level, marital status, type of health insurance, or treatment adherence (p > 0.05 in all cases). The prevalence of hypertension was 30.2% and that of diabetes mellitus was 10.8%. Adjusted models did not show statistically significant associations between the analyzed variables and the practice of medication acquisition. Conclusion. Among older adults with hypertension and diabetes mellitus in Peru, medication acquisition is predominantly carried out through medical prescriptions, with no significant associations found with sociodemographic characteristics, type of health insurance, or treatment adherence
