    Association of the neutrophil/lymphocyte index with the mortality of covid-19 patients at Hospital II Clínica Geriátrica San Isidro Labrador October 2020 – February 2021. Lima – Peru

    The neutrophil-lymphocyte index is a hematological marker previously studied for inflammation in various diseases. In COVID-19 infection, the NLR has gained significance for determining severity and predicting mortality in infected patients. This thesis seeks to associate the NLR with the mortality of COVID-19 patients at the San Isidro Labrador Geriatric Clinic, Lima, from October 2020 to February 2021. The study evaluated 180 patients diagnosed with COVID-19, using hematological values to determine the NLR and seek associations using SPSS. It found that 72.8% of deceased patients were male, with an average age of 76.1 years. The odds ratio was calculated, revealing that patients with an NLR >7 had a 9.7 times higher risk of death. The logistic regression analysis found that patients with elevated NLR had 1.5 times higher mortality risk (p: 0.000). It was concluded that NLR is a good predictor of mortality in COVID-19 infected patients.

    Characteristics of the Neutrophil Lymphocyte Index in the Acute Phase of Outpatient Covid-19 patients at the Conde de la Vega Baja health center in Lima, Peru, December 2020- January 2021

    COVID-19 causes an inflammatory pulmonary response triggered by the cytokine storm. The clinical laboratory has contributed with analyses that help monitor patient progression, with the neutrophil-lymphocyte index (NLR) providing significant prognostic value, as it is associated with lymphopenia due to lymphocyte migration towards the lungs. The aim of this study was to determine the characteristics of the neutrophil-lymphocyte index in the acute phase of ambulatory COVID-19 patients at the Conde de la Vega Baja Health Center in Lima, Peru, from December 2020 to January 2021. This was a descriptive, observational (non-experimental), and cross-sectional study involving 30 patients with non-probabilistic convenience sampling. Participants had a recent reactive rapid test result for IgM or a positive molecular or antigen test, and did not have hematological or infectious diseases. The only measured variable was the neutrophil-lymphocyte index, and blood sample collection and processing followed established protocols. Data analysis was performed using Excel 2016, with tables and graphs for means and medians.

    Comparative analysis of the performance of two automated hematological platforms in a level III-I Hospital of the MINSA-Peru in 2023

    Objective: To compare the analytical performance of the two hematological platforms Beckman Coulter DxH 900 and Mindray BC 6200 used in a level III-I Hospital in Minsa-Peru 2023. Materials and Methods: Based on the CLSI EP15-A3(1) guide, 5 replicates will be performed for each control level over 5 consecutive days, totaling 25 runs per level for leukocytes, erythrocytes, hemoglobin, and platelets. Based on the CLSI EP6-A(2) guide, 5 samples with equidistant concentrations will be tested 3 times for leukocytes and platelets. Results: On the Beckman Coulter DxH900 platform, the Sigma values are 10.9 for leukocytes, 4.6 for erythrocytes, 5.3 for hemoglobin, and 8.1 for platelets. The results for %CVWL(Lab) were lower than the %CVWL(Fab), thus the verification is accepted for each analyte. The overall precision determination for each analyte was based on the limiting level, so the imprecision is 1.5 for leukocytes, 0.8 for erythrocytes, 0.6 for hemoglobin, and 1.6 for platelets. For the Mindray BC6200 platform, the Sigma values are 6.5 for leukocytes, 8.5 for erythrocytes, 7.4 for hemoglobin, and 4.6 for platelets. The %CVWL(Lab) results were lower than the %CVWL(Fab), so the verification is accepted for each analyte. However, the %CVR(Lab) for platelets was rejected at level 1 because it exceeded the %CVR(Fab) and accepted for other levels, but the intralaboratory condition is considered acceptable. The overall precision determination for each analyte was based on the limiting level, with imprecision of 1.6 for leukocytes, 0.7 for erythrocytes, 0.3 for hemoglobin, and 4.8 for platelets. Conclusions: Both platforms met the manufacturer's accuracy and precision specifications, with a favorable Sigma, considering that both platforms worked with the same short quality requirement (VBD). The Beckman platform performed better compared to the Mindray platform.

    Correlation between leukocytes and protein C, reactive in patients hospitalized for Covid-19 at Hospital San José 2020

    The COVID-19 pandemic has affected millions of people and caused hundreds of thousands of deaths worldwide. Pneumonia and severe hypoxemia associated with severe acute respiratory syndrome caused by coronavirus infection (SARS-CoV-2) can lead to acute respiratory distress syndrome (ARDS), which is associated with a high mortality rate. Severe acute respiratory syndrome clinical examinations found leukocytosis with the severity of infection, such as C-reactive protein. Laboratory findings show a positive relationship between the increase in C-reactive protein and leukocytes, which may relate to the severity of the inflammatory condition. Therefore, this project aims to determine the relationship between leukocytes and C-reactive protein. In this study, clinical records were used, meeting the inclusion and exclusion criteria. The method used will be a hypothetical-deductive approach with a quantitative, applied, correlational, cross-sectional, and retrospective design, utilizing SPSS tools, with results indicating a positive correlation between leukocyte levels and C-reactive protein.

    Correlation between the blood profile and glucemia in patients with type ii diabetes cared for in a hospital in Arequipa, period 2020 to 2023

    The study, conducted at a hospital in Arequipa, analyzed the laboratory data of 97 patients with type II diabetes. Data were collected to determine the relationship between hematologic profiles and blood glucose levels. The Spearman correlation coefficient was used, with a P-value of 0.000 and a very strong positive correlation for the variables hemoglobin, hematocrit, total white blood cells, neutrophils, eosinophils, monocytes, lymphocytes, and glucose (+0.75 to +0.90), a high positive correlation for band cells and glucose (+0.51 to +0.75), and a moderate positive correlation for basophils and glucose (+0.11 to +0.50), indicating that most of the study elements—hemoglobin, hematocrit, total and percentage white blood cell count—are related to glucose levels.

    Diagnostic performance of Platelet Indices in the identification of degrees of severity of Gastric Cancer in a hospital center, period 2021 – 2023

    Stomach cancer is a type of cancer that affects the stomach lining due to the uncontrolled proliferation of its cells, being considered by the WHO as one of the main global health issues. Timely diagnosis, follow-up, and evaluation of people at risk for stomach cancer are essential, and laboratories play a critical role in this process, offering tests for clinicians. The role of platelets in inflammatory processes related to neoplasms is currently being discussed, and it is likely that platelet activation by neoplastic cells contributes to metastasis and cancer progression by stimulating deep vein thrombosis, thrombo-inflammation, and the formation of neutrophil extracellular traps that hide cancer cells. The information provided by platelet analysis is important, as these values can indicate the severity level developed by a person with stomach cancer and potentially improve therapeutic strategies. However, these markers are still under investigation and should be complemented with clinical and epidemiological data to help forecast patient health and provide more personalized treatment.

    Diagnostic validity of the brazilian formula compared to the precipitation method for the estimation of low density lipoprotein in the Zarate health center in lima Perú, year 2022.

    To determine the diagnostic validity of the Brazilian formula compared to the precipitation method for estimating low-density lipoprotein. Methodology: The research method is hypothetical-deductive, quantitative, descriptive, and non-experimental, based on 353 lipid profile results from patient records between January 2 and December 31, 2018. Results: Of the 353 results, 237 (67.1%) were from women, and 116 (32.9%) from men. The most common age group was adults aged 30-59 years, with 175 results (49.6%). There was a moderate level of agreement for the Brazilian formula. Conclusion: The Brazilian formula showed 93.8% sensitivity, 81.7% specificity, 93.5% positive predictive value, and 82.6% negative predictive value compared to the precipitation method.

    DNA extraction from samples fixed on slides for molecular detection of Mycobacterium Leprae Lima, 2023

    Leprosy is an infectious disease caused by Mycobacterium leprae. In Peru, the standard laboratory diagnosis is bacilloscopy from smear-fixed samples. In recent years, other countries have started using PCR testing as an alternative for diagnosis due to its high sensitivity. Objective: To extract DNA from smear-fixed samples for molecular detection of Mycobacterium leprae. Methodology: The study included 50 samples from skin incision scrapings fixed on glass slides (30 samples with positive bacilloscopy and 20 samples with negative bacilloscopy). DNA was extracted using a silica column and the 16S rRNA sequence was amplified by real-time PCR (qPCR). Results: The quality of the extracted DNA ranged from low to good, which did not affect amplification through qPCR. DNA concentrations ranged from 0.5 ng/mL to 67.2 ng/mL. qPCR results showed 100% categorical agreement for the 30 positive samples and 40% disagreement for the 20 negative samples. Conclusion: Good results were obtained from DNA extraction of Mycobacterium leprae from smear-fixed skin incision scraping samples.

    Efficiency in the alarm signs of immature cells reported from the automated hematology analyzer of the “Beckman Coulter 600“ equipment with the revision of the peripheral plate in adult patients, Hospital Nacional Hipólito Unanue, El Agustino Peru, December 2019 - February 2020

    The objective was to determine the efficiency of immature cell alarms reported by the automated hematological analyzer in the "Beckman Coulter 600" with peripheral smear review in adult patients at the National Hipólito Unanue Hospital from December 2019 to February 2020. A total of 489 reports of immature leukocytes were evaluated. The efficiency, sensitivity, specificity, positive and negative predictive values, as well as the Kappa concordance index, were calculated. An efficiency of 95.3%, sensitivity of 70.0%, specificity of 95.8%, positive predictive value of 25.9%, and negative predictive value of 99.4% were found. The Kappa concordance index was 0.36. The efficiency level of the immature cell alarms in the central laboratory, reported by the Beckman Coulter equipment at the National Hipólito Unanue Hospital, was 95.3%. The sensitivity and specificity of the alarms of the Beckman Coulter automated equipment were 70.0% and 95.8%, respectively. The positive and negative predictive values of the alarms were 25.9% and 99.4%. The degree of concordance between the immature cell alarms of the automated hematological analyzer and the peripheral smear was 0.36, classified as mild.

    Evaluation of 3 indirect methods for estimated platelet count in peripheral blood slim Lima ‐ Perú, 2022

    The inaccuracy in platelet counts using automated equipment in cases of thrombocytopenia or certain types of thrombocytosis may cause alterations in platelet variability due to the limitations of the methodology. When "Flags" or alerts appear in hematology analyzers, their verification is still carried out using a peripheral blood smear, where qualitative and quantitative alterations in the three blood cell lines can be observed. In Metropolitan Lima and Callao, Clinical Laboratory Technologists who estimate platelet counts in blood smears after detecting platelet alerts in their equipment use various indirect methods based on field parameters or erythrocytic indices, such as the 20,000 factor method, Fonio method, Dameshek method, and others that are less common. Objective: To evaluate the difference between three indirect methods for estimating platelet counts compared to the gold standard in peripheral blood smears. Materials and Methods: The research method is hypothetical-deductive, applied, quantitative, and experimental with a diagnostic study design.

    Evaluation of reticulocytic hemoglobin as a biomarker of iron deficiency anemia in adults at the Hospital Nacional Dos de Mayo, Lima 2023

    Iron deficiency is the most frequent cause of anemia, and reticulocyte hemoglobin is a new hematogram parameter of great value in diagnosing and managing iron-deficient erythropoiesis. Objective: To determine the usefulness of reticulocyte hemoglobin in diagnosing iron deficiency anemia in adult Peruvian patients. Methods: The clinical records of 120 adult patients who had undergone bone marrow aspiration at the National Hospital Dos de Mayo, according to selection criteria, were reviewed from January to December 2023. A study form was used to collect the results of total hemoglobin, reticulocyte hemoglobin, ferritin, and C-reactive protein (CRP) levels. Bone marrow hemosiderin values, which were used as the standard test to determine iron deficiency, were also collected. Anemia was defined in women as hemoglobin levels less than 11 g/dL and in men less than 12 g/dL. An elevated CRP value greater than 10 mg/L was considered indicative of an inflammatory state. Participants were divided into two groups based on hemosiderin levels: the iron deficiency anemia group (hemosiderin ≤ 2+) and the non-iron deficiency anemia group (hemosiderin > 2+). Results: The frequency of iron deficiency anemia was 77.5%, and the frequency of inflammatory states was 76.8%. No significant differences in reticulocyte hemoglobin levels were found according to gender or age group in patients with iron deficiency anemia. According to the ROC analysis, the area under the curve (AUC) for reticulocyte hemoglobin was 0.525 (p=0.679) and for ferritin, 0.624 (p=0.041). The optimal cutoff point for reticulocyte hemoglobin was 33.9 pg, with a sensitivity of 43% and specificity of 70.4%. For ferritin, the cutoff value was 501 ng/mL, with a sensitivity of 48.4% and specificity of 77.8%. Reticulocyte hemoglobin levels were significantly affected by inflammation, unlike ferritin. Conclusion: Reticulocyte hemoglobin and ferritin have poor diagnostic efficacy in patients with iron deficiency anemia who present an inflammatory state.

    A carefully designed research instrument will be developed, which will not only facilitate the precise collection of data but will also serve as a valuable tool for future investigations in this field. The goal is to compare and analyze the results obtained to identify sensitivity, specificity, and predictive value, in addition to integrating the breath test as the first-choice method for detecting Helicobacter pylori gastrointestinal infections in patients from a private clinic. The method used will be the hypothetical-deductive approach, which involves formulating hypotheses to predict observable future results, with the objective of validating them. The research will be quantitative, focusing on obtaining measurable results. The study will follow a non-experimental, descriptive, and cross-sectional design, and the sample will be census-based, including 100% of the population, composed of 100 patients attending the gastroenterology service who were referred to the Clinical Laboratory of a private clinic in 2022 and met the inclusion and exclusion criteria.

    Factors associated with anemia in first-time teenage pregnant women cared for in external offices of the Institute National Maternal Perinatal – 2022

    The main objective of the research was to determine the factors associated with anemia in first-time adolescent pregnant women attending the outpatient clinics of the National Perinatal Institute in 2022. The study employed a deductive-prospective method, basic type, with a quantitative approach, non-experimental design, and descriptive level. The sample consisted of 180 first-time pregnant adolescents attending the outpatient clinics of the National Maternal and Perinatal Institute, using census sampling. Data collection was done via survey and documentary analysis, with a questionnaire used to identify factors associated with anemia. According to the results, the anemia level in first-time pregnant women was moderate in 58.89% of the cases. The study concluded that sociodemographic, obstetric, and nutritional factors were most associated with anemia, according to 54.7% of the anemia cases found. It is recommended to inform the women about the consequences of anemia and emphasize the importance of maintaining a nutritious and healthy diet.

    SARS-CoV-2 infection spreads between individuals through emitted aerosols. The objectives are to determine the relationship between hemoglobin and erythrocyte distribution width with different degrees of severity of COVID-19 at Carlos Lanfranco Hospital in 2020; to determine hemoglobin levels and erythrocyte distribution width in COVID-19 patients, and to determine the severity proportion in COVID-19 patients. Data collection will occur in the hematology area of the laboratory, reviewing clinical records of patients admitted for COVID-19. Hemoglobin levels and erythrocyte distribution width will be collected, along with age, sex, and severity level. Frequency analysis will be performed for COVID-19 positive cases between July and December 2020, along with statistical techniques including chi-square, t-test, and logistic regression. The EpiInfo statistical software will be used.

    Currently, in Peru, the prevalence of anemia remains high, according to the latest demographic and health surveys conducted by the National Institute of Statistics and Informatics. Anemia continues to be one of the most common diseases in children, which is why the objective of this research is to determine the relationship between iron-deficiency anemia and risk factors in children under 5 years old at the Unidad Vecinal No. 3 health center during 2023. The study follows a hypothetical-deductive research method, aiming to test a hypothesis, and later obtain conclusions based on findings. The study has a quantitative approach based on statistical measurement and analysis, allowing the construction of theories. It is applied research, focusing on identifying how scientific knowledge can address or solve a specific practical need. The research design is observational, as the study will be conducted without manipulating variables, and data will be analyzed from phenomena in their natural context. Additionally, it is cross-sectional with a descriptive correlational focus, as data collection occurs at a single point in time.

    Of the total population diagnosed with Acute Myeloid Leukemia treated at the National Police Hospital Complex of Peru, Luis N. Sáenz, we found leukocytosis in 75 (80.64%). Conclusion: Males have a higher likelihood of suffering from Acute Myeloid Leukemia. The most common leukocyte alteration in patients with Acute Myeloid Leukemia was blasts, with the highest percentage in the 20-50% blast group (86.02%).
