(Universidad Privada Norbert Wiener, 2021-12-10) Ramón Villanueva, Cinthia Jackeline; Juárez Moreyra, Enrique
Introduction: The inappropriate use of antimicrobials is generating an increase in antimicrobial resistance, which has become a global problem. Objective: To determine the consumption and cost of antimicrobials with special usage considerations in the 'San Pedro' internal medicine ward of the Dos de Mayo National Hospital in 2019. Methodology: This was a descriptive, observational, cross-sectional, and retrospective study. A documentary analysis technique was used for data collection, with information extracted from the pharmacy department's computer system. The ATC/DDD methodology was used to calculate the DDD/100 bed-days to represent antimicrobial consumption, and an arithmetic calculation was performed for the cost. Results: 13 antimicrobials were identified, of which meropenem had the highest demand, representing 35.70% of the annual total, while linezolid had the lowest turnover at 0.03%. The most consumed antimicrobial was meropenem, which had a DDD/100 bed-days of 10.81, followed by vancomycin with 9.27 DDD/100 bed-days. The total annual cost was 110.02 thousand soles. Conclusions: The consumption of 13 antimicrobials was determined, with meropenem being the most consumed and generating the highest total cost of 59.51 thousand soles.

Estudio de utilización de antimicrobianos restringidos en apendicitis aguda en el servicio de Cirugía Pediátrica del Hospital San Bartolomé durante el periodo de enero a julio de 2010
(Universidad Privada Norbert Wiener, 2013) Juárez Moreyra, Enrique; Salazar Tuanama, Rita Haydee
Acute appendicitis is the inflammation of the cecal or vermiform appendix, which begins with obstruction of the appendiceal lumen, resulting in increased intraluminal pressure due to mucus accumulation associated with the low elasticity of the serosa. Clinically, it is divided into complicated appendicitis: perforated, gangrenous appendix, or periapendicular abscess with subsequent compromise of adjacent structures; and uncomplicated appendicitis: absence of perforation, gangrene, or periapendicular abscess. The prophylactic use of antimicrobials directly reduces hospital stay by providing adequate protection to the patient with a single dose of medication, avoiding the prolonged intrahospital stay implied by extended treatment regimens, and also reduces the incidence of infectious complications, resulting in shorter hospitalization. In antimicrobial treatment, cephalosporins (18.8%), aminoglycosides (27.2%), nitroimidazoles (30.4%), lincosamides (15.2%), amphenicols (7.9%), and penicillins (0.5%) were used. The most commonly used antimicrobial was metronidazole (costing 91.76 nuevos soles), representing 30.4% of the total antimicrobials. The most frequently used restricted antimicrobials were metronidazole 500 mg (ampoule), cefotaxime 500 mg (ampoule), and ceftazidime 1 g (ampoule). An evaluation was made of the different antimicrobials in terms of cost and quantities. Analytical monitoring of leukocyte levels was carried out to determine inflammation and diagnose the patient with acute appendicitis. Out of a total of 77 pediatric patients treated for acute appendicitis at the San Bartolomé Hospital from January to June 2010, distributed by sex (41 male patients and 36 female), it was found that the most affected age group was 6 to 10 years, representing 46% of cases.

Reticencia a la vacunación COVID-19 en pobladores del asentamiento humano Lomas de Cajamarca del distrito de Comas-Lima Metropolitana, 2021-2022
(Universidad Privada Norbert Wiener, 2022-10-04) Cubas Fernández, Yolanda; Morales Granados, Jessica Odil; Juárez Moreyra, Enrique
Vaccine hesitancy is a term that defines the attitude of rejection or delay in accepting vaccines. In the context of the pandemic, the speed of vaccine development and implementation may have caused insecurity and fear of adverse effects, leading to hesitancy toward the COVID-19 vaccine and potentially hindering effective pandemic control levels. Objective: To determine the main factors contributing to COVID-19 vaccination hesitancy among residents of the Lomas de Cajamarca settlement in the Comas district, Metropolitan Lima, 2021–2022. Methodology: An analytical, descriptive, observational, and cross-sectional study was conducted among a population of 313 adults aged 18 and older through an in-person survey. Results: Cross-tabulations in IBM SPSS V.25 statistical software and chi-square tests identified the main factors contributing to COVID-19 vaccination hesitancy: the perception of insufficient testing to demonstrate vaccine safety (47.2%), fear of adverse effects (41.7%), and the perception of a low risk of complications from COVID-19. The groups with the highest levels of hesitancy included Evangelicals (p-value: 0.000), individuals without formal education (p-value: 0.001), adults aged 60 and older (p-value: 0.023), and those without income (p-value: 0.019). Conclusion: 11.5% (n=36) of residents were not vaccinated due to perceptions of vaccine insecurity, low perceived risk of contracting COVID-19, and misinformation.
