    Pharmacovigilance is a discipline that ensures patient safety related to medication use through a set of systemic processes. However, in Peru, adverse drug reaction (ADR) reporting activities in hospitals are very limited due to a lack of knowledge about pharmacovigilance. Objective: To establish the relationship between the level of knowledge of pharmacovigilance and the practice of ADR reporting among healthcare personnel at the Sandia Hospital in 2021. Methods: This was a descriptive, correlational, prospective, and cross-sectional study with a quantitative and basic approach. A questionnaire was used for data collection, and a representative sample of 80 professionals was included. Results: The sample consisted of 12.5% doctors, 31.3% nurses, 12.5% obstetricians, 28.8% nursing technicians, and 2.5% pharmaceutical chemists and dentists, with a predominance of females at 65%. The most common age range and years of experience were 31–40 years and 1–5 years, both with 37.5%. The level of knowledge of pharmacovigilance was regular (76.3%), while the practice of ADR reporting was low (56.3%). The p-value was 0.023. Conclusion: The variables are significantly related.
