Our study was conducted directly in the hospital setting. Patients with chronic diseases often require the use of multiple clinically prescribed medications, resulting in polypharmacy. Thus, after observing these events, it was deemed appropriate to conduct the following research to detect the most relevant potential drug interactions in hospitalized patients. This will help avoid negative clinical outcomes and improve drug administration, prioritizing the patient's health and life. The study methodology was prospective, descriptive, and cross-sectional. Data collection was performed using the Uptodate®-Lexicomp® and DrugInteractionChecker clinical evidence databases to detect drug interactions. In conclusion, it was determined that 97.8% of potential drug interactions exist.
