In this critical review, titled “Effect of the Ketogenic Diet on Seizure Reduction in Children with Drug-Resistant Epilepsy,” the aim was to assess the effectiveness and tolerance of two dietary approaches: the Modified Atkins Diet (MAD) and the traditional Ketogenic Diet (KD). The main clinical question was: Will the ketogenic diet, whether in its classic or modified version, have an effect on seizure reduction in children of both sexes with refractory epilepsy? An Evidence-Based Nutrition (EBN) criterion was employed. 25 scientific articles were identified, and 12 relevant studies were selected for analysis, evaluated using the CASPE critical reading tool. Finally, a randomized clinical trial titled “Modified Atkins Diet versus Ketogenic Diet in Children with Drug-Resistant Epilepsy: A Meta-Analysis” was chosen, considered relevant due to its level of evidence I and strong recommendation grade, according to the author's experience. This study provided promising results by demonstrating that both ketogenic nutritional therapy, in its classic (KD) and modified (MAD) versions, significantly reduced seizures and/or achieved seizure freedom. The adverse effects associated with both therapies can be managed through careful monitoring and follow-up. Both treatments showed acceptable tolerance and can be considered as options within the framework of nutritional intervention for these patients.
