Publicación Acceso abierto Errores de prescripción en recetas médicas atendidas en la farmacia hospitalaria del área de emergencia adulto de la Clínica Internacional sede San Borja durante el periodo noviembre–diciembre de 2018(Universidad Privada Norbert Wiener, 2023-02-23) Rafael Ortiz, Zaida Olivia; Caldas Herrera, EmmaThis research aims to assess the errors in prescription orders received at the hospital pharmacy of the adult emergency service at Clínica Internacional San Borja during the last two months of 2018. Through an observational, descriptive, retrospective, and cross-sectional study comprising a sample of 386 prescriptions, the analysis and results showed an error rate of over 90%. It was also observed that the main errors related to physician information were the expiration date on the prescription (94.3%, 364 cases) and illegible handwriting (71.5%, 276 cases). Regarding patient data, the error rate ranged between 2.3% and 2.8%, with a reliability index of nearly 96%. The most significant failure was associated with patient information, specifically personal data, with the CIE-10 code showing 59.3% (229 cases). Between 52.3% and 91.5% of orders had deficiencies in medication identification, with a 95% error rate. The main errors were the route of administration (91.5%, 353 cases), international nonproprietary name (DCI) (72.3%, 279 cases), duration (68.4%, 264 cases), and concentration (52.3%, 202 cases), respectively
