    Ahorro económico por empleo del sistema de dispensación en dosis unitaria en el servicio de Cuidados Intensivos del Hospital de Emergencias Pediátricas, Lima 2020

    The objective was to determine the economic savings generated by using the Unit Dose Dispensing System (UDDS) in the Intensive Care Unit of the Pediatric Emergency Hospital. Method: The study was basic, with a quantitative, correlational descriptive, retrospective, and cross-sectional design. The sampling was non-probabilistic convenience sampling. The sample included 316 medication and/or device requests during the last quarter of 2019. Data were collected using an analysis matrix validated by expert judgment, based on data obtained from the pharmacy service's information system at the Pediatric Emergency Hospital, Lima, Peru. Results: Of the 316 requests (122 from October, 143 from November, and 51 from December 2019), the average cost per medication request was S/ 43.76 soles, and for medical device requests, it was S/ 66.06 soles. The average economic savings per returned medication request were S/ 9.49 soles due to suspension or treatment change and S/ 10.52 soles due to discharge or death. The savings for medical devices were S/ 31.82 soles for suspension or treatment change and S/ 51.54 soles for discharge or death. Conclusion: The economic savings generated by using the UDDS in the Intensive Care Unit of the Pediatric Emergency Hospital during the last quarter of 2019 were 14% for medications and 26% for medical devices.

    Comparacion de tres metodos coproparasitoscopicos para el diagnóstico de parasitosis intestinal en niños de 4 a 11 años del colegio Virgen Del Rosario, distrito de Ventanilla – 2018

    In Peru, enteroparasitosis appears to be distributed according to geographical regions, with various studies showing a predominance of helminths in the jungle and protozoa on the coast and in the highlands. The objective was to compare three coproparasitological methods for diagnosing intestinal parasitosis in children aged 4 to 11 at the Virgen del Rosario School in the Ventanilla District in 2018. Materials and methods: The research was descriptive, observational, cross-sectional, and prospective. A total of 100 fecal samples were analyzed using the Direct Method, the Spontaneous Sedimentation in Tube Technique (SSTT), and the Sheather Technique. Results: The study evidenced polyparasitic infections by protozoa: Entamoeba coli, Endolimax nana, Iodamoeba butschilii, and Giardia lamblia (the latter being the most prevalent at 33%). No helminths were found. The SSTT showed good performance with high sensitivity and specificity for Giardia lamblia, Iodamoeba butschilii, and Entamoeba coli (100%), and for Endolimax nana (92.2% and 100%, respectively). Conclusions: It was demonstrated that the Spontaneous Sedimentation in Tube Technique (SSTT) was highly sensitive and specific as a diagnostic test for determining enteroparasitosis, suggesting it could become a useful alternative in developing countries like ours.

    Complicaciones metabólicas de la nutrición parenteral en pacientes de la Unidad de Cuidados Intensivos del Hospital de Emergencia Grau. Enero – agosto. 2018

    The primary objective of this research study was to determine the metabolic complications of parenteral nutrition in patients in the Intensive Care Unit of the Grau Emergency Hospital, from January to August 2018. The study used a quantitative, observational, descriptive, cross-sectional, and retrospective methodology. Where the results were obtained in 20 patients who met the inclusion criteria, where the sociodemographic characteristics were presented in 60% of the age group from 60 to 69 of the total population, the most prevalent diagnosis being abdominal sepsis, of which the male sex was the one that presented the most metabolic complications (15.0%) and female (10.0%) and without complications of male sex 50.0% and female 25.0%, with hyperglycemia 98.0%, hypertriglyceridemia 71%, cholestasis in which bilirubin was found 32.3%, FA 58.1%, GGT 3.3%, TGO 9.8%, and TGP 7.2% who received PN for more than 14 days, presented 27 more frequent metabolic complications such as hypophosphatemia, hyponatremia, hyperkalemia and Hypomagnesemia, which concluded that 90.0% of metabolic complications were uneventful and 10.0% were problematic in intensive care patients receiving parenteral nutrition. Hypophosphatemia, hyponatremia, and hyperkalemia were the most common metabolic complications, with 55.56%, 22.20%, and 14.82% of patients, while hypomagnesemia was less common (7.42%).

    Cuantificación de partículas extrañas en las mezclas de nutrición parenteral sistema binario en la Central de Elaboración de Medical Nutrición. Lima, 2016

    The objective of this research, titled "Quantification of Foreign Particles in Binary System Parenteral Nutrition Admixtures at the Medical Nutrición Compounding Center, Lima, 2016," was to determine the quantification of foreign particles in binary system parenteral nutrition (PN) admixtures at the Medical Nutrición compounding center in Lima in 2016. The study was observational, descriptive, prospective, and cross-sectional. A data registration form was used. The method employed for the determination of the number and size of foreign particles was the Microscopic Particle Count Test (Method 2). Results: The average quantity of foreign particles was 0.695 particles per mL for particles >10 µm, and 0.235 particles per mL for particles >25 µm. None of the samples (0.0%) exceeded the established limits. Conclusion: The product complies with the parameters established by USP 39 Chapter 788 related to the number and size of particles in injectables.

    Determinación de calcio y fósforo en mujeres post menopáusicas como factor de riesgo de osteopenia en el asentamiento humano “Jaime Yoshiyama” del distrito de Ventanilla, Lima – Perú, en el período de abril 2018–diciembre 2019

    Osteoporosis primarily affects postmenopausal women, mainly those between 50 and 60 years old, manifesting as deterioration of bone tissue and bone structure. The study determined the levels of calcium and phosphorus in the blood. A descriptive, cross-sectional study was conducted using the colorimetric method on a sample of 100 apparently healthy women. Results: The analysis of the samples showed that 62% had decreased phosphorus levels and 61% had decreased calcium levels, with a 95% confidence level. This study reveals that low serum levels of calcium and phosphorus are independent risk factors for the development of osteopenia, especially during the menopausal period.

    Estudio de utilización de antimicrobianos restringidos en apendicitis aguda en el servicio de Cirugía Pediátrica del Hospital San Bartolomé durante el periodo de enero a julio de 2010

    Acute appendicitis is the inflammation of the cecal or vermiform appendix, which begins with obstruction of the appendiceal lumen, resulting in increased intraluminal pressure due to mucus accumulation associated with the low elasticity of the serosa. Clinically, it is divided into complicated appendicitis: perforated, gangrenous appendix, or periapendicular abscess with subsequent compromise of adjacent structures; and uncomplicated appendicitis: absence of perforation, gangrene, or periapendicular abscess. The prophylactic use of antimicrobials directly reduces hospital stay by providing adequate protection to the patient with a single dose of medication, avoiding the prolonged intrahospital stay implied by extended treatment regimens, and also reduces the incidence of infectious complications, resulting in shorter hospitalization. In antimicrobial treatment, cephalosporins (18.8%), aminoglycosides (27.2%), nitroimidazoles (30.4%), lincosamides (15.2%), amphenicols (7.9%), and penicillins (0.5%) were used. The most commonly used antimicrobial was metronidazole (costing 91.76 nuevos soles), representing 30.4% of the total antimicrobials. The most frequently used restricted antimicrobials were metronidazole 500 mg (ampoule), cefotaxime 500 mg (ampoule), and ceftazidime 1 g (ampoule). An evaluation was made of the different antimicrobials in terms of cost and quantities. Analytical monitoring of leukocyte levels was carried out to determine inflammation and diagnose the patient with acute appendicitis. Out of a total of 77 pediatric patients treated for acute appendicitis at the San Bartolomé Hospital from January to June 2010, distributed by sex (41 male patients and 36 female), it was found that the most affected age group was 6 to 10 years, representing 46% of cases.

    Intervenciones farmacéuticas en la validación de formulaciones de nutrición parenteral emitida por la Unidad de Cuidados Intensivos Neonatales del Hospital Nacional Guillermo Almenara Irigoyen en el Periodo Agosto 2014 a Enero 2015

    Objective: To verify whether parenteral nutrition formulations issued by the neonatal intensive care unit meet the nutritional requirements, according to the recommendations of the American Society for Parenteral and Enteral Nutrition (ASPEN) and the European Society of Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) guidelines. Method: The research design was observational, descriptive and retrospective, conducted over a period of six months. The study sample was determined using a statistical equation and 55 prescriptions were randomly selected per month. The 330 selected prescriptions were reviewed and validated, verifying the data: name, weight, diagnoses, route of administration and the values ​​of the formulation components, comparing them with the nutritional requirement tables defined according to the international guidelines of ASPEN and ESPGHAN for neonates. The collected data was transferred to an Excel file, then this database was exported to an SPSS version 24.0 file for analysis. The statistical analysis consisted of the preparation of simple and double-entry frequency tables, and descriptive statistics were calculated: mean, standard deviation with a 95% confidence interval. Results: The highest percentage of errors was found in macronutrient requirements, with 5.2% with values ​​above the requirement, followed by micronutrients with 2.6% and electrolytes with 1.7%. Conclusion: The formulations issued during the study period presented the highest percentage of values ​​above the requirement for macronutrients, micronutrients, and electrolytes with 5.2%, 2.6%, and 1.7%, respectively.

    Principales indicaciones del uso de la nutrición parenteral en las clínicas y hospitales realizadas en la Central de Mezclas Medical Nutrición, enero–junio. Lima, 2017

    The present research work entitled “Main indications for the use of parenteral nutrition in clinics and hospitals prepared at the Central Mixing Unit Medical Nutrition, January – June, Lima 2017”. The main objective was to determine the principal indications for the use of parenteral nutrition in clinics and hospitals prepared at the Central Mixing Unit Medical Nutrition, from January to June, Lima 2017. The study was descriptive, cross-sectional, and retrospective, based on a review of medical prescriptions. The variables were underlying diagnosis and types of parenteral nutrition. Results: out of 425 patients (100%) who received parenteral nutrition, 241 were neonates (56.7%), 134 were adults (31.5%), and 50 were pediatric patients (11.8%). Their main indication in neonates was prematurity (49%), with parenteral nutrition 2:1 plus lipids in 88%, the most frequent age group being 10–13 days (22%). The main indication in pediatric patients was postoperative care (50%), with parenteral nutrition 2:1 plus lipids (54%), their age group being 2–33 months (60%). The main indication in adults was postoperative care (41%), with parenteral nutrition 3:1 (61%), their age group being 57–65 years (22%). Conclusion: the most relevant indications were prematurity and, in second place, postoperative patients who could not use the enteral route.
