Methodology: A descriptive, cross-sectional, retrospective, and observational design was applied to assess competencies, skills, and attitudes. Two instruments were developed: a questionnaire to evaluate the knowledge and attitude of the Technical Directors, and a technical data sheet to assess skills and the organoleptic evaluation of pharmaceutical specialty medicines. The study included a total of 257 community pharmacies (pharmacies and drugstores). Data analysis was performed using the statistical software SPSS, version 25. Results: Of the 257 (100%) respondents, the largest group corresponded to the medium knowledge level with 64.59% (166), followed by the high knowledge level with 20.22% (53), and the low knowledge level with 14.79% (38). Regarding skills, the group with low skills predominated with 45.14% (116), followed by medium-level skills with 38.52% (99), and high-level skills with 16.34% (42). In terms of attitude, the group with low-level attitudes was the most frequent with 38.91% (100), followed by medium attitudes with 35.80% (92), and high attitudes with 25.29% (65). Conclusion: Among the 257 Technical Directors, the predominant level of knowledge was medium in relation to the organoleptic detection of counterfeit medicines. In contrast, both skills and attitudes were predominantly at a low level.
