The objective of this research was to: evaluate medical prescriptions and drug interactions in the Pharmaceutical Office of the IAFAS-EP at the Central Military Hospital, Lima, 2024. Methodology: Descriptive, quantitative, non-experimental, and cross-sectional study. The sample consisted of 378 medical prescriptions from hospitalized patients, selected through simple random sampling. Data collection forms were used to identify prescription errors and drug interactions. Results: None of the prescriptions met all 11 evaluated indicators, although 81.2% achieved compliance with 10 of them. Patient data showed 100% compliance, while the prescriber’s expiration date registered 0%. Regarding medication data, 81.2% were complete. A total of 23% of prescriptions presented errors, with the most frequent being dosage concentration errors (10.8%). A total of 502 interactions were identified in 49.5% of the prescriptions, predominantly pharmacodynamic (78.1%) and classified as severe (70.3%). Conclusion: No prescription fully met the established indicators, and nearly half presented severe pharmacodynamic interactions. The findings highlight the need to strengthen pharmaceutical validation and medical pharmaceutical coordination to optimize therapeutic safety in the hospital.
