Ítem Acceso abierto Critical review: use of megestrol acetate as single or combined therapy in patients with anorexia-cachexia syndrome and its impact on the patient's nutritional status(Universidad Privada Norbert Wiener, 2023-10-04) Ballen Castañeda, Daniela; Bohórquez Medina, Sofia LorenaMegestrol acetate is the most studied and used drug in patients with anorexia-cachexia syndrome. Therefore, the secondary research titled "Critical Review: Use of Megestrol Acetate as a Single or Combined Therapy in Patients with Anorexia-Cachexia Syndrome and Its Impact on Nutritional Status" aimed to identify the impact of megestrol acetate on the nutritional status of patients with anorexia-cachexia syndrome. The clinical question was: Does the use of megestrol acetate as a single or combined therapy in patients with anorexia-cachexia syndrome improve their nutritional status in indicators such as weight loss, food intake, appetite, functional capacity, and muscle strength? The Evidence-Based Nutrition (EBN) methodology was applied, and a search was conducted in databases like PUBMED, MEDLINE, SCIELO, SCOPUS, and SCIENCE DIRECT. As a result, 32 articles were found, of which 13 were selected and evaluated using the CASPe tool. For this critical review, the chosen systematic review and network meta-analysis titled "Efficacy and Safety of Pharmacological Interventions for Cachexia: A Systematic Review and Network Meta-Analysis" was selected, which has an A1-level of evidence and strong recommendation grade. The review concluded that megestrol acetate improves weight and appetite with doses of at least 400mg/day and treatment of at least 8 weeks, with moderate to low evidence quality, and there is no conclusive evidence regarding its impact on other clinical outcomes, such as functional capacity, muscle strength, or broader and possibly more relevant results, such as quality of life and survival.
