In children, it is the parents who are responsible for their children's overall health care, and the oral cavity is a part of it. Objective: To determine the cognitive level of parents about toothpaste and its relationship to the prevention of oral health in their children from the Juana Marlene Uribe Romero school in the district of San Martín de Porres. Methodology: The approach was quantitative, as numerical data was collected using a Likert scale. The design was basic and non-experimental because the study variables were not manipulated. Surveys were applied to 70 parents to collect data on the cognitive level and prevention, while an observation form based on the ICDAS and IHOS was used for oral health. Results: The results showed that the cognitive level of parents is significantly positively related to prevention (0.003 < 0.05 and r = 0.515, moderate) and oral health (0.002 < 0.05 and r = 0.613, high). Additionally, the cognitive level of parents regarding toothpaste was 31% very good and 40% good. For oral prevention in children, 37% was good and 44% was average, and for children's oral health, 63% was good and 4% was average. Conclusion: There is a significant positive relationship between the cognitive level of parents about toothpaste and the prevention of oral health in children from the Juana Marlene Uribe Romero primary school in San Martín de Porres, 2022, with a moderate relationship in prevention (r = 0.515) and a high relationship in oral health (r = 0.613)
