    Análisis de prescripciones médicas durante la pandemia de COVID-19, Lima - 2021

    Objective: To determine the characteristics of compliance with Good Prescribing Practices (GPP) and analyze the results of the drug utilization study in medical prescriptions dispensed between July and December 2020. Method: The method used was deductive, a basic type, with a quantitative approach, and a non-experimental, descriptive, observational, retrospective design. This was a Quantitative Investigation. 106 medical prescriptions captured in different pharmaceutical establishments in Metropolitan Lima were analyzed. Results: The analysis showed an 82.0% compliance rate with GPP. Broken down by non-compliance criteria, the most prominent omissions were the consignment of the disease diagnosis (62.4% of prescriptions) and illegibility (19.8%). Patient characteristics detailed an average age of 51.95 years. Regarding patient sex, 53.0% of prescriptions belonged to males. The medical specialty that prescribed the most was general medicine (24.5%). The medications with the highest prescription rates were Azithromycin and Paracetamol (9.1%) and Ivermectin (8.5%). The largest percentage of prescriptions came from the public sector (42.5%) and the private sector (33%). The quantity of medications per prescription showed 5 or more medications in 6.3% of cases and 4 medications in 6.1% of cases. The most prescribed dosage form was tablets (49.0%). Conclusion: This study determined that there is an 82.0% compliance rate with Good Prescribing Practices in medical prescriptions prescribed during the COVID-19 pandemic.

    Buenas prácticas de dispensación y su relación con la seguridad del paciente en el Hospital Regional Hermilio Valdizán Medrano – 2026

    La dispensación de medicamentos es una de las etapas más importantes dentro de la atención farmacéutica hospitalaria, ya que influye directamente en el cuidado de la persona y en la prevención de eventos asociados al medicamento. El uso correcto de estas “Prácticas de Dispensación” es un elemento clave en la atención de salud, especialmente en hospitales regionales de referencia. “Objetivo”: Determinar la relación entre las “buenas prácticas de dispensación” en la seguridad del paciente del “Hospital Regional Hermilio Valdizán Medrano” en el año 2026. “Materiales y métodos”: Investigación no experimental, correlacional y cuantitativa con un enfoque transversal, con población censal de 62 trabajadores del servicio farmacéutico. Se empleó un cuestionario tipo Likert validado por expertos y se analizarán los datos con estadística de descripción y correlación de Spearman (p < 0,05), a fin de establecer el vínculo entre las prácticas de dispensación y la seguridad del paciente. Resultados: Donde se observa que en la variable de Buenas prácticas de dispensación se encuentra en un nivel alto con el 90.3% y en la variable de la Seguridad del paciente en un nivel buenos con el 67.7%. Concluyendo: Que, si existe una relación de manera relevante entre las “buenas prácticas de dispensación” y la “seguridad del paciente” en el Hospital Regional Hermilio Valdizán Medrano durante el año 2026, a través de un coeficiente de Rho= 0.666 y una significancia de p= 0.000, rechazando la hipótesis nula y aceptando la alterna.

    Cumplimiento de las buenas prácticas de prescripción de recetas atendidas a pacientes ambulatorios en un instituto pediátrico Lima – Perú enero a marzo 2021

    The research methodology was a basic, non-experimental, quantitative study with probabilistic sampling. The sample consisted of 364 medical prescriptions from outpatient patients who obtained medication from the emergency pharmacy and met the inclusion criteria. An 18-item data collection form was developed to gather information. Data processing was performed using Excel 2010 and SPSS version 23. It was determined that 100% of the prescriptions had some deficiency or omission. The compliance percentages for good prescribing practices across different dimensions were: patient data, 83.8%; medication data, 74.7%; prescription validity, 1.9%; and prescription legibility, 52.2% (with 47.8% non-compliance). The research concluded that the results demonstrate that the prescriptions reviewed do not meet the requirements of current regulations, which has a negative impact on the treatment and health recovery of the pediatric patient.

    Errores de expendio de medicamentos del técnico en farmacia por la ausencia del químico farmacéutico en los establecimientos farmacéuticos del distrito de San Juan de Lurigancho, Lima 2020

    The objective of this study was to identify dispensing errors made by pharmacy technicians due to the absence of a Pharmaceutical Chemist in pharmaceutical establishments in the district of San Juan de Lurigancho. The study was descriptive with a correlational level, considering inclusion and exclusion criteria. Chi-square was used to evaluate the probability of a discrepancy equal to or greater than that existing in the data, along with a Pareto diagram. A total of 385 users from the areas of Las Flores and Zarate were surveyed. The absence of the Pharmaceutical Chemist was identified in 32.2% of cases, primarily due to personal reasons (59.4%) and scheduling issues (17.6%). Regarding dispensing errors, 19.7% failed to indicate frequency and dosage, 19.5% failed to explain proper storage methods, 19.3% failed to advise checking expiration dates, and 18.0% failed to emphasize following the doctor's instructions. The Pareto diagram identified these four main errors, which accounted for 76.5% of all errors. Additionally, it was estimated that when the Pharmaceutical Chemist is present, 12.9% fail to provide the correct pharmaceutical form, while this percentage increases to 34.9% in their absence (p-value = 0.000), indicating a significant difference in the proportion of this error when the Pharmaceutical Chemist is absent. Conclusion: The absence of the Pharmaceutical Chemist contributes to dispensing errors, which can further deteriorate the quality of life of patients.

    Errores de prescripción de recetas atendidas en una oficina farmacéutica de Inkafarma San Juan 1. Lima 2021

    The irrational use of medicines is a global problem due to medication errors, with prescribing being the most common error. The objective of this research was to evaluate prescribing errors in prescriptions filled at an Inkafarma pharmaceutical office in San Juan 1, Lima, in 2021. Methodology: The study was applied type, with a deductive method, and an observational, prospective, cross-sectional design. The population consisted of 2,000 prescriptions, of which 322 were analyzed. A data collection form was used as the instrument, covering four dimensions: prescriber data, patient data, medication data, and prescription legibility, all of which must comply with the manual of good prescribing practices. Results: In the patient data dimension, 61.8% were non-compliant. In the medication data dimension, 69.3% were non-compliant. In the prescriber data dimension, there was a minimum error of 0.9%, and for legibility, 7.1% were non-compliant. Conclusion: All of the analyzed prescriptions presented at least one error in their dimensions. Patient data had the highest overall error rate, with the INN (International Nonproprietary Name) indicator specifically having 20.5% of errors.

    Errores de prescripción en las recetas atendidas en la farmacia ambulatoria del Centro Médico Naval en el periodo enero a marzo del 2021. Lima 2021

    Medication errors account for approximately 16%, equivalent to 80,000 deaths per year. This problem is linked to multiple factors that converge to produce a severely negative impact on both the patient and the healthcare system. The present research aims to provide information on prescription errors so that healthcare facilities can implement protocols to help reduce the error rate. Objective: To evaluate prescription errors in the prescriptions processed at the outpatient pharmacy of the Naval Medical Center during the period from January to March 2021. Methodology: This is an applied study with a deductive method and an observational, retrospective, cross-sectional design. A data collection form was used to analyze a sample of 336 prescriptions from a population of 2,700 prescriptions processed at the outpatient pharmacy of the Naval Medical Center during the period from January to March 2021. The evaluation was carried out by analyzing 16 indicators grouped into four dimensions: patient data, prescriber data, medication data, and legibility. Results: The percentages of errors were 9.38%, 18.82%, 47.27%, and 38.99% for the dimensions of patient data, prescriber data, medication data, and legibility, respectively. Conclusion: The prescriptions processed at the outpatient pharmacy of the Naval Medical Center during the period from January to March 2021 presented errors in all prescriptions analyzed.

    Errores de prescripción en las recetas médicas atendidas en el servicio de farmacia del Policlínico Municipal de Santa Anita. Lima 2021

    Prescribing errors are frequent, leading to medication misuse and jeopardizing patients' health restoration. The objective of this study was to evaluate prescribing errors in medical prescriptions filled at the pharmacy service of the Santa Anita Municipal Polyclinic. For this, an observational, cross-sectional, and basic study was conducted. The sample consisted of 255 medical prescriptions, and a data collection form adapted by the researcher was used. The percentage of prescriptions with errors in prescriber data was 7.1%. The percentage of prescriptions with errors in patient data was 73.7%, with the most prominent being in the gender indicator (64.7%) and diagnosis (31%). The percentage of prescriptions with errors in medication data was 34.5%, with the most prominent being in the concentration (19.2%), the INN (International Nonproprietary Name) (16.5%), and the route of administration (9.4%). The percentage of prescriptions with errors in legibility was 7.5%. It is concluded that there is a high percentage of prescribing errors in the patient data dimension, which demonstrates a failure to complete all required data on the medical prescription.

    Errores de prescripción en recetas atendidas en farmacia central de pacientes hospitalizados del Hospital Regional Docente de Cajamarca, 2020

    The objective of this research was to determine prescription errors in prescriptions filled at the central pharmacy for hospitalized patients at the Regional Teaching Hospital of Cajamarca in 2020. METHODOLOGY: An inductive method was used to study the behavior of each dimension. The research approach was a quantitative analysis, with a basic study type and a descriptive level. The design was non-experimental, as the variable was not intentionally manipulated. The POPULATION consisted of 5,840 prescriptions filled from the Surgery, Medicine, Gynecology, and Pediatrics departments. The SAMPLE was 361 prescriptions, for which a systematic random start sampling method was used. Data was collected by reviewing the prescribed prescriptions. The technique used was a documentary analysis using a checklist. The instruments were validated by expert judgment, and SPSS Version 25.0 was used. Data analysis was performed by calculating the percentages and comparing them with the results from other national and international research to better understand the issue of prescription errors.

    Errores de prescripción en recetas médicas atendidas en Farmacia Universal sede Lima Cercado – 2020

    The difficulty in writing medical prescriptions is a worldwide problem. A prescription is a legal health document that includes pharmacological or non-pharmacological directions and must be legible. Timely error detection will prevent negative effects on patient health. The objective of this study was to determine prescribing errors in medical prescriptions filled at the Farmacia Universal, Cercado de Lima branch, in 2020. Methodology: The method was deductive-synthetic, with a quantitative approach. The research type was basic, with a descriptive research level and a descriptive, observational-cross-sectional design. The sample consisted of 359 medical prescriptions, and a checklist was used for data collection. Results: The results determined that 72.14% of evaluated prescriptions had prescribing errors. Specific errors were identified: illegible handwriting (33.1%); errors related to prescriber data (78.27%); errors related to patient information (97.8%); errors related to medication data (51.3%); and errors related to therapeutic indications (96.1%). Prescription dates were present in 72.7% and absent in 27.3%. Validity dates were present in 1.7% and absent in 98.3%. Conclusion: 72.14% of the evaluated prescriptions had prescribing errors.

    Errores de prescripción en recetas médicas atendidas en la farmacia hospitalaria del área de emergencia adulto de la Clínica Internacional sede San Borja durante el periodo noviembre–diciembre de 2018

    This research aims to assess the errors in prescription orders received at the hospital pharmacy of the adult emergency service at Clínica Internacional San Borja during the last two months of 2018. Through an observational, descriptive, retrospective, and cross-sectional study comprising a sample of 386 prescriptions, the analysis and results showed an error rate of over 90%. It was also observed that the main errors related to physician information were the expiration date on the prescription (94.3%, 364 cases) and illegible handwriting (71.5%, 276 cases). Regarding patient data, the error rate ranged between 2.3% and 2.8%, with a reliability index of nearly 96%. The most significant failure was associated with patient information, specifically personal data, with the CIE-10 code showing 59.3% (229 cases). Between 52.3% and 91.5% of orders had deficiencies in medication identification, with a 95% error rate. The main errors were the route of administration (91.5%, 353 cases), international nonproprietary name (DCI) (72.3%, 279 cases), duration (68.4%, 264 cases), and concentration (52.3%, 202 cases), respectively

    Errores de prescripción en recetas médicas del establecimiento SISOL Salud y consultorios particulares, atendidas en Boticas Perú distrito de Villa María del Triunfo. Lima 2021

    The objective of this research was to evaluate prescription errors in medical prescriptions from the SISOL SALUD facility and private clinics, filled at Boticas Perú in the district of Villa María del Triunfo. The study's methodology was deductive, with a quantitative approach and an applied, observational, cross-sectional, and retrospective design. Of 1,000 medical prescriptions collected from both the SISOL SALUD facility and private clinics, a sample of 517 was used for analysis based on the dimensions of "prescriber data," "patient data," "medication data," and "other prescription data." The results for the SISOL SALUD facility showed errors of 52.2% in prescriber data, 48.9% in patient data, 86.1% in medication data, and 98.9% in other prescription data. In private clinics, the results were 36.2% errors in prescriber data, 96.4% in patient data, 97.6% in medication data, and 98.9% in other prescription data. A comparison of medical prescription errors between the two institutions showed that private clinics had a higher average of errors with 36.1% of indicators presenting errors, compared to the SISOL SALUD facility's average of 25.8%. It was therefore concluded that private clinics have a greater number of errors in their prescriptions compared to the SISOL SALUD facility.

    Errores de prescripción y características de medicamentos en recetas atendidas de la oficina farmacéutica del distrito de Pillco Marca en Huánuco 2021

    The objective was to describe prescribing errors and medication characteristics in prescriptions filled at the pharmaceutical office in the Pillco Marca district, Huánuco, in 2021. Methods: A quantitative, descriptive, and cross-sectional approach was used. The technique was observation, and the instrument was a checklist applied to 357 prescriptions. The data were analyzed using descriptive statistics presented in tables and figures. Results: Compliance for patient data on prescriptions was 56.6%, for medication data it was 79.5%, and for prescriber data it was 63.3%. The predominant medication family was antibiotics (59.9%), with the most common route of administration being oral (53.8%) and the most frequent pharmaceutical form being syrup (36.4%). Conclusion: The most common prescribing errors identified were those with three errors per prescription, accounting for 26% of cases and involving patient, medication, and prescriber data.

    Errores de prescripción y dispensación de medicamentos a pacientes hospitalizados con COVID-19 en el Hospital Nacional Arzobispo Loayza, agosto–diciembre 2020

    Objective: To determine the relationship between medication prescribing and dispensing errors in hospitalized COVID-19 patients at the Arzobispo Loayza National Hospital, from August to December 2020. Materials and methods: This research is a basic study with a correlational design and a quantitative approach. The sample consisted of 377 medical prescriptions (therapeutic sheets and their respective care records) filled at the Hospitalization Pharmacy service. An observational and documentary analysis technique was used, applying a previously validated checklist as the instrument to include indicators for the two study variables. Results: Prescription errors were found in 17.5% of cases, with illegibility being the most frequent at 52.2%. Inadequate compliance with medication dispensing was 19.4%, with the reception and validation of the prescription showing the highest non-compliance at 34.8%. For the hypothesis test, a bilateral significance value of less than 0.05 was found (p=0.000), leading to the acceptance of the alternative hypothesis and the rejection of the null hypothesis. A strong positive correlation was also found (Phi Coefficient = 0.919). Conclusion: A significant relationship was determined to exist between medication prescribing and dispensing errors in hospitalized COVID-19 patients at the Arzobispo Loayza National Hospital from August to December 2020.

    Errores de prescripciones en recetas médicas de establecimientos farmacéuticos en el contexto del COVID-19 del distrito de Bellavista de la Provincia Constitucional del Callao, primer semestre 2021

    The general objective of this research was to evaluate prescribing errors in medical prescriptions from pharmaceutical establishments in the context of COVID-19 in the Bellavista district, Constitutional Province of Callao, during the first semester of 2021. The theoretical framework was based on the Manual of Good Prescribing Practices of the Ministry of Health (2005). The methodology used was an analytical method, with a quantitative approach and an applied type. The design was non-experimental, cross-sectional, and descriptive. The study population was unknown, so a probabilistic sampling with replacement for infinite populations was chosen. This resulted in the evaluation of 384 medical prescriptions using a data collection form that was validated by expert judgment. Results: It was found that 99.5% (n=382) of medical prescriptions did not comply with at least one of the data points required by the manual of good pharmaceutical practices. 80.7% (n=310) had errors related to patient data, 29.2% (n=112) had omissions in medication data, 43.2% (n=166) omitted prescriber data, and 96.1% (n=369) omitted information on medical indications. Conclusion: The high incidence of prescribing errors is due to the omission of key indicators such as prescription validity, patient age, medication route of administration, patient diagnosis, and the specialist's national registry number.

    Errores en la medicación de pacientes y grupo profesional de salud en el área de Medicina Interna de la Clínica Internacional sede San Borja de abril 2020–marzo 2021.Lima 2021

    Medication Errors (MEs) that occur during the medication utilization process and involve healthcare professionals in the hospital setting constitute a relevant clinical problem that poses a risk to patient safety, making preventative measures necessary. The objective of this study was to evaluate the relationship between patient MEs and the healthcare professional group in the internal medicine area of the Clínica Internacional, San Borja branch, from April 2020 to March 2021. The research was applied type, with an observational, cross-sectional design. A data collection form was used. The population was 2,446 Medical Prescriptions (MPs), and the sample was 333 randomly selected prescriptions, analyzed with a 95% confidence level. Results: The relationship between patient MEs and the healthcare professional group showed 5.7% in prescribing errors committed by the surgeon, followed by 4.2% in administration committed by the licensed nurse, and 1.8% in dispensing committed by the pharmacy assistant, pharmaceutical chemist assistant, and pharmacy technician. Conclusion: The most relevant relationship between patient MEs and the healthcare professional group in the internal medicine area of the Clínica Internacional, San Borja branch, was found in prescribing errors committed by surgeon doctors.

    Evaluación de las prescripciones médicas e interacciones medicamentosas en la oficina farmacéutica de la IAFAS-EP del Hospital Militar Central. Lima. 2024

    Prescription errors and drug interactions represent a significant risk to patient safety in hospital settings. The objective of this research was to: evaluate medical prescriptions and drug interactions in the Pharmaceutical Office of the IAFAS-EP at the Central Military Hospital, Lima, 2024. Methodology: Descriptive, quantitative, non-experimental, and cross-sectional study. The sample consisted of 378 medical prescriptions from hospitalized patients, selected through simple random sampling. Data collection forms were used to identify prescription errors and drug interactions. Results: None of the prescriptions met all 11 evaluated indicators, although 81.2% achieved compliance with 10 of them. Patient data showed 100% compliance, while the prescriber’s expiration date registered 0%. Regarding medication data, 81.2% were complete. A total of 23% of prescriptions presented errors, with the most frequent being dosage concentration errors (10.8%). A total of 502 interactions were identified in 49.5% of the prescriptions, predominantly pharmacodynamic (78.1%) and classified as severe (70.3%). Conclusion: No prescription fully met the established indicators, and nearly half presented severe pharmacodynamic interactions. The findings highlight the need to strengthen pharmaceutical validation and medical pharmaceutical coordination to optimize therapeutic safety in the hospital.

    Evaluación de recetas con errores de prescripción atendidas en farmacia de sala de operación del Hospital Cayetano Heredia, marzo–mayo 2021

    In many countries, including Peru, medication problems exist due to various types of prescribing errors. Hospitals, health centers, and clinics issue medical prescriptions daily, morning, noon, and night, with errors or omissions in the data, many of which unfortunately lead to medication errors. The objective of this study was to evaluate prescribing errors in prescriptions filled at the operating room pharmacy of the Cayetano Heredia Hospital from March to May 2021, using a descriptive non-experimental cross-sectional methodology, in which 528 prescriptions were evaluated. Results: 48% of prescriptions did comply with legible handwriting and prescription date, while 43% did not comply with these characteristics. Regarding the evaluation of medication-related data, 46% of prescriptions did contain the data, and 52% of prescriptions did not contain the data. The study concludes that nearly half of the medical prescriptions studied did not comply with legible handwriting or the prescription date, and that more than half of the prescriptions did not comply with providing the medication data.

    The present research aimed to evaluate the functioning of the Unit Dose Drug Distribution System (UDDDS) in the Pharmacy Service of Huaycán Hospital during the period September 2017 to February 2018. An observational, descriptive, and retrospective study was conducted, which included patients within the aforementioned period. Clinical records, pharmacotherapeutic follow-up logs, reports of dispensing errors within the UDDDS, reports of adverse drug reactions, records of drug returns, and statistical sources were reviewed. The number of patients under pharmacotherapeutic follow-up was identified, with the highest percentage recorded in September 2017 (3.7%). The determination of suspected adverse drug reactions showed the highest incidence (86.7%) in December 2017. The number of dispensing errors recorded was 0.96% during September, where most involved only one error per prescription detected by the UDDDS. Furthermore, 9.22% of patients under pharmacotherapeutic follow-up presented drug-related problems. During the study period, a cost saving of 5.37% was observed. It was concluded that the evaluation of the study population demonstrated reliability and completeness, making it possible to assess the functioning of the UDDDS.

    Factores asociados al error en la medicación en los servicios de urgencias y emergencias.

    Los errores de la medicación en los servicios de urgencias y emergencias significan la elevación de las cifras de mortalidad y un mayor gasto económico. Objetivo: Sistematizar las evidencias sobre los factores asociados al error en la medicación en los servicios de urgencias y emergencias. Material y Métodos: Revisión sistemática observacional y retrospectiva de tipo cuantitativa, la selección fue a través de evaluación de Grade. Se revisaron 10 estudios, el 40% corresponden al país de España, 10% al país de Argentina, 10% al país de México, 10% al país de Brasil, 10% al país de Taiwán, 10% al país de EU y 10% al país de Costa Rica. Se encontrados en las bases de datos: Cochrane, Bireme, Scielo, Pubmed, Medline, Elservier.; corresponden al tipo cuantitativo y de diseños de revisión sistemática, metanálisis y retrospectivos. Resultados: Hay una coincidencia de errores de la medicación como selección incorrecta de los medicamentos (10%), prescripción incompleta (30%), falta del cumplimiento terapéutico (20%), fallos en la administración (60%), transcripción (10%), dispensación (10%), sobrecarga laboral (30%) presentando mejoras con los registros informatizados (30%) y presencia del farmacéutico (10%). Conclusión: los errores de la medicación son frecuentes en los servicios de urgencias y emergencias, es necesario registros informatizados y una cultura de seguridad y calidad como elementos necesarios para la prevención.

    Factores que intervienen en el error en la administración de medicamentos de las enfermeras en el servicio de emergencia

    Objetivos: Determinar los Factores que intervienen en el Error en la Administración de Medicamentos de las Enfermeras en el Servicio de Emergencia. Materiales y Métodos: Las Revisiones Sistemáticas son un diseño de investigación observacional y retrospectivo, que sintetiza los resultados de múltiples investigaciones primarias. Son parte esencial de la enfermería basada en la evidencia por su rigurosa metodología, identificando los estudios relevantes para responder preguntas específicas de la práctica clínica. Resultados: Se seleccionaron 10 artículos, 20% corresponden a Irán, México y Colombia; 10% encontramos a Egipto, Brasil, España y Jordania respectivamente. Los artículos en un 90% (n=9/10) son de corte transversal, principalmente en los países de Irán, México, Colombia, Brasil, Egipto, Jordania y con un 10 % (n=1/10) a revisiones sistemáticas en España. El total del 100% (10) artículos revisados, el 60% (n=6/10) evidencia que el factor que influye en el error en la administración de medicamento está relacionado con el desconocimiento farmacológico, 30% (n= 3/10) evidencia que el factor que influye en el error en la administración de medicamentos es sobrecarga laboral y con un 10% (n=1/10) se evidencia que el factor que influye en el error en la administración de medicamentos es la comunicación enfermera – Medico. Conclusiones: Se evidencia que los factores que intervienen en el Error en la Administración de Medicamentos de las enfermeras son: desconocimiento farmacológico, sobrecarga laboral y comunicación enfermera - Medico. Los resultados proporcionan información valiosa que puede utilizarse para mejorar la administración de medicamentos contribuyendo a una atención de calidad haciendo énfasis en los factores de mayor prevalencia.
