The patient tested positive for SARS-CoV-2. Obstetric evaluation revealed FHR: 50 beats per minute ("sustained bradycardia") and the onset of labor. Laboratory tests found a hemoglobin level of 9.1 g/dl. The patient underwent an emergency cesarean section, resulting in a stillbirth: Female, weight: 3500 g, height: 49 cm, Apgar: 0-0. According to the surgical report, a pathological placenta was found with 70% fundal abruptio placentae and bloody amniotic fluid. The patient experienced uterine atony, prompting activation of the red code, uterine packing, and uterine artery ligation. She received two packed red blood cell transfusions. She was discharged in stable condition and transferred to the ICU, where she was monitored and given electrolyte replacement therapy. Once stable, she was discharged from the ICU on January 17 and transferred to the hospitalization ward for hemoglobin control. On January 18, a postoperative hemoglobin test showed 6.7 g/dl, leading to the administration of one packed red blood cell unit. By January 19, her post-transfusion hemoglobin level was 7.5 g/dl, stabilizing her anemia. The patient showed favorable evolution and was discharged on January 21.Ítem Acceso abierto Pregnant woman with uncontrolled chronic hypertension associated with advanced maternal age and hypothyroidism, complicated by fetal death, attended at the National Hospital Daniel Alcides Carrión 2017.(Universidad Privada Norbert Wiener, 2023-02-07) Caceres Rojas, Mariela Pilar; Cruz Stuart, Liz Janette; Torres Osorio, Juan MacedonioAn academic paper is presented that discusses the case of a multigravida at 19 weeks of gestation with uncontrolled chronic hypertension, advanced maternal age, hypothyroidism, and complicated by fetal death. Initially, she was attended to in the emergency department of the Peru-Korea Maternal Child Center (I-4), where she was referred to the Daniel Alcides Carrión National Hospital (III-1). After her blood pressure was controlled, she was discharged with instructions to begin prenatal care in the outpatient clinic. A few days later, the patient was treated at the Bahía Blanca health center (I-2) for a hypertensive crisis and was urgently transferred to the local Ventanilla hospital (II-1). After evaluation, she was once again referred to the Daniel Alcides Carrión National Hospital, where she required hospitalization in the Intensive Care Unit and multidisciplinary management due to the complications that arose from the hypertensive condition.Ítem Acceso abierto Preterm premature rupture of membranes associated with fetal death in pregnant women attended at María Auxiliadora Hospital - San Juan de Miraflores 2019(Universidad Privada Norbert Wiener, 2022-08-04) Diaz Tamariz, Cecilia Antonella; Sanz Ramirez, Ana MaríaMaterial and Methods: Observational, descriptive, retrospective study of clinical history review, clinical case of a pregnant woman with preterm pregnancy and non-stressful test with obvious signs of funicular compression, treated at the María Auxiliadora hospital in 2019. A review of literature for discussion of the clinical case. Results: The clinical case of a 35-year-old patient, without prenatal care, who went to the emergency service due to loss of amniotic fluid at 27 weeks of gestational age is presented. After 23 days of hospitalization, a non-stressful test is performed where the baseline is 162 beats per minute in 30 minutes of recording, variability in a range of 5 to 7 beats, compensatory accelerations, mild and severe variable decelerations, absence of uterine contractions. The pregnancy ends abdominally with fetal death. The patient progressed favorably and was discharged on the fourth postoperative day. Conclusion: Electronic fetal monitoring performed periodically in patients with premature rupture of membranes is a useful procedure for detecting signs of funicular compression.Ítem Acceso abierto Risk factors influencing preeclampsia cases at Hospital San Juan de Lurigancho, 2016(Universidad Privada Norbert Wiener, 1905-07-09) Cuenca Fernandez, Carmen Liseth; Ayala Peralta, Félix DasioIn the present study, a methodology is used to determine the sociodemographic, cultural, and economic characteristics of women over 18 years old who had an abortion at the Hipólito Unanue National Hospital between January and October of 2015. It was revealed that the sociodemographic characteristics include: early age, cohabiting marital status, secondary education level, currently working, being from the coastal region, residing in El Agustino, and experiencing psychological family pressure due to economic instability. Additionally, the cultural characteristics highlighted include: machismo, due to the lack of partner participation in household chores, a situation unfortunately justified by the respondents; and the Catholic religion, which most respondents profess. Finally, the most notable economic characteristics are: a monthly income between 600 and 1000 soles, monthly expenses less than 600 soles, three people living in the household, no economic dependency, two family members contributing to household expenses, and living in a brick-built house.
