Ítem Acceso abierto Effectiveness of laparoscopic distal gastrectomy compared to open distal gastrectomy in reducing intraoperative blood loss and hospital stay in patients with early gastric cancer(Universidad Privada Norbert Wiener, 2018-11-25) Ildefonso Alberto, Ximena Genoveva; Villa Collazos, Katherin; Rivera Lozada De Bonilla, OrianaMaterial and Methods: Systematic observational and retrospective quantitative review, subjected to critical selection, using the Grade evaluation system to identify the level of evidence, found in the following databases: Cochrane Library, British Medical Journal, PubMed, Lancet , Wiley Online Library, Journal of Rheumatology. The 10 articles reviewed systematically, 60% (n= 6/10) are Meta-analysis, 20% (n= 2/10) are cohort studies and 20% (n= 2/10) are randomized clinical trials. These studies come from the countries of China (50%), followed by Japan (10%), Australia (10%), Korea (10%), the USA (10%), and England (10%). Results: The evidence found, 100% (n=6/10) indicates that Laparoscopic Distal Gastrectomy is effective in reducing intraoperative blood loss and hospital stay in patients with early gastric cancer. Materials and Methods: The type of research was quantitative, high-quality articles reviewed from systematic reviews and meta-analysis, the population was 25 articles; The sample was 12 scientific articles published and indexed in the scientific database, the database search tool: Scielo Pubmed, Ebsco and World Journal of Gastroenterology. The GRADE method was used for evaluation, which evaluated the quality of evidence and the strength of recommendation. Results: Of the 100%, 32% of studies correspond to Japan, Brazil and the USA, 18% Italy and 50% China, in relation to the type it was 100% quantitative and study designs 41% correspond to meta-analysis, 17 % to systematic review, randomized and retrospective clinical trial and 8% to cohort. Conclusions: 92% of the articles reviewed show the effectiveness of laparoscopic gastrectomy, because it presents less blood loss, fewer complications, shorter hospital stay, fewer post-surgical complications, 8% show that both laparoscopic and conventional gastrectomy are equally effective. Effective, however, laparoscopic gastrectomy cannot be routinely recommended over the conventional technique for the treatment of advanced gastric cancer.Ítem Acceso abierto Level of anxiety in the preoperative period and its relationship with pain management in patients undergoing total gastrectomy in the general surgery service of a national hospital in Lima - 2023(Universidad Privada Norbert Wiener, 2023-12-03) Tapia Colca, Katerin Melchorita; Mori Castro, Jaime AlbertoPatients who are going to undergo surgery usually go through not so pleasant experiences, one of them anxiety and pain, these two are very important to know since this can prevent possible complications. The objective of this research project is to analyze the relationship between the level of anxiety in the preoperative period and pain management in patients undergoing total gastrectomy in the General Surgery Service of a National Hospital in Lima - 2023. This research has a quantitative approach, non-experimental design. There will be a population of 110 patients who are scheduled for total gastrectomy. The survey technique will be used and the Spielberger State-Trait Anxiety Inventory (STAI) will be applied to measure anxiety and the Visual Analogue Scale (VAS) to evaluate pain; Both instruments have been validated by expert judgment and the reliability of the Anxiety instrument was carried out by Mejía, in 2009 with a Crombach's alpha of 0.89; In the case of the Canorio and Cardenas VAS Scale in 2021, they determined a score of 0.96, in Crombach's alpha; thus determining that both tests are reliable. The results will be collected, processed, coded in the data matrix designed in Excel to be later presented in corresponding tables and graphs.
