    Effectiveness of erythromycin infusion before gastric endoscopy in preventing complications in patients with upper gastrointestinal bleeding

    Objective: To analyze the evidence on the effectiveness of erythromycin infusion before gastric endoscopy in preventing complications in patients with upper gastrointestinal bleeding. Materials and Methods: The type of research was quantitative, the study design was a systematic review, the population was 35 articles, and the sample was 10 scientific articles published and indexed in scientific databases, the instrument was a search based of data: Cochrane Library, Scielo British Medical Journal, PubMed, Lancet, Wiley Online Library and epistemonikos, to evaluate the articles the GRADE method was used which evaluated the quality of evidence and the strength of recommendation. Results: In the final selection, 10 articles were chosen, we found that 10% correspond to South Korea, 20% to Saudi Arabia, 20% France, 10% China, 10% Iran and 30% the USA. In relation to the designs and types of studies, the majority were 30% clinical trials, 30% systematic review, 20% meta-analysis, 10% cases and controls and 10% randomized trial. Conclusions: It is concluded that 9 of 10 articles show the effectiveness of erythromycin infusion because it significantly improves the visualization of the gastric mucosa, reduces the possibility of a second endoscopy and reduces the duration of the endoscopic procedure. And 1 of 10 articles do not show endoscopic visualization because it was not analyzed because disparate definitions between studies did not allow for meaningful clinical inferences.

    Effectiveness of nasogastric tubes in patients with gastrointestinal bleeding

    Objective: Systematize the evidence on the effectiveness of the use of the nasogastric tube in patients with gastrointestinal bleeding. Material and Method: the design used was a systematic review. The population was made up of 20 articles from indexed journals, which, through inclusion and exclusion criteria, determined a sample of 10 scientific articles. The GRADE system was used to evaluate the quality and strength of recommendation of the evidence. Results: From the review carried out, 40% (4/10) come from the United States, 20% (2/10) come from France, 10% (1/10) from Turkey, 10% (1/10) Spain , 10% (1/10) Japan and 10% (1/10) Israel. According to the study design, 30% (3/10) were systematic reviews, 20% (2/10) randomized clinical trials, 40% (4/10) observational cohort and 10% (1/10) quasi-experimental. According to the quality of evidence, 50% (5/10) were high and the other 50% (5/10) were moderate. Conclusions: There is significant evidence of the effectiveness of the use of the nasogastric tube in patients with digestive bleeding.

    Effectiveness of predictive assessment using the AIMS65 scale for treatment needs in patients with upper gastrointestinal bleeding in emergency services

    Objective: Systematize the evidence from studies regarding the effectiveness in the predictive assessment of the need for treatment of the AIMS65 scale in patients with upper gastrointestinal bleeding in emergency services. Material and Methods: A systematic review of international and national research is carried out, obtained through databases: Epistemonikos, NCBI, PubMed, Elsevier, Google Academic and Scielo, which have been evaluated through the grade system to determine the degree of evidence. . The search was carried out by applying DeCS and MeSH descriptors for the search in English and Spanish respectively. 60% (6/10) of the articles reviewed correspond to the United States, South Korea, and Peru, with 20% each respectively, and the remaining 40% (4/10) correspond to Australia, Pakistan, Qatar and Japan respectively. Results: 90% (09/10) of the evidence indicates the effectiveness in the predictive assessment of the need for treatment of the AIMS65 scale in patients with upper gastrointestinal bleeding in emergency services, and 10% (1/10) indicate the ineffectiveness of the predictive assessment of the need for treatment of the AIMS65 scale in patients with upper gastrointestinal bleeding in emergency services Conclusions: 9 of 10 evidence demonstrate the effectiveness in the predictive assessment of the need for treatment of the AIMS 65 scale in patients with upper gastrointestinal bleeding in emergency services, 1 of 10 evidence demonstrates non-effectiveness.

    Effectiveness of the OTSC clip in the management of patients with gastrointestinal bleeding

    Objective: To analyze the evidence on the effectiveness of the OTSC clip in the management of patients with gastrointestinal bleeding. Methods: The study was quantitative, with a systematic review design. The population consisted of 15 articles, and the sample included 10 scientific articles published in scientific databases. The instrument used was a search in the following databases: PubMed, UEG Journal, Clinical Endoscopy, Wiley Online Library, WJG – Gastroenterology, Journal of Gastroenterology, EBSCO, and Endosc Int Open. The articles were evaluated using the GRADE system, which assessed the quality of evidence and the strength of recommendations. Results: In this study, 10 articles were analyzed, and the use of the OTSC clip was found to be effective for daily routine practice, as its placement has become a new option for the treatment of both upper and lower gastrointestinal bleeding. Similarly, Regina and colleagues concluded in their study that the application of the OTSC clip is the first-line treatment of choice for patients experiencing high-risk bleeding episodes. Likewise, TH Kothari concluded that closure with OTSC clips is effective for closing gastrocutaneous fistulas, with patients recovering using this novel system. Ravishankar also agrees that the use of OTSC is effective and superior to the treatment of non-variceal upper gastrointestinal bleeding, and thus it should be considered as a first-line treatment. According to the study conducted by Mathew, it was concluded that 100% of the studies demonstrate that the use of the OTSC clip is effective in the management and treatment of patients with gastrointestinal bleeding caused by perforations and fistulas, thereby improving life expectancy, reducing long hospital stays, surgical interventions, high costs, and promoting faster recovery and reintegration into daily and work activities.
