Ítem Acceso abierto Clinical case: renal failure and renal transplantation(Universidad Privada Norbert Wiener, 2022-03-18) Retamal Salazar, Alejandro Augusto; Benites Azabache, Juan CarlosA 30-year-old female patient with a documented history of recurrent urinary tract infections since the age of 10, diagnosed with end-stage chronic renal failure at age 15, and placed on the waiting list for a kidney transplant with a cadaveric donor. After 3 years, she was selected to receive the kidney from cadaveric donor EDJ1402007; she progressed favorably according to renal function laboratory markers. After 10 years of good progress, she presented an episode of active gastric ulcer with active bleeding and received multiple blood transfusions. At the same time, she contracted pulmonary tuberculosis from a family contact, so immunosuppressants were discontinued, resulting in the loss of the renal graft. She received first-line antituberculosis therapy, with therapeutic success demonstrated by laboratory exams. Currently, histocompatibility studies show that the patient is hypersensitized (4) with anti-HLA I – II antibodies and a calculated PRA of 100%, making her chances of re-transplantation remote.
