Métodos: Se realiza una investigación cuantitativa, práctica, con un diseño no experimental, de tipo correlacional y con un enfoque transversal. La población censal estará formada por 80 enfermeros de la central de esterilización. Se utilizará un cuestionario validado de 20 ítems para evaluar el conocimiento en tres dimensiones: limpieza, desinfección y esterilización, con una confiabilidad de alfa de Cronbach de 0.845. Para evaluar las prácticas, se utilizará una lista de cotejo validada, con un coeficiente de confiabilidad KR-20 de 0.823, centrada en actividades como inspección, empaque y secado. Los datos se recopilarán mediante encuestas y observaciones directas, y se analizarán con SPSS 26. Los hallazgos se mostrarán mediante tablas y gráficos, con el objetivo de mejorar la formación del personal y hacer más eficientes los procesos de esterilización, lo que ayudará a garantizar la seguridad de los pacientes durante las cirugías.Ítem Acceso abierto Effectiveness of heated and humidified CO2 for reducing intraoperative hypothermia in patients undergoing laparoscopic surgery(Universidad Privada Norbert Wiener, 2021-11-07) Jesús Arias, Henry Ramón; Bastidas Solis, Miriam CeciliaObjective: To analyze and systematize the evidence on the effectiveness of heated and humidified CO2 for reducing intraoperative hypothermia in patients undergoing laparoscopic surgery. Materials and methods: The type of research was quantitative, the study design was a systematic review, the population was 45 articles, and the sample was 10 scientific articles published and indexed in scientific databases, the instrument was a search in the databases: Cochrane Library, Scielo, PubMed and EBSCO, for the evaluation of the articles the GRADE system was used which evaluated the quality of evidence and the strength of recommendation. Results: Of the 100% articles, 10% correspond to Canada, 10% France, 20% Australia, 10% United Kingdom, 20% New Zealand, 10% Sweden, 10% Austria and 10% Germany. In relation to the designs and types of studies, 50% were randomized clinical trials, 20% systematic review, 20% meta-analysis and 10% retrospective. Conclusions: It was concluded that 60% of the studies showed that heated and humidified carbon dioxide (CO2) was effective in reducing intraoperative hypothermia in patients undergoing laparoscopic surgery, reducing postoperative pain, reducing the consumption of analgesics, therefore reducing the cost per patient, improving patient survival and patients had fewer adverse effects and 40% showed that carbon dioxide (CO2) was not effective since it was not able to show significant changes in the intraoperative body temperature of patients operated on with this gas and there was fogging of the chamber.Ítem Acceso abierto Effectiveness of laparoscopic distal gastrectomy compared to open distal gastrectomy in reducing intraoperative blood loss and hospital stay in patients with early gastric cancer(Universidad Privada Norbert Wiener, 2018-11-25) Ildefonso Alberto, Ximena Genoveva; Villa Collazos, Katherin; Rivera Lozada De Bonilla, OrianaMaterial and Methods: Systematic observational and retrospective quantitative review, subjected to critical selection, using the Grade evaluation system to identify the level of evidence, found in the following databases: Cochrane Library, British Medical Journal, PubMed, Lancet , Wiley Online Library, Journal of Rheumatology. The 10 articles reviewed systematically, 60% (n= 6/10) are Meta-analysis, 20% (n= 2/10) are cohort studies and 20% (n= 2/10) are randomized clinical trials. These studies come from the countries of China (50%), followed by Japan (10%), Australia (10%), Korea (10%), the USA (10%), and England (10%). Results: The evidence found, 100% (n=6/10) indicates that Laparoscopic Distal Gastrectomy is effective in reducing intraoperative blood loss and hospital stay in patients with early gastric cancer. Methodology: Systematic observational and retrospective review, the search has been restricted to articles with full text, and the selected articles were subjected to a critical reading, using the Jover evaluation to identify their degree of evidence. Results: in the final selection, 15 articles were chosen, most of which are of a quasi-experimental quantitative type where it is evident that laparoscopic surgery has multiple advantages in patient recovery. When small incisions are made, the patient's recovery is faster, there is less pain and the disability is shorter, they can resume their normal activities quickly, they can walk, go up and down stairs, leave their house, shortly after be intervened. Conclusions. Results: Michele Scandola, in his study, concluded that robot-assisted surgery is more effective, with a shorter operative time compared to conventional laparoscopic surgery. Ahmet Gocmen, in his study, concluded that conventional laparoscopic surgery is more effective due to a shorter operative time and shorter hospital stay. Materials and Methods: A quantitative research approach was used, and the study design involved a systematic review. The population consisted of 46 articles, with a sample of 10 scientific articles published and indexed in scientific databases. The search covered the following databases: Scielo, PubMed, Epistemonikos, Cochrane Plus, and EBSCO. The articles were evaluated using the GRADE system, which assessed the quality of evidence and the strength of recommendations. Results: Ten articles were analyzed in this study. These articles showed that the most commonly used method was meta-analysis, which accounted for a higher percentage (40%) compared to other research designs. Conclusions: It was concluded that 60% of the studies demonstrate that laparoscopic surgery was effective in the treatment of gastric cancer, as it showed the patient's quick recovery, was cost-effective, and resulted in fewer adverse effects, thus reducing the incidence of possible complications. However, 40% of the studies indicate that robotic surgery is a promising approach for the treatment of gastric cancer, particularly regarding safety, feasibility, technique, and oncological efficacy.Ítem Acceso abierto Effectiveness of vaginal hysterectomy versus vaginal hysterectomy assisted by laparoscopy in patients with uterine pathologies(Universidad Privada Norbert Wiener, 2018-11-04) Sanchez Julián, Sandra Marisol; Calsin Pacompia, WilmerMaterials and Methods: The study design of this research was quantitative, and the study design was a systematic review. The population consisted of a total of 50 scientific articles, and the sample was made up of the bibliographic review of 10 scientific articles. The quality of the evidence and the strength of recommendations were evaluated using the GRADE system. Results: In summary, 8 out of the 10 pieces of evidence found indicate that vaginal hysterectomy is more effective, as the patient underwent surgery with a shorter time, not prolonged. Additionally, bleeding was less, and patients were reintegrated into their daily activities in a minimal amount of time, with a reduction in postoperative pain. In the final selection, 10 articles were chosen, the majority having been analyzed according to research design: longitudinal studies with 50%, systematic review with 20%, descriptive studies with 20% and cohort studies with 10%. Results: It was evident in the research reviewed that 70% of the effectiveness of laparoscopic surgery is greater than conventional surgery, since the results presented benefits in terms of decreased hospital stay (60%), less pain in the recovery (40%) and prompt reintegration to daily activities (30%), on the other hand, 30% of the studies reveal that both procedures are effective, and that although the laparoscopic approach presents fewer alterations in the surgical wound It still presents a higher economic cost compared to the conventional one, and in terms of the other variables, no major difference was found in both procedures. Results: The 10 articles systematically reviewed, 80% (n=08/10) of these, show the better effectiveness of laparoscopic inguinal hernia surgery compared to conventional surgery, it has a better result and is a viable and safe strategy for the patient undergoing this surgical intervention, in addition to reducing the days of hospital stay. Materials and methods: The systematic review of 10 scientific articles found in the following databases: PubMed, Clinical Key, Springer Link, OvidInsights beta, Wiley One Library. These sources allowed us to find 10 scientific pieces of evidence. Among the 10 pieces of evidence, 60% (6/10) are experimental studies, 30% (3/10) are systematic reviews, and 10% (1/10) is a meta-analysis. Results: 100% (n = 10/10) of the evidence found (15,16,17,18,19,20,21,22,23,24,25) indicate that laparoscopic nephrectomy is more effective in reducing complications and hospital stay in adult living donors. It is concluded that 9 out of 10 studies show laparoscopy as the most effective method, allowing complete treatment, postoperative recovery, and lower morbidity compared to laparotomy. Data collection was done through a literature review of national and international research articles that aimed to evaluate the effectiveness of total abdominal hysterectomy versus laparoscopic hysterectomy in patients with cervical cancer and uterine fibroids. Twenty scientific articles were reviewed, with a sample of 10 articles published in scientific databases in Spanish and English, no older than ten years. The selected articles were evaluated using the GRADE method, which allowed for the assessment of the quality of evidence and grading the strength of recommendations in the context of clinical practice guidelines development and systematic reviews. Ten scientific articles were reviewed, of which 30% were retrospective, 20% were quasi-experimental, 20% experimental, 20% systematic review, 10% prospective, and 10% observational. Ninety percent of the reviewed articles concluded that total laparoscopic hysterectomy is more effective than total abdominal hysterectomy. Material and methods: quantitative research , descriptive and cross-sectional; It will be carried out in a national hospital of level of care III-1 in the department of Lima, in the surgical center service to a population of 34 nurses who make up the total sample of the population. The techniques to be used will be the survey and observation; and, the instruments will be the questionnaire and checklist, which were designed by the researchers fernández and rosillo in their work entitled “knowledge and practice of the cleaning, disinfection and sterilization process of laparoscopic surgery instruments, hospital iii josé cayetano heredia piura, 2016”; which were validated through expert judgment using the corrected rank proportion coefficient, obtaining 0.929 and 0.957 respectively; and were subjected to pilot testing, they have Crombach's alpha reliability of 0.864 for the questionnaire, and 0.893 for the checklist. Methodology: It is a cross-sectional, non-experimental correlational study. The population will be made up of the registered personnel of the surgical center and sterilization center service. The technique that will be used are Instruments that will help collect the data: The collection of information will be carried out through the survey and observation technique; A questionnaire and a checklist will be used as instruments. During processing, authorization will be requested and applied. The reliability through the SPS - VS 17 program, the reliability through the analysis of Crombach's Alpha coefficient, the Spearman Rank correlation test and for the analysis of Chi square variables, and it will be supported by the average of the correlations within the items. The objective of the work under study is: Determine the relationship between knowledge and practice of the sterilization process of laparoscopy instruments in the nurses of the sterilization center of the Lambayeque Regional Hospital, 2022. Material and methods: Hypothetical-deductive, with a quantitative approach , applied, cross-sectional, with descriptive correlational design. The population will be made up of the nurses from the sterilization center in a sample of 13 professionals. The technique used will be the survey and observation, the questionnaire will be applied through two instruments for knowledge and the checklist for practice, taken from the thesis carried out by the authors Fernández Rodríguez, Rosa and Rosillo Moscol, Alma, whose Validity of the questionnaire was CPRic=0.92962667, the checklist CPRic=095740444 and Crombach's Alpha reliability for the questionnaire was 0.864, for the Crombach's Alpha checklist it was 0.893, both instruments are reliable. The data obtained will be processed in the SPS statistical program, the statistical tests to be used will be Spearman and phi coefficient.Publicación Acceso abierto Laparoscopia en apendicitis aguda con peritonitis complicada en gestante de primer trimestre en clínica particular de Lima marzo 2022(Universidad Privada Norbert Wiener, 2024-12-30) Portales Tapia, Araceli Del Rosario; Arone Palomino, Susana FilomenaObjetivo: Describir el manejo de apendicitis aguda complicada mediante tecnica laparoscopica en gestante de primer trimestre en Clínica privada de Lima en marzo 2022. Material y métodos: Estudio observacional, descriptivo, retrospectivo y revisión de caso clínico sobre el manejo de Laparoscopia en apendicitis aguda con peritonitis complicada en gestante de primer trimestre en clínica privada de Lima en marzo, 2022. Se realizó revisión de literatura para discusión de caso clínico. Descripción del caso clínico: Se presenta el caso de un paciente primigesta 7ss, quien ingreso por un cuadro de 2 días de evolución, caracterizado por dolor abdominal difuso de moderada intensidad asociado a nauseas, vómitos y deposiciones liquidas, mismas que fueron aumentando de intensidad paulatinamente. Mediante la correlación clínico-patológica se llegó al diagnóstico de Abdomen agudo - Apendicitis aguda. Al paciente se le realizaron exámenes para descartar etiologías infecciosas, fue tratado mediante antibióticos y analgesia, continuando el pase a sala de operaciones donde confirman el diagnostico de apendicitis con peritonitis generalizada. Paciente continua hospitalización por 5 días con monitoreo continuo antibiótico terapia analgesia horaria, control de drenaje Jackson pratt y BHE; al encontrarla estable al PO5 se le procede a retiro de drenaje y alta con indicaciones. Paciente continúa su gestación llegando a término en octubre 2022 vía parto por cesárea con recién nacido vivo sin complicaciones Conclusión: El diagnostico de apendicitis aguda en gestante del primer trimestre de una clínica en Lima se basa principalmente en el cuadro clínico correlacionados con exámenes e imágenes diagnósticas. En este caso en específico se utiliza laparoscopia como técnica menos invasiva donde no evidenciamos complicaciones a corto o largo plazo sobre la madre, el embrión o nacido vivo. A continuación se presenta el caso, junto con una revisión sobre el tema de apendicitis aguda; abarcando aspectos relevantes sobre su incidencia en la gestación, cuadro clínico y tratamiento.
