Effectiveness of the nasal mask compared to nasal prongs in reducing severe nasal injury in premature infants undergoing CPAP

Objective: to systematize the evidence on the effectiveness of the facial mask compared to the nasal prong to reduce severe nasal injury in premature infants subjected to positive airway pressure (CPAP). Material and Methods: Systematic reviews are an observational and retrospective research design, which synthesizes the results of multiple primary investigations. They are an essential part of evidence-based nursing due to their rigorous methodology, identifying relevant studies to answer specific questions of clinical practice. The systematic review of the 10 were found in the following Pubmed databases, all of them were analyzed according to the GRADE scale to determine their strength and quality of evidence. Results: Regarding nasal trauma in general it varied from 78% in prongs compared to 85% in the nasal mask. Similarly, the incidence of moderate to severe ranged from 31% in nasal prongs compared to 24% in nasal masks. The incidence of severe nasal trauma was lower with the use of 0% nasal masks. Overall, the results suggest that the incidence of nasal trauma is considerable. Conclusion: positive airway pressure when used with a nasal mask is just as effective as with nasal prongs, but the former causes less trauma, this because the mask rests on the nose, bridge and philtrum; that with constant pressure hinders the perfusion of the local tissue triggering skin injury, while with nasal prongs the constant pressure is on the columella and anterior part of the nasal septum.
