    Critical review: effect of oral glutamine supplementation on mucositis induced by cancer treatment of head and neck cancer

    The information search was carried out in the Medline and Cochrane databases, finding 23 articles, 11 being selected, which have been evaluated by the CASPE critical reading tool, finally the selected article allowed us to conclude that oral glutamine supplementation reduces the incidence and severity of mucositis in adult patients with head and neck cancer

    Critical review: nutritional management in patients with neoplasia of the head and neck undergoing chemo and/or radiotherapy treatment

    Nutritional management is important for oncological patients as it helps keep them stable during chemotherapy and/or radiotherapy treatment. This secondary research titled: ‘CRITICAL REVIEW: NUTRITIONAL MANAGEMENT IN PATIENTS WITH HEAD AND NECK NEOPLASIA UNDER CHEMOTHERAPY OR RADIOTHERAPY TREATMENT’ aimed to determine the best nutritional management for patients with head and neck neoplasia undergoing chemotherapy or radiotherapy treatment, identify the common problems that occur before, during, and after chemotherapy and radiotherapy treatment concerning the nutritional status of cancer patients. The clinical question was: ‘What is the nutritional management for patients with head and neck neoplasia undergoing chemotherapy or radiotherapy treatment?’ The Evidence-Based Nutrition (EBN) methodology was used. Information was gathered from PubMed/MEDLINE, ScienceDirect, WOS, and SCOPUS, yielding 20 articles, with the final selection based on the ‘CASPE’ tool from the meta-analysis titled: 'Nutrition management for patients with head and neck cancer during peri-radiotherapy: A systematic review and quality appraisal of clinical practice guidelines using the AGREE II instrument.' The critical commentary concluded that there were discrepancies in existing clinical practice guidelines (CPGs), including nutritional screening, assessment, intake, nutrients, the absence of essential parts of the CPGs, the statement of external review, the method for making recommendations, strategies for improving acceptance, and the implications of resource use in applying the CPGs. Specialized CPGs should be developed for the nutritional treatment of head and neck cancer patients during radiotherapy.

    Nutritional support in patients with cancer head and neck

    Head and neck cancer patients have high prevalence rates of malnutrition, negatively impacting their survival, therefore nutritional support measures are absolutely necessary. Objective: to analyze the evidence on nutritional support in patients with head and neck cancer. Clinical question: What is the nutritional support provided to patients with head and neck cancer? Methodology: a critical review of the different research works that could answer the clinical question was carried out. Studies were included where nutritional support measures were evaluated in patients with head and neck cancer, those studies published in Spanish, English and Portuguese, between 2010 and 2020. Likewise, the Boolean operators (AND, OR and NOT) were used to connect the keywords. The search was restricted to articles with full text, which were subjected to critical reading, using the CASPe evaluation system. The CASPe critical reading program was used to evaluate the quality of the evidence of the selected articles, from the different information seekers. Results: 40 research papers were found, of which 12 were selected. According to the findings of the reviewed scientific studies, it is evidenced that there are multiple measures of nutritional support in patients with head and neck cancer. Conclusion: there are various nutritional support measures in patients with head and neck cancer, which include: perioperative education, screening at the beginning of care, evaluating nutritional status, early resumption of postoperative nutrition, perioperative supplementation, immunonutrition and feeding tube placement selectively.

    Revisión crítica: efecto de la suplementación con curcumina durante la quimioterapia / radioterapia en pacientes adultos con cáncer de cabeza y cuello

    La curcumina es el compuesto activo de la planta medicinal cúrcuma, que pertenece a la familia Zingiberáceas y tiene su origen en Asia. Diversos estudios experimentales han determinado su potencial biológico y terapéutico, principalmente atribuyéndole propiedades antioxidantes, antiinflamatorias, antimoduladoras, hipolipemiantes y antineoplásicas. La presente revisión crítica tuvo como objetivo evaluar la efectividad de la suplementación con curcumina en relación con los desenlaces primarios (sintomatología asociada a la quimioterapia) y secundarios (potencial quimiopreventivo y terapéutico). Se aplicó la nutrición basada en evidencia (NuBE). Se llevó a cabo una búsqueda bibliográfica utilizando bases de datos científicas reconocidas, como Web of Science, Science Direct y PubMed. Se seleccionaron un total de 28 estudios clínicos, de los cuales se incluyeron únicamente 12 artículos (entre ensayos clínicos y revisiones sistemáticas) que cumplieron con los criterios de elegibilidad. Cada artículo fue evaluado de manera crítica utilizando la herramienta CASPE. Finalmente, se eligió un ensayo clínico aleatorizado titulado “Effects of nanomicelle curcumin capsules on prevention and treatment of oral mucositis in patients under chemotherapy with or without head and neck radiotherapy: a randomized clinical trial”. Se concluyó que la suplementación con curcumina es efectiva para reducir la sintomatología asociada a la quimioterapia y radioterapia en pacientes con cáncer de cabeza y cuello.

    Revisión crítica: efecto de la suplementación oral con glutamina en la severidad de mucositis de pacientes con cáncer de cabeza y cuello

    La mucositis oral constituye una complicación frecuente en pacientes que reciben radioterapia y quimioterapia. Se ha propuesto que la glutamina oral puede tener un efecto beneficioso al disminuir la severidad de la mucositis oral y mitigar otras complicaciones asociadas a la misma. El propósito de la investigación fue evaluar el efecto de la suplementación oral con glutamina en la severidad de mucositis en pacientes con cáncer de cabeza y cuello. La recopilación de información se llevó a cabo en las bases de datos PubMed, ScienceDirect y Scopus, identificándose un total de 29 artículos. De estos, 10 fueron sometidos a evaluación empleando los criterios de la herramienta de lectura crítica CASPe. Finalmente, se seleccionó una revisión sistemática y metaanálisis que, según la experiencia del investigador, poseía un nivel de evidencia A1 y un grado de recomendación Fuerte. Al realizar la discusión crítica se determinó que el uso de glutamina oral contribuye a disminuir la severidad de la mucositis oral inducida por radiación de forma estadísticamente significativa, sin efectos adversos clínicos para el paciente. Sin embargo, aún existen limitaciones con respecto a la dosis, frecuencia y administración. Por consiguiente, se requieren investigaciones adicionales que permitan superar las restricciones actuales de esta estrategia dieto terapéutica para el manejo futuro de la mucositis oral en pacientes oncológicos.
