    Buenas prácticas de prescripción e intervenciones farmacéuticas en pacientes atendidos en emergencia de la Clínica Internacional sede San Borja enero–marzo 2021

    A medical prescription is the result of a logical-deductive process in which the prescriber reaches a diagnostic orientation and a therapeutic decision. The objective of this study was to evaluate compliance with good prescribing practices and pharmacist interventions in patients treated in the emergency department of the Clínica Internacional, San Borja branch, from January to March 2021. Method: The study was descriptive with a prospective design. A sample of 354 prescriptions was considered, in which good prescribing practices and pharmacist interventions were evaluated according to the study's dimensions. Results: Compliance with the registration of patient data on medical prescriptions was 82.32%. Prescriber data was complied with at 100%. Compliance with medication data was 80.68%, and 19.32% did not comply. It was found that 7.35% of prescriptions required dose adjustment, 3.25% had drug interactions, 1.25% had contraindications, and 18.2% presented therapeutic duplication. Conclusion: It was determined that there is compliance with good prescribing practices and pharmacist interventions in the prescriptions of patients treated in the emergency department of the Clínica Internacional, San Borja branch.

    Buenas prácticas de prescripción y uso de inhaladores antiasmáticos para pacientes pediátricos en recetas emitidas del Hospital José Agurto Tello de Chosica de abril 2019 a marzo 2020. Lima 2021

    The objective of this research is to determine good prescribing practices and the use of antiasthmatic inhalers for pediatric patients in prescriptions issued at the José Agurto Tello Hospital in Chosica from April 2019 to March 2020. Methodology: This study used a quantitative approach, with a basic type, a descriptive level, and a non-experimental design. The instrument used was a data collection form. The population consisted of 923 prescriptions, and the sample was comprised of 50% of these prescriptions, selected randomly. The results were processed using descriptive statistics. Results: Omissions were found in patient age (18%), clinical history (0.4%), type of care (5%), diagnosis (0.4%), ICD-10 code (4%), according to PNUME (0.2%), concentration (14%), pharmaceutical form (0.2%), quantity (2%), dose (50.9%), route of administration (63%), frequency (51%), duration of treatment (52%), and legibility (13%). The evaluation of antiasthmatic inhaler use showed the highest consumption of salbutamol (81.6%) in the school-age group (35.6%), with the diagnosis being appropriate (94%). Conclusion: Only 26% of prescriptions complied with 17–19 indicators, 64% with 14–16, and 10% with fewer than 14. It was found that bronchodilators were the most consumed inhalers in the emergency area.

    Características de la prescripción – indicación de antibióticos en gestantes del Instituto Nacional Materno Perinatal julio 2019–junio 2021

    The research aimed to determine the characteristics of antibiotic prescription-indication in pregnant women at the National Maternal Perinatal Institute, July 2019 - June 2021. Methodology: Applied type, deductive method, quantitative approach, non-experimental, cross-sectional, and retrospective design. Data was collected from 328 medical records using a validated instrument, organized in Microsoft Excel, and analyzed with IBM SPSS version 24. Results: A moderate level of 73% was identified regarding the characteristics of antibiotic prescription-indication. The most frequent indications were preoperative antibiotic prophylaxis at 31.7% and urinary tract infections at 28.66%. Obstetric-gynecology specialists prescribed 95% of the antibiotics, with the highest frequency in the third trimester at 82.1%. The most used antibiotics were Beta-lactams at 79.27%, particularly Cefazolin 1g injectable at 25.8% and Cephalexin 500 mg tablets at 13.9%. Dosages were appropriate in 99.70%, and 93.3% of the prescribed antibiotics were classified as risk category B. Conclusion: The characteristics of prescription-indication were appropriate, as prescriptions were issued by specialized physicians, taking care to ensure accuracy in the indication, dosage, type of antibiotic, frequency, and compliance with the FDA risk classification categories.

    Características de la prescripción, disponibilidad y expendio de antimicrobianos en establecimientos farmacéuticos (farmacias y boticas), del distrito de Puente Piedra – DISA V. Enero 2018

    The research work has as its objective: to determine the “Characteristics of Prescription, Availability, and Dispensing of Antimicrobials in Pharmaceutical Establishments (Pharmacies and Drugstores) of the District of Puente Piedra – DISA V, January 2018.” Methodology: This corresponds to a descriptive, observational, prospective, cross-sectional, quantitative, non-experimental study. The spatial universe comprised 18 sectors with a total of 237 pharmaceutical establishments (5 pharmacies and 232 drugstores) in the district of Puente Piedra, during the period from January 1 to January 31, 2018. Data collection was carried out using standardized forms (Care and Dispensing). Results: The results obtained indicate that the proportion of prescriptions with prescribed antimicrobials was 84%. Furthermore, 70% of the prescribed antimicrobials belonged to the National Essential Medicines List (PNUME), 60% contained their International Nonproprietary Name (INN), and the proportion of dispensed antimicrobials that were substituted was 2.2%, with a 95% confidence level. The proportion of prescribed antimicrobials with registered dosage was 87%, of which 98% had the administration frequency recorded and 99% had the duration of treatment documented. Conclusion: This research allowed us to determine the most frequent problems patients face when receiving a medical prescription. The dispensing process often results in the inappropriate dispensing of antimicrobials, and the lack of availability contributes to the development of antimicrobial resistance.

    Características de la prescripción, disponibilidad y expendio de antimicrobianos en farmacias y boticas del distrito de Independencia – DISA V, setiembre 2014

    This research focused on the prescription, availability, and dispensing practices of antimicrobials in pharmacies and drugstores in the district of Independencia, DISA V – Lima, September 2014. The study was descriptive, cross-sectional, and retrospective, and data were collected through a dispensing/care form that recorded information about antimicrobials prescribed according to patients’ conditions. Results showed that 67.13% of prescribed antimicrobials were included in the National List of Essential Medicines, but only 41.71% were prescribed using their International Nonproprietary Name, a value below the expected target. Additionally, 23.48% of antimicrobials were substituted at the time of dispensing. The evaluation of treatment parameters indicated that dosage (87.85%), frequency of administration (88.67%), and treatment duration (88.95%) were outside the expected range. In pharmacies and drugstores, 23.48% of antimicrobials were replaced due to lack of availability or recommendation of alternatives, and only 67.13% of dispensed antimicrobials corresponded to the National List of Essential Medicines. These findings reveal shortcomings in antimicrobial prescribing and dispensing practices, underscoring the need for improved compliance with national guidelines to ensure safe and rational use of medicines.

    Introduction: Antimicrobials are the cornerstone in the treatment of infectious diseases. However, their inappropriate use by prescribers (physicians), dispensers (pharmacists), or patients increases antimicrobial resistance and reduces the effectiveness of available drugs. The consequences of irrational antimicrobial use are clinical, ecological, and economic, contributing to high morbidity and mortality. Objective: To determine the characteristics of prescription, availability, and dispensing of antimicrobials in pharmacies and drugstores in the district of Santa Anita – DISA IV – Lima Este, August 2014. Methodology: A descriptive, cross-sectional study was conducted using dispensing/care forms to collect data regarding antimicrobials administered to patients according to their illnesses. The study was applied to 4 pharmacies (5%) and 83 drugstores (95%) in Santa Anita district, totaling 87 pharmaceutical establishments. The objective was to assess the proportion of antimicrobials dispensed and their compliance with medical prescriptions. Data were analyzed using SPSS version 22 and Microsoft Excel 2010. Results: Among prescribed antimicrobials, 60% were indicated by International Nonproprietary Name (INN), 82% belonged to the National List of Essential Medicines (PNUME), and 82% were completely dispensed. Additionally, 88% of antimicrobials were dispensed correctly according to the medical prescription. Conclusion: The study determined that in pharmacies and drugstores of Santa Anita – DISA IV, Lima Este (August 2014), 60% of antimicrobials were prescribed by INN, 82% corresponded to the PNUME, and the oral route was the most prescribed pharmaceutical form. Moreover, the majority of antimicrobials were fully and correctly dispensed.

    Características de prescripción de antibióticos en las recetas médicas dispensadas en la farmacia del consultorio externo del Hospital Nacional Dos de Mayo, marzo–agosto 2016

    The characteristics of antibiotic prescribing in medical prescriptions dispensed at the outpatient pharmacy of the Dos de Mayo National Hospital do not comply with good prescribing practices. There was a higher percentage incidence in the lack of legibility in prescriptions, the lack of the doctor's signature, the lack of the doctor's stamp, the lack of treatment duration, and the lack of medication dosage. The majority of medical prescriptions (90.2%) contained only one antibiotic per prescription, while antibiotic combinations were present in 9.8% of medical prescriptions. In the ATC classification of antibiotics, 100% predominance was found in the anatomical group Anti-infectives for systemic use and the therapeutic subgroup antibacterials for systemic use. The therapeutic group with the greatest presence was β-lactam antibacterials, penicillins. Finally, Amoxicillin was the most used chemical substance or active ingredient in the research. In the ATC classification of antibiotics, 100% predominance was found in the anatomical group Anti-infectives for systemic use and the therapeutic subgroup antibacterials for systemic use.

    Características y nivel de cumplimiento de buenas prácticas de prescripción de antibióticos en recetas atendidas en boticas del distrito de Santa Anita – Lima 2022

    The present research was titled "Compliance with Good Prescription Practices in Medical Prescriptions Dispensed in Pharmacies in Santa Anita, 2022." The objective was to determine the characteristics and level of compliance with good prescription practices in antibiotic prescriptions dispensed in pharmacies in the district of Santa Anita, Lima, 2022. The method used was deductive, with a quantitative approach, basic type, and non-experimental design. The study population consisted of 5,000 prescriptions, from which a non-probabilistic sample of 400 users was analyzed. The observation technique was applied, and the data were analyzed using descriptive statistics represented in tables and graphs. Results: The most prescribed antibiotics were ciprofloxacin (18%), amoxicillin (14%), and azithromycin (11%). The most common routes of administration were oral (81%), parenteral (14%), and topical (5%). Potential adverse reactions were type A in 88% and type B in 12%. Additionally, patient compliance was 45.33%, compliance with medication data was 76%, and compliance with prescription practices was 70.66%. Conclusion: The level of compliance with good prescription practices for antibiotics in pharmaceutical prescriptions in Santa Anita averaged 63.99% across the analyzed prescriptions.

    Caracterización de interacciones medicamentosas potenciales en las prescripciones médicas atendidas a pacientes ambulatorios

    The objective of this study was to determine the characterization of potential drug interactions present in medical prescriptions attended to outpatients at the InkaFarma pharmacy located in Flora Tristán, La Molina district, between April and June 2014. A descriptive, cross-sectional, retrospective, and observational study was designed for this purpose. Medical prescriptions from various specialties were considered. According to the inclusion criteria, 191 medical prescriptions were analyzed. It was found that 67% did not present drug interactions, while 33% did. Regarding sex distribution, 41% of the population were male and 58% female; 35 females and 28 males presented drug interactions. Concerning the onset of interactions, the majority (77%) were undocumented, 18% had a slow onset, and 6% had a rapid onset. Regarding the severity, 81% of the interactions were moderate, 10% were major, and 9% were minor. By mechanism of action, 48% were pharmacodynamic and 37% were pharmacokinetic. Regarding age groups, the majority of the population were adults (83 patients), followed by young people (81 patients). The specialty with the highest number of drug interactions was internal medicine, with 17 cases out of 88 prescriptions attended.

    Comprensión de la prescripción médica y cumplimiento del tratamiento farmacológico en los usuarios de la botica bienestar y hogar del distrito Lurín. Lima. 2023

    El presente estudio tuvo como finalidad determinar la relación que existe entre la comprensión de la prescripción médica y el cumplimiento del tratamiento farmacológico en los usuarios de la botica Bienestar y Hogar del distrito Lurín. Lima. 2023. Respecto a la metodología fue una investigación tipo básica, con un método hipotético deductivo, con un enfoque cuantitativo, de nivel descriptivo correlacional con un diseño no experimental de corte transversal. La muestra estuvo conformada por 216 usuarios; como técnica se aplicó una encuesta y como instrumento para la recolección de datos se usó el cuestionario de 24 ítems previamente validados por tres profesionales de la Universidad Norbert Wiener que cuentan con la experiencia requerida en la materia. Los resultados mostraron que la variable comprensión de la prescripción médica tuvo un nivel alto con 40,28 %, así mismo se observó en cuanto el cumplimiento del tratamiento farmacológico un nivel medio siendo el 43,06 %, el cual se sustenta con la correlación de Spearman de 0,469 considerándose este resultado como positiva moderada. Finalmente se concluye que existe una relación, con una significancia de p=0,000 siendo menor que el 0.01, entre la comprensión de la prescripción médica y el cumplimiento del tratamiento farmacológico en los usuarios que acuden con receta médica a la botica Bienestar y Hogar del distrito Lurín.     Cumplimiento de buenas prácticas de dispensación de medicamentos en la farmacia asistencial del distrito de Lurín Lima Provincias, 2022

    The objective of this research was to determine whether good medication dispensing practices are being followed in the pharmacy of the Lurín district, Lima provinces, 2022. The methodology applied a quantitative approach, descriptive level, basic type, non-experimental cross-sectional design, with a population of 320 users of the establishment and a sample of 153. Results: It was determined that good dispensing practices are being followed in the pharmacy of the Lurín district, Lima provinces, 2022, as the analysis of symmetric measures showed a significance level below 0.05 (0.000), and the contingency coefficient for user age was 0.746 and for user gender was 0.653. Conclusion: It is essential to maintain and improve the development of good dispensing practices.

    Patients, upon attending the hospital, seek quality care and prescriptions that are properly completed and free of health risks. Objective: To quantify the percentage of compliance with Good Prescribing Practices and the presence of Drug Interactions in medical prescriptions from the Emergency Department. Methodology: An inductive method with a quantitative approach, basic type, and a descriptive, retrospective design was applied. A total of 330 medical prescriptions from the department were reviewed. Data were processed using Excel and SPSS. Results: A 100% non-compliance rate was observed in prescriptions regarding the overall presence of prescribing indicators. The patient data dimension showed 99% compliance with its indicators, medication data 97%, and prescriber data 82%, with deficiencies in “legible handwriting” (41%) and “expiration date” (0%). Regarding drug interactions, a prevalence of 19% was identified, predominantly pharmacodynamic type at 18%. The most frequent clinical severity was moderate (72%), followed by mild (25%). Conclusion: It was concluded that there was no full compliance in prescriptions, and drug interactions of moderate severity were identified.

    Cumplimiento de buenas prácticas de prescripción en las recetas atendidas por la Unidad de Farmacia de Dosis Unitaria del Instituto Nacional de Salud del Niño San Borja. Lima 2022

    The purpose of this thesis study was to analyze the "compliance with good prescription practices in prescriptions handled by the Unit Dose Pharmacy of the National Institute of Child Health San Borja in 2022." The methodology of this research was observational, descriptive, prospective, and cross-sectional. A sample of 385 medical prescriptions was randomly selected, and the statistical program SPSS 25.0 was used for data analysis. For data collection, a verification sheet was designed as an instrument, validated by highly qualified expert professionals. It was determined that 100% of the medical prescriptions had some form of deficiency or omission when prescribing treatments. Regarding the results, it was evidenced that 100% included patient last names and first names, phone numbers, medical history, age, and ICD-10 codes. Additionally, 100% used the International Nonproprietary Name (INN), and 100% recorded the drug concentration. In the medical prescriptions, 75.5% included posology, 100% indicated the quantity, and 82.9% specified the pharmaceutical form. Regarding prescriber details, 100% of the prescriptions included the prescription date, but 100% lacked an expiration date. Finally, based on the results, it was concluded that there is noncompliance regarding the proper completion of prescription data, thus failing to fully meet the requirements stipulated by the Peruvian Ministry of Health regulations.

    Cumplimiento de buenas prácticas de prescripción en recetas atendidas en el servicio de dosis unitaria de un establecimiento de salud público, periodo mayo-octubre, 2018

    The objective of this research thesis was to evaluate compliance with the manual of good prescribing practices on prescription sheets filled in the unit dose service of a public health facility from May to October 2018. The study was observational, descriptive, cross-sectional, and retrospective. The sample consisted of 858 medical prescription sheets for hospitalized patients from various hospital services, which were filled at the public health facility's unit dose service in Lima, Peru. The sample met both inclusion and exclusion criteria. For data collection and analysis, a checklist instrument was created and validated by three expert professionals to ensure greater reliability. The results confirmed that good prescribing practices were not being followed. This is a crucial tool for ensuring adequate and efficient medical prescriptions that benefit the hospitalized patient.

    Cumplimiento de las buenas prácticas de prescripción de medicamentos por dosis unitaria en la farmacia de emergencia del Instituto Nacional Materno Perinatal de Lima, 2022

    Objective: To evaluate compliance with Good Prescription Practices (GPP) for unit dose medications in the emergency pharmacy of INMP in Lima, 2022. Method: This study employed a quantitative approach, applied type, observational, descriptive, and retrospective design. It analyzed a sample of 20,497 prescription sheets. The instrument used was a data collection form. The results were clear, aligned with the objectives, and supported by the "SPSS version 25" program for statistical analysis of the variable and its dimensions. Results: The findings reveal that the majority of medical prescriptions comply with GPP. The level "complies" predominates in all dimensions. Specifically, in the "medication data" dimension, the "complies" level was 85.5% in 371 prescriptions analyzed. In the "prescriber data" dimension, the "complies" level was 78.1% in 339 prescriptions analyzed. The other levels of the variable compliance with GPP were less than or equal to 53.7%. Conclusion: The study concluded that the majority of medical prescriptions comply with GPP for Unit Dose medications in the emergency pharmacy of INMP in Lima, 2022.

    The research aimed to determine compliance with good prescribing practices (GPP) in prescriptions filled at the outpatient pharmacy service of the Anglo-American Clinic from September to November, in San Isidro. Methodology: This is a descriptive study with a quantitative approach and a non-experimental design. The population size was 740 prescriptions, and a sample of 253 prescriptions was evaluated based on the proposed GPP dimensions. Results: It was found that 100% (253) of the analyzed medical prescriptions from the Anglo-American Clinic's outpatient pharmacy fully complied with GPP standards across the four dimensions of the study. Additionally, a specific analysis was conducted according to the study objectives: 100% (253) showed a high level of compliance with GPP related to Patient Data; 100% (253) showed a high level of compliance with Physician Data; 98.42% (249) showed a high level of compliance with Medication Data; and 92.49% (234) showed a high level of compliance with Prescription Validity and Legibility. Conclusion: It was determined that all indicators highly comply with the standards specified in the GPP guidelines.

    Cumplimiento de las buenas prácticas de prescripción para pacientes con tratamiento de quimioterapia en el servicio de oncohematología de la Clínica Internacional, periodo marzo–agosto de 2014

    This research sought to evaluate compliance with Good Prescribing Practices for antineoplastic medications in the Oncohematology Service of Clínica Internacional during the period March–August 2014. With authorization from the research department, an observational, descriptive, retrospective, cross-sectional case-series study was conducted. A total of 415 chemotherapy prescriptions issued between March and August 2015 were analyzed according to the Peruvian General Health Law (Law No. 26842, Article 26) and the Ministry of Health’s Good Prescribing Practices Manual. The results revealed significant deficiencies in prescription documentation. The most frequent omissions included: lack of clinical history (67.47%), omission of patient weight (99.28%), omission of patient height (99.52%), omission of body surface area (98.07%), omission of diagnosis (91.33%), and missing information about the attending physician (62.17%). These omissions represent critical elements necessary for the appropriate validation of prescriptions. In conclusion, the study demonstrated that the minimum standards required to ensure safe and adequate prescribing practices for chemotherapy patients were not met at Clínica Internacional, confirming the need for improvement in prescription quality.

    Cumplimiento de las recetas medicas con respecto a los aspectos legales y farmacologicos atendidas en Boticas ByS en el distrito de San Isidro julio 2017–febrero 2018

    The objective of the study was to evaluate the legal and pharmacological aspects of medical prescriptions filled for patients at the Boticas y Salud drugstores in the San Isidro district from July 2017 to February 2018. Methodology: The design was observational and cross-sectional. Out of a population of 870 medical prescriptions, a sample of 585 was selected. All of these prescriptions were evaluated for Good Prescribing Practices (GPP) across the dimensions of prescriber data, patient data, and medication data. Additionally, 281 medical prescriptions were used for the identification of drug interactions and the evaluation of therapeutic indication. Results: The medical prescriptions filled did not comply with the legal and pharmacological aspects. Compliance for prescriber data was nearly total at 90.8%. Compliance for patient data was significantly lacking at 15.2%, and compliance for medication data was similarly lacking at 9.2%. Regarding therapeutic indication data, 5% had an incorrect dose, 8.3% had an incorrect frequency, and 15.4% had an incorrect treatment duration. For drug interactions, 65.9% were pharmacodynamic, 26.4% were pharmacokinetic, and 12.5% were of unknown type. By severity, 9.2% were major, 85.1% were moderate, and 4.9% were minor. Conclusion: The medical prescriptions filled do not comply with Good Prescribing Practices. Furthermore, while the majority comply with therapeutic indication correspondence, drug interactions are present in 65.9% of the total prescriptions filled at Boticas y Salud in the San Isidro district.

    Errores de prescripción de recetas atendidas en una oficina farmacéutica de Inkafarma San Juan 1. Lima 2021

    The irrational use of medicines is a global problem due to medication errors, with prescribing being the most common error. The objective of this research was to evaluate prescribing errors in prescriptions filled at an Inkafarma pharmaceutical office in San Juan 1, Lima, in 2021. Methodology: The study was applied type, with a deductive method, and an observational, prospective, cross-sectional design. The population consisted of 2,000 prescriptions, of which 322 were analyzed. A data collection form was used as the instrument, covering four dimensions: prescriber data, patient data, medication data, and prescription legibility, all of which must comply with the manual of good prescribing practices. Results: In the patient data dimension, 61.8% were non-compliant. In the medication data dimension, 69.3% were non-compliant. In the prescriber data dimension, there was a minimum error of 0.9%, and for legibility, 7.1% were non-compliant. Conclusion: All of the analyzed prescriptions presented at least one error in their dimensions. Patient data had the highest overall error rate, with the INN (International Nonproprietary Name) indicator specifically having 20.5% of errors.

    Medication errors account for approximately 16%, equivalent to 80,000 deaths per year. This problem is linked to multiple factors that converge to produce a severely negative impact on both the patient and the healthcare system. The present research aims to provide information on prescription errors so that healthcare facilities can implement protocols to help reduce the error rate. Objective: To evaluate prescription errors in the prescriptions processed at the outpatient pharmacy of the Naval Medical Center during the period from January to March 2021. Methodology: This is an applied study with a deductive method and an observational, retrospective, cross-sectional design. A data collection form was used to analyze a sample of 336 prescriptions from a population of 2,700 prescriptions processed at the outpatient pharmacy of the Naval Medical Center during the period from January to March 2021. The evaluation was carried out by analyzing 16 indicators grouped into four dimensions: patient data, prescriber data, medication data, and legibility. Results: The percentages of errors were 9.38%, 18.82%, 47.27%, and 38.99% for the dimensions of patient data, prescriber data, medication data, and legibility, respectively. Conclusion: The prescriptions processed at the outpatient pharmacy of the Naval Medical Center during the period from January to March 2021 presented errors in all prescriptions analyzed.
