Material and methods: An observational, descriptive retrospective study, reviewing the perinatal clinical history of a case of PROM in preterm labor at a public health facility in Lima during February 2017. Results: The predominant symptom was the loss of amniotic fluid for more than 48 hours. Pulmonary maturation was performed with betamethasone. Antepartum antibiotic therapy with intravenous cefazolin was administered. During the first stage of labor, fever, tachycardia, leukocytosis, and fetal tachycardia occurred, which led to the termination of the pregnancy via cesarean section, resulting in a live newborn weighing 2585 g, meconium-stained, with a gestational age of 39 weeks. Broad-spectrum antibiotic therapy with intravenous ceftriaxone + gentamicin + clindamycin was used post-cesarean. During the postoperative period: wound infection, endometritis, and pelvic peritonitis developed, requiring total abdominal hysterectomy. Conclusion: Prolonged PROM in preterm pregnancy is an important risk factor for maternal morbidity due to causing chorioamnionitis, endometritis, pelvic peritonitis, and wound infection; additionally, the maternal evolution was slow, progressing to acute surgical abdomen, leading to total hysterectomy, resulting in a prolonged hospital stay.
