    Conocimiento sobre el cáncer de estómago y la práctica preventiva en pacientes que asisten a la consulta de gastroenterología de un Hospital de Lima 2024

    Objetivo general: ¨Determinar como el conocimiento sobre el cáncer de estómago se relaciona con la práctica preventiva en pacientes que sean atendidos en el Consultorio de Gastroenterología de un Hospital Nacional¨ Metodología: una investigación cuantitativa, de nivel correlacional, no experimental, aplicada y de corte transversal. Población: conformada por 95 pacientes que asisten al consultorio de gastroenterología del hospital objeto de estudio, la muestra presentada es censal por ser una población mínima y manejable, se tomaron en consideración estos dos criterios de exclusión e inclusión y con el propósito de bajar y manejar el porcentaje de información. Instrumento: Es el cuestionario y la técnica presentada para obtener los de datos será la encuesta; para la primera variable de conocimiento sobre la enfermedad del cáncer de estómago usaremos un cuestionario de 20 preguntas que sean de selección simple, para la variable practica el cuestionario estará conformado por 17 preguntas en escala de Likert, diseñado por Avendaño 2022. Los resultados se plasmarán en gráficos y tablas con un previo estudio de análisis estadístico descriptivo por medio del software SPSS, en el cual incluya un procesamiento basado en Excel, donde se utilizará como tanteo de hipótesis la RHO de Spearman, mediante el tiempo que dure este estudio la investigación mantendrá un comportamiento ético en donde los códigos estándares y leyes que representan la ética se deberán mantener en la investigación realizado.

    Critical review: diet risk factors associated with gastric cancer.

    In our country, gastric cancer continues to be the second malignant neoplasm with the worst prognosis and the highest number of deaths. Therefore, the objective of this review was to know the risk factors that are associated with the development of gastric cancer, for this purpose the evidence-based nutrition (NUBE) methodology was used. The search strategy was developed with the keywords, and it was carried out in the Scopus, Embase, Pubmed and Cochrane Library databases. Of those, a total of 60 were obtained, of which 10 were selected for analysis through the CASPe tool, to be able to classify them according to degree of evidence and level of recommendation. The selected article was classified with an AI level of evidence and a strong recommendation; salt in the diet is an independent risk factor for the development of gastric cancer, just as alcohol consumption is more likely to be a risk factor. risk of excessive consumption. Likewise, red meat and a high-fat diet increase the risk, while fresh fruits, vegetables and certain micronutrients such as selenium and vitamin C have protective effects.

    Critical review: effect of early enteral nutrition in the postoperative period of patients with gastric cancer undergoing oncological surgery

    It is presumed that early initiation of enteral nutrition in patients with gastric cancer who have undergone gastrectomy would improve some clinical outcomes. The present secondary research entitled "Critical Review: Effect of early enteral nutrition in the postoperative period of patients with gastric cancer undergoing oncological surgery", aimed to analyze the effects (clinical outcomes and nutritional aspects) of early enteral nutrition 24 or 48 hours after oncological surgery in patients with gastric cancer. The clinical question was: Does early enteral nutrition improve clinical outcomes and nutritional aspects in patients with gastric cancer undergoing oncological surgery? The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in Scielo, Pubmed, Mdpi, Jboun, Ncbi, Science Direct, Hindawi, thus finding 21 articles, 10 being selected, which were evaluated by the CASPE critical reading tool, finally selecting the clinical trial titled as: “Effect of Early Enteral Nutrition Support Combined with Chemotherapy on Related Complications and Immune Function of Patients after Radical Gastrectomy”, which has a level of evidence AI and grade of recommendation strong, according to the expertise of the researcher. The critical comment allowed us to conclude that the early start of enteral nutrition could improve certain nutritional components such as total protein and blood albumin, level of immune cells, as well as total drainage through gastric tube and hospitalization time; although additional research is required to corroborate these results.

    Critical review: Influence of nitrite and nitrate consumption associated with the predisposition and development of gastric cancer

    The dietary component, specifically the excessive consumption of food additives in gastric cancer, is one of the main factors influencing the progression or clinical course of this neoplastic pathology. The present secondary research titled "Influence of Nitrite and Nitrate Consumption on the Predisposition and Development of Gastric Cancer" aimed to analyze the influence of nitrite and nitrate consumption on the development and progression of gastric cancer in adult patients. The search was performed in scientific databases such as PubMed, Scopus, EBSCO, and ProQuest, selecting 15 of the 50 reviewed articles due to their high methodological quality, meeting the exclusion criteria established by the researcher, using the CASPe tool. The meta-analysis selected was "Association Between Nitrite and Nitrate Intake and Risk of Gastric Cancer: A Systematic Review and Meta-Analysis," with an "AII" level of evidence and a "Strong" recommendation grade. It concluded that moderate or high consumption of nitrites increases the risk of developing gastric cancer by 12%, while nitrates were associated with a lower risk of gastric cancer progression, according to the critical review.

    Critical review: perioperative immunomodulatory enteral nutrition in patients with gastric cancer.

    The fifth most common neoplasm in Latin America is gastric cancer, which unfortunately compromises the absorption of nutrients and negatively impacts the immune system. The objective of this research was to carry out a critical review on whether enteral nutrition through the perioperative immunomodulatory diet will be beneficial in patients with gastric cancer. The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in Science Direct, Pubmed, Springer, 13 articles were selected that have been evaluated by the “Critical Appraisal Skills Program Spain Caspe” tool for critical reading, where finally a systematic review and meta-analysis titled as Impact of Preoperative Immunomodulatory Nutrition in Outcomes of Patients Undergoing Surgery for Gastrointestinal Cancer, a Systematic Review and Meta-analysis, which has a level of evidence A I and a Strong recommendation grade, according to the researcher's expertise. The critical commentary allowed us to conclude that immunomodulatory enteral nutrition is beneficial in patients with gastric cancer and that there is sufficient evidence to answer this question.

    Critical review: predictive value of the nutritional prognostic index in patients with gastric cancer.

    The present secondary research titled as critical review: predictive value of the nutritional prognostic index in patients with gastric cancer aimed to determine the predictive value of the nutritional prognostic index in the survival of patients with gastric cancer. The clinical question was: Will the predictive value of the nutritional prognostic index improve the survival of adult and elderly patients with gastric cancer? The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in Science Direct, PubMed, HINARI, Lilacs, Scielo, Dialnet, Redal, finding 60 articles, 11 being selected that have been evaluated by the CASPe critical reading tool, finally selecting the systematic review titled Prognostic Nutritional Index Predicts Outcomes of Patients after Gastrectomy for Cancer: A Systematic Review and Meta-Analysis of Nonrandomized Studies which has an AI level of evidence and a strong recommendation grade, according to to the expertise of the researcher. The critical comment allowed us to conclude that the nutritional prognosis index improves the overall survival of patients with gastric cancer; however, taking into account the biases exposed, attention must be paid to the data analyzed.

    Diagnostic performance of Platelet Indices in the identification of degrees of severity of Gastric Cancer in a hospital center, period 2021 – 2023

    Stomach cancer is a type of cancer that affects the stomach lining due to the uncontrolled proliferation of its cells, being considered by the WHO as one of the main global health issues. Timely diagnosis, follow-up, and evaluation of people at risk for stomach cancer are essential, and laboratories play a critical role in this process, offering tests for clinicians. The role of platelets in inflammatory processes related to neoplasms is currently being discussed, and it is likely that platelet activation by neoplastic cells contributes to metastasis and cancer progression by stimulating deep vein thrombosis, thrombo-inflammation, and the formation of neutrophil extracellular traps that hide cancer cells. The information provided by platelet analysis is important, as these values can indicate the severity level developed by a person with stomach cancer and potentially improve therapeutic strategies. However, these markers are still under investigation and should be complemented with clinical and epidemiological data to help forecast patient health and provide more personalized treatment.

    Effectiveness of laparoscopic distal gastrectomy compared to open distal gastrectomy in reducing intraoperative blood loss and hospital stay in patients with early gastric cancer

    Material and Methods: Systematic observational and retrospective quantitative review, subjected to critical selection, using the Grade evaluation system to identify the level of evidence, found in the following databases: Cochrane Library, British Medical Journal, PubMed, Lancet , Wiley Online Library, Journal of Rheumatology. The 10 articles reviewed systematically, 60% (n= 6/10) are Meta-analysis, 20% (n= 2/10) are cohort studies and 20% (n= 2/10) are randomized clinical trials. These studies come from the countries of China (50%), followed by Japan (10%), Australia (10%), Korea (10%), the USA (10%), and England (10%). Results: The evidence found, 100% (n=6/10) indicates that Laparoscopic Distal Gastrectomy is effective in reducing intraoperative blood loss and hospital stay in patients with early gastric cancer. Conclusion: Laparoscopic Distal Gastrectomy is effective in reducing intraoperative blood loss and hospital stay in patients with early gastric cancer.

    Effectiveness of laparoscopic gastrectomy versus conventional gastrectomy in patients with gastric cancer

    Objective: Analyze and systematize the evidence on the effectiveness of laparoscopic gastrectomy versus conventional gastrectomy in patients with gastric cancer. Materials and Methods: The type of research was quantitative, high-quality articles reviewed from systematic reviews and meta-analysis, the population was 25 articles; The sample was 12 scientific articles published and indexed in the scientific database, the database search tool: Scielo Pubmed, Ebsco and World Journal of Gastroenterology. The GRADE method was used for evaluation, which evaluated the quality of evidence and the strength of recommendation. Results: Of the 100%, 32% of studies correspond to Japan, Brazil and the USA, 18% Italy and 50% China, in relation to the type it was 100% quantitative and study designs 41% correspond to meta-analysis, 17 % to systematic review, randomized and retrospective clinical trial and 8% to cohort. Conclusions: 92% of the articles reviewed show the effectiveness of laparoscopic gastrectomy, because it presents less blood loss, fewer complications, shorter hospital stay, fewer post-surgical complications, 8% show that both laparoscopic and conventional gastrectomy are equally effective. Effective, however, laparoscopic gastrectomy cannot be routinely recommended over the conventional technique for the treatment of advanced gastric cancer.

    Effectiveness of laparoscopic versus robotic surgery for the care of patients with gastric cancer in the removal of tumors

    Objective: To systematize the evidence on the effectiveness of laparoscopic surgery versus robotic surgery for the treatment of gastric cancer patients in tumor removal. Materials and Methods: A quantitative research approach was used, and the study design involved a systematic review. The population consisted of 46 articles, with a sample of 10 scientific articles published and indexed in scientific databases. The search covered the following databases: Scielo, PubMed, Epistemonikos, Cochrane Plus, and EBSCO. The articles were evaluated using the GRADE system, which assessed the quality of evidence and the strength of recommendations. Results: Ten articles were analyzed in this study. These articles showed that the most commonly used method was meta-analysis, which accounted for a higher percentage (40%) compared to other research designs. Conclusions: It was concluded that 60% of the studies demonstrate that laparoscopic surgery was effective in the treatment of gastric cancer, as it showed the patient's quick recovery, was cost-effective, and resulted in fewer adverse effects, thus reducing the incidence of possible complications. However, 40% of the studies indicate that robotic surgery is a promising approach for the treatment of gastric cancer, particularly regarding safety, feasibility, technique, and oncological efficacy.

    Effectiveness of leaf consumption in reducing the risk of gastric cancer in adults

    Objective: To systematize the evidence on the efficacy of garlic consumption in reducing the risk of gastric cancer in adults. Material and method: This is an observational, retrospective study of the systematic review of full-text articles, which identified their degree or quality of evidence according to the Grade System. The population consisted of 10 articles obtained from the following databases: Scielo, Pubmed, Cochrane Library, Medline, Embace, Dialnet, and Lilacs. Of these, 70% were systematic review designs and 30% were randomized clinical studies, corresponding to the following countries: China 30%, United States 30%, Iran 20%, Italy 10% and Australia 10%. RESULTS: The review of 10 articles shows 100% efficacy in reducing the risk of gastric cancer in adults when consuming garlic. Conclusions: From the review of 10 articles, 10 out of 10 demonstrate the effectiveness of garlic consumption for reducing the risk of gastric cancer in adults; due to its bioactive sulfur compounds; it decreases the bioactivation of carcinogens, reducing the risk of suffering from it by half; due to the presence of antibacterials acting against another bacteria, for Helicobacter Pylori, present in the stomach; daily consumption is 2 to 5 grams of fresh garlic (a clove of garlic); allicin acts when chewed and thanks to its powerful biological effect; it is responsible for the benefits it brings to our health by protecting it from cancer.

    Estado nutricional en pacientes con cáncer gástrico con tratamiento de quimioterapia en el Instituto Regional de Enfermedades Neoplásicas IREN Centro – Concepción 2022

    En un paciente con cáncer gástrico el estado nutricional juega un papel muy importante para lograr la tolerancia y sobre todo completar el tratamiento, por ello el objetivo de este trabajo fue: Determinar el estado nutricional en pacientes con cáncer gástrico con tratamiento de quimioterapia en el Instituto Regional de Enfermedades Neoplásicas IREN – Centro, Concepción 2022, para lo cual, la metodología empleada es inductivo de diseño no experimental, descriptivo, transversal y retrospectivo; se recolectó información de 30 historias clínicas de pacientes que cumplían los criterios de inclusión. Los resultados mostraron que el 53,34% fueron de sexo femenino y el 46,66% fueron del sexo masculino, siendo la edad más frecuente de 58 a 68 años (36,67%), el estadio IV (60%), el 96,67% cuenta con SIS, el nivel de instrucción más frecuente es secundaria (43,33%) y el 66,67% tiene como tipo de alimentación dieta completa. Respecto al nivel de nutrición según el IMC, se tiene desnutrición moderada (10%), desnutrición leve (3,3%), normal (73,3%) y sobrepeso (13,3%), respecto a la variación de peso se tiene que el 4,3% tiene un buen estado nutricional, el 23,3% malnutrición moderada y el 33,3% malnutrición grave y un nivel de riesgo nutricional severo respecto al tipo de quimioterapia 20% (Folfox 6), 6,67% (Folfiri) y 6,67% (Flot). Conclusión: se evidenció que el 73,3% presentó un estado nutricional normal según el IMC, un buen estado nutricional el 43,3% según variación de peso después de la tercera sesión de quimioterapia y el 33,34% presentan malnutrición grave respecto al tipo de quimioterapia siendo el 20% con el tipo de esquema Folfox 6.

    Factores de riesgo de cáncer gástrico asociado al estilo de vida y medicación con inhibidores de bomba de protones en usuarios que acuden a la botica Ahorro y salud. Ate-Lima 2024

    El estudio tiene por finalidad determinar los factores de riesgo de cáncer gástrico asociados al estilo de vida y medicación con inhibidores de la bomba de protones en usuarios que acuden a la botica Ahorro y Salud. Ate- Lima 2024. El tipo de investigación fue básica, deductivo, no experimental y corte transversal. La población fue de 120 usuarios que acuden aproximadamente en 2 semanas a la botica Ahorro y Salud y la muestra fue 92 de dichos usuarios. Como resultados destacados se reportó que los usuarios que asisten a la botica Ahorro y Salud-Ate tenían un estilo de vida de consumo poco saludable (63,0%), donde los productos eran el tabaco, la sal y el alcohol, siendo estos factores de riesgo de cáncer gástrico. Asimismo, respecto a la medicación con inhibidores de bomba de protones de los usuarios que acuden a la botica Ahorro y Salud se evidenció que un 66% poseían adherencia al tratamiento con IBP, el Omeprazol fue el medicamento más prescrito (37,9%), siendo el reflujo gastroesofágico el síntoma más diagnosticado y por último la náusea fue la reacción adversa con mayor presencia en la muestra estudiada (13% - a veces). Se concluyó que los factores de riesgo de cáncer gástrico se encontraban asociados a un estilo de vida poco saludable y la medicación con omeprazol como inhibidor de la bomba de protones con mayor prescripción entre los usuarios que acuden a la botica Ahorro y Salud.     Nivel de conocimiento y calidad del autocuidado en pacientes con cáncer gástrico, clínica aliada, Lima, 2022

    Objetivo: “Determinar la relación que existe entre el nivel de conocimiento y calidad de autocuidado en pacientes con cáncer gástrico”, la población estará constituida por 100 pacientes con cáncer gástrico en la Clínica Aliada, Lima. Muestra: el total de la población, Diseño Metodológico: El tipo de investigación será aplicada. Diseño será observacional, descriptivo, correlacional, prospectivo y transversal. Instrumentos: Para evaluar la variable “Conocimiento” se utilizará un cuestionario validado en el 2021 por Basilio et al, confiable mediante Alpha de Cronbach de 0.680 y para evaluar la variable “El autocuidado del paciente con cáncer gástrico de la Clínica Aliada – Lima” se aplicará un cuestionario confiable mediante Alpha de Cronbach con índice de 0.830. La técnica de recolección de datos será la encuesta. Procesamiento estadístico y análisis de datos: se realizará mediante técnicas descriptivas y frecuencias expresadas en porcentajes, el análisis estadístico será según sea su distribución a través de pruebas estadísticas paramétricas (Correlación de Pearson) o no paramétricas (Correlación Rho de Spearman)

    Nutritional status assessment in patients with stomach cancer

    Nutritional interventions during hemodialysis help patients with chronic kidney disease improve their nutritional status and prevent malnutrition. The present secondary research, titled "Critical Review: Effect of Protein Supplementation on Albumin Levels in Adult Patients with Chronic Kidney Disease," aimed to analyze whether oral protein supplementation increases serum albumin levels in adult patients with chronic kidney disease undergoing hemodialysis. The clinical question was: Does protein supplementation improve serum albumin levels in patients with chronic kidney disease undergoing hemodialysis? Evidence-based nutrition (NuBE) methodology was used. The search was conducted in Medline and Cochrane databases, finding 21 articles, of which 8 were selected and evaluated using the CASPE critical reading tool. The final selected randomized clinical trial was titled "Intradialytic Protein Supplementation Reduces Inflammation and Improves Physical Function in Maintenance Hemodialysis Patients," which has an I evidence level and strong recommendation grade, according to the researcher’s expertise. The critical commentary concluded that protein supplementation in hemodialysis patients increases serum albumin levels and has other associated effects related to inflammation.

    Nutritional therapy in patients with gastric cancer.

    In Latin America, gastric cancer is the fifth most common neoplasm. Unfortunately, it is diagnosed in advanced stages, so nutrient absorption is affected and the risk of malnutrition increases, negatively affecting the immune system. The objective of the present review was to know the effect of nutritional therapy on the treatment and prognosis of patients with gastric cancer, to know what type of nutritional intervention has a better impact on the prognosis of patients with gastric cancer. The Evidence-Based Nutrition (NuBE) methodology was used. A systematic search of articles was carried out in a precise and non-repetitive manner using Scopus, ScienceDirect, Pubmed, Cochrane Library and Scielo as databases. Of which 86 articles were identified, 21 were selected for evaluation by the “Critical Appraisal Skills Program Spain Caspe” tool. Finally, the review article with the highest level of recommendation was chosen to write the critical comment. According to the level of recommendation, we can affirm that early intervention with enteral nutrition results in greater benefit and applicability in clinical practice even over parenteral nutrition, which confirms the importance of prescribing early nutrition in clinical practice. .

    Quality of care and emotional status in gastric cancer patients at a specialized institute in Lima, 2021

    Objective: Determine how the Quality of Care is related to the Emotional State in Patients with Gastric Cancer, from a specialized Institute in the city of Lima 2021. Material and method: The research responds to the hypothetical - deductive method, it has a quantitative approach, it is basic, applied, the population that constitutes it makes a total of 360 patients with Gastric Cancer who are in the Surgery service of a Specialized Hospital (INEN). ) of the City of Lima 2021, from this a sample of 65 was obtained, the interview technique and an instrument for each variable were used, for the first instrument the Servqual model was used, which consists of 21 Items and 5 dimensions (tangible elements, reliability, responsiveness, security and empathy) to measure Service Quality. For the second instrument, the Knowing My Emotions Test was used, which with 40 questions evaluates the emotional state in its five dimensions ( Socialization. Self-esteem, problem solving, happiness-optimism and emotion management, both with the ordinal Likert scale. Content validity was carried out and submitted to expert judgment; reliability was found by applying Cronbach's alpha reliability coefficient, obtaining a value of 0.95 for Quality of Care and 0.86 for Emotional State, assuming it was very reliable to be applied in the investigation. Data processing was done using the statistical package (SPSS) version 22.0, Excel was also used to extract data according to a matrix structured as a database in relation to the measuring instrument.

    Risk factors for gastric cancer in the adult population

    Methodology: It is a systematic review of 10 articles, 30% from Colombia, 20% from Peru, Mexico and Ecuador respectively and 10% from Panama. The majority of articles are case-control studies with 70% and 30% cross-sectional studies. These articles were obtained from the Pubmed and scielo databases. Results: the 10 articles reviewed show that the risk factors for gastric cancer are: 40% salt, 30% alcohol consumption, 20% having a family history, not using adequate refrigeration to preserve food and low level of education, as well as foods exposed to smoke, cooking with firewood, consuming reheated foods, low consumption of fruits and vegetables and tobacco respectively. And 10% have inadequate eating habits, physical inactivity and a sedentary lifestyle, stress, pharmacological habits, as well as low socioeconomic level, being over 50 years old, having H. Pylori, consumption of red pork meat and sausages. 3 times a week and be female. Conclusion: High consumption of salt, as well as alcohol, constitutes 70% of the risk factors for gastric cancer.
