Ítem Acceso abierto Association between the cardiotocographic parameters of the non-stressful test and pre-eclampsia in pregnant women treated at the San José Hospital, Callao, 2016 and 2017(Universidad Privada Norbert Wiener, 2020-10-16) Muñoz Molina, Eulalia Jesús; Manchego Guillen, Maria LuisaAn observational, retrospective, and cross-sectional study was conducted, reviewing 118 medical records of pregnant women with preeclampsia who attended the Fetal Medicine Unit at San José Hospital, Callao, for a non-stress test (NST). The results showed that 80.5% of the pregnant women had preeclampsia without severe criteria, and 19.5% had severe criteria. Baseline fetal heart rates of 120-160 bpm were observed in 97.5% of cases, 72.9% of variability ranged from 5-9 or >25 bpm, 72.9% of accelerations were ≥5 bpm, 87.3% showed absent decelerations, and 96.6% had fetal movements ≥5. Only variability showed slight alteration as it did not fall within the normal range. NST reactivity was present in 95.8% of cases, 3.4% were non-reactive, and 0.8% were pathological. A statistically significant association was established between fetal movements as the main CTG parameter of the NST and preeclampsia without severe criteria, with a significance level of 0.013 < 0.05. NST results also showed a significance level of 0.034 < 0.05, statistically associated with preeclampsia without severe criteria. In conclusion, the parameter affected by preeclampsia without severe criteria is variability, while fetal movements and NST results demonstrate statistical association.Ítem Acceso abierto Cardiotocographic findings in a non-reassuring fetal state. She was hospitalized, and labor induction was initiated, with an initial Non-Stress Test (NST) of 9/10. After one day of induction with stationary progression and a Stress Test (ST) resulting in 4/10, which was diagnosed as a non-reassuring fetal status, an emergency cesarean section was performed. A newborn with an APGAR score of 6 at one minute and 8 at five minutes was delivered. Gestational age according to Capurro was 41 weeks, with the presence of meconium-stained amniotic fluid. The clinical case is of a pregnant woman who was admitted in an emergency with a history of fetal death and diabetes mellitus, diagnosed with: 29 weeks of gestation, vulvovaginitis, threatened preterm labor, and diabetes due to history. Laboratory tests confirmed uncontrolled diabetes, diabetic ketoacidosis, and urinary tract infection. Multidisciplinary treatment was initiated, with strict glycemic control and monitoring of fetal well-being. The initial Non-Stress Test showed a non-reactive pattern with silent variability, absence of accelerations, and decelerations. The clinical evolution of the patient was stable, but as metabolic control improved, variability improved, and a reactive pattern was obtained. The patient stayed for 45 days and after a medical meeting, a cesarean section was performed at 35 weeks, resulting in a female newborn weighing 2710 grams, with an APGAR score of 8 at one minute and 9 at five minutes. The patient was discharged in stable condition 48 hours after the cesarean. Conclusion: The usefulness of the Non-Stress Test for monitoring fetal well-being in diabetic pregnant women is evident.
