Methodology: The systematic review is a type of observational, retrospective design widely used in research, a selection of 10 articles was made. Of 10 articles reviewed, according to the quality of evidence, 100% of the articles are of high quality. Results: Of 100% of the articles, 80% of the studies show that Alteplase is effective in adult and elderly patients. It is recommended to use it up to 4.5 hours after the onset of symptoms, it is beneficial and safe; It should be administered intravenously. Patients have fewer cases of mild intracerebral hemorrhage within 10 days, less symptomatic intracerebral hemorrhage within 36 hours, and less major systemic hemorrhage. The 20% according to the European Union label should be given before 4.5 hours since it would increase the net benefit of alteplase in 90 days, without an increase in mortality. Conclusions: It is concluded that the use of Alteplase intravenously, before 4.5 hours, is effective; in early thrombolysis for the management of ischemia in adult and elderly patients with stroke.
