    Conocimiento sobre los cuidados con traqueostomía y práctica de enfermería en los servicios de uci de una clínica privada de Lima, 2024

    El objetivo general de la presente investigación será determinar cómo el conocimiento sobre el cuidado del paciente con traqueostomía se relaciona con la práctica del personal de enfermería en los servicios de UCI de una clínica privada de Lima, 2024. La metodología adoptada se basará en el enfoque cuantitativo, método hipotético-deductivo, el tipo aplicada, diseño no experimental, corte transversal y alcance correlacional. La población y muestra estará comprendida por 70 enfermeros de la UCI de una clínica privada. La recolección de datos será a través de la aplicación de dos instrumentos previamente validados: guía de cuidados de enfermería en pacientes con traqueostomía y Cuestionario del conocimiento sobre el cuidado del paciente con traqueostomía. Para el análisis e interpretación de los datos se empleará el estadístico de Rho de Spearman o el de Pearson previa aplicación de la prueba de normalidad, para el análisis descriptivo se utilizarán tablas de frecuencias y gráficos.

    Effectiveness of early tracheostomy compared to late tracheostomy in reducing pneumonia in patients with prolonged mechanical ventilation.

    Objective: Analyze the evidence on the efficacy of early tracheostomy compared to late tracheostomy to reduce pneumonia in patients with prolonged mechanical ventilation. Materials and Methods: It was a systematic review, with the search restricted to full-text articles, subjected to a critical reading using the Jover evaluation to identify their degree of evidence; of the total 100% reviewed scientific articles, 60% were from China, 20% from Brazil, 10% from the United States, and 10% from Belgium. Furthermore, the reviewed scientific articles were all systematic reviews, 100% from China, the United States, and Belgium. Results: According to the findings of the reviewed studies, 60% showed that early tracheostomy is more effective than late tracheostomy in reducing pneumonia in patients with prolonged mechanical ventilation. Moreover, 40% showed that both early and late tracheostomy are effective in reducing pneumonia in patients with prolonged mechanical ventilation. Conclusions: Therefore, the evidence from the reviewed articles, 6/10, shows that early tracheostomy is more effective than late tracheostomy in reducing pneumonia in patients with prolonged mechanical ventilation; it is also associated with shorter hospital stays, fewer days on mechanical ventilation, and lower mortality rates.

    Effects of cardiorespiratory physiotherapy on physical deconditioning in tracheotomized patients, Intensive Care Unit - Hospital III Iquitos, 2024

    The intensive care unit (ICU) provides life support and treatment to critically ill patients who require assisted mechanical ventilation, invasive monitoring, cardiovascular support, and prolonged prescribed or unavoidable rest. This creates limitations, deficiencies, and disabilities during their stay, developing pathological conditions related to the cardiovascular, respiratory, and musculoskeletal systems. This increases mortality in patients hospitalized with complications of functional disability and generalized muscular disorders, decreasing their quality of life after hospital discharge. The American Thoracic Society associates ICU-acquired weakness (ICUAW) with generalized weakness in the limbs. It generates multiple impacts, such as in the nervous system, with critical patient polyneuropathy and skeletal muscle dysfunction. ICUAW and neuromuscular dysfunction are predictors for difficulties in weaning from mechanical ventilation (MV). Neuromuscular blockers and corticosteroids are related to ICUAW. This results in difficulties in weaning from MV and consumes approximately 40% of ICU resources. Mechanical ventilation is the most common reason for ICU admission, and when prolonged and unable to be withdrawn, tracheostomy is suggested as the only alternative. Current evidence recommends integrating an exercise program within the ICU, reducing the rate of functional disability.

    This research will be carried out within the framework of the specialty of nursing in intensive care and will focus on the knowledge and practices of this professional group regarding endotracheal aspiration of patients receiving mechanical ventilation, due to the presence of pneumonia caused by hospital-acquired infections that put their lives at risk and require actions that provide them with an opportunity to survive.

    Tracheostomy is a surgical procedure in which an opening is made in the trachea to prevent or relieve airway obstruction and/or serve as access for suctioning, mechanical ventilation, and other modes of oxygen delivery. Objective: Determine the relationship between knowledge and nursing care in tracheostomized patients in the Intensive Care Unit at a National Hospital in Lima, 2022. Methods: The research method will be hypothetical-deductive, the approach is quantitative, the type is descriptive. and design is correlational; The population and sample subject to study will be made up of 106 nurses; the sampling being non-probabilistic for convenience and intentional.
