Julio–diciembre 2014(Universidad Privada Norbert Wiener, 2015) Vega Silva, Yohana Gisella; Caldas Herrera, EmmaThe present thesis aims to determine adherence to thromboprophylaxis according to the Clinical Practice Guideline for venous thromboembolic disease, which has been identified as a principal factor of hospital morbidity and mortality. Delays in the development and implementation of clinical guidelines can result in high mortality among medical and surgical patients. Therefore, rapid evaluation of risk factors, prompt response, and adherence to the guideline are required for effective thromboprophylaxis. Reviewed studies show that the use of thromboprophylaxis guidelines has not been consistently implemented in patients meeting the criteria for treatment, despite the fact that effective prophylaxis improves outcomes. This underscores the importance of identifying the main risk factors or criteria for thromboprophylaxis. Objectives: To determine adherence to thromboprophylaxis according to the Clinical Practice Guideline for venous thromboembolic disease in hospitalized patients at Clínica Internacional, San Borja branch, from July to December 2014. Subsequently, to establish the percentage of hospitalized patients receiving pharmacological and non-pharmacological treatment according to the institutional clinical practice guideline for venous thromboembolic disease. Materials and Methods: A retrospective, descriptive statistical methodology was applied to hospitalized patients at Clínica Internacional, San Borja branch. The analysis material consisted of patient medical records, using the decision algorithm from the thromboprophylaxis guideline as the instrument. Each day of the patient’s hospital stay was reviewed, obtaining specific and relevant data for the study. Results were tabulated and presented using statistical graphs. Data processing was performed using SPSS 20 and Microsoft Office Excel 2010. Results: A total of 320 complete clinical records from July to December 2014 were collected, organized, and analyzed. Of these, 257 patients (80.3%) were medical and 63 patients (19.7%) were surgical. Risk stratification showed 116 patients (36.3%) as low risk, 133 (41.6%) moderate risk, and 71 (22.2%) high risk. Thromboprophylaxis was administered to 154 patients (48.1%). According to surgical specialty, prophylaxis was given to 25 patients (16.2%) in general surgery, 11 (7.1%) in traumatology, 27 (17.5%) in gynecology, 1 (0.6%) in neurosurgery, and 10 (6.5%) in urology. Pharmacological prophylaxis was administered to 59 patients (38.3%). The clinical guideline was applied in 285 patients (89.1%): 226 medical (70.6%) and 59 surgical (18.5%). The probability of guideline use was higher in medical patients than in surgical patients. Conclusions: Thromboprophylaxis has proven to be an effective method to prevent venous thromboembolism in hospitalized patients according to the criteria established in the institutional clinical practice guideline. It was observed that 285 patients (89.1%) complied with thromboprophylaxis, while 35 patients (11.4%) did not, based on guideline criteria. Proper use and dissemination of the guideline for implementing thromboprophylaxis in the appropriate patients significantly reduces thrombotic events in both medical and surgical hospitalized patients.Publicación Acceso abierto Efectividad de la compresión neumática intermitente para la prevención de tromboembolismo venoso en los pacientes de unidad de cuidados intensivos(Universidad Privada Norbert Wiener, 2018-11-25) Cervantes Nina, Miguel; Calsin Pacompia, WilmerLa recolección de datos se realizó a través de la revisión bibliográfica de artículos de investigaciones tanto nacionales como internacionales que tuvieron como tema principal la efectividad la compresión neumática intermitente para la prevención tromboembolismo venoso en los pacientes de la unidad de cuidados intensivos; de todos los artículos que se encontraron, se incluyeron los más importantes según nivel de evidencia y se excluyeron los menos relevantes. Se estableció la búsqueda siempre y cuando se tuvo acceso al texto completo del artículo científico.Publicación Acceso abierto Eficacia de la compresión neumática intermitente para la prevención de tromboembolismo venoso en pacientes postoperados(Universidad Privada Norbert Wiener, 2020-01-19) Ñahuincopa Benito, Pilar Rocio; Pretell Aguilar, Rosa MaríaObjetivo: Sistematizar las evidencias sobre la eficacia de la comprensión neumática intermitente para la prevención de tromboembolismo venoso en pacientes postoperados. Material y Método: La revisión sistemática consta de 10 artículos científicos los que fueron obtenidos de la siguiente base de datos: Pubmed, Epistemonikos, Journals books, Cochrane Biblioteca. De los 10 artículos, el 100% son de alta calidad, el 60% son revisión sistemática, el 20% son metaanálisis y 20% son ensayos controlados aleatorizados. De acuerdo con la fuerza de recomendación es fuerte al 100%, según el país donde se ejecutaron las investigaciones corresponde a EE. UU el 50%,10% en Singapur, Irlanda, China, Londres y España. Resultados: De los 10 artículos revisados, el 80% demuestran que la compresión neumática intermitente es eficaz para la prevención de tromboembolismo venoso en pacientes postoperados. Por otro lado el 20% demuestran eficacia al uso de un enfoque combinado para la prevención de tromboembolismo venoso en pacientes postoperados. Conclusiones: 8 de los 10 artículos demuestran la eficacia de la compresión neumática intermitente en la prevención de tromboembolismo venoso en pacientes postoperados y 2 de 10 demuestran que la compresión neumática intermitente no fue eficaz por sí sola para la prevención de tromboembolismo venoso en pacientes postoperados.
