Effectiveness of cleaning and disinfecting the laryngoscope in a surgical center to reduce complications due to infections
(Universidad Privada Norbert Wiener, 2020-02-08) Acuña Guerzoni, Liliana Patricia; Espinoza Cordero, Tula del Carmen; Pretell Aguilar, Rosa María
Objective: Systematize evidence on the Effectiveness of cleaning and disinfection of the laryngoscope in a Surgical Center to reduce complications due to infections. Materials and Methods: The systematic review consists of 10 scientific articles. According to the type of study design, 50% (n=5/10) is a systematic review, 30% (n=3/10) is an experimental study, 10% (n=1/10) is a cohort study and 10% is a cohort study. (n=1/10) descriptive study, which agrees with the expected result in the present study. According to the quality of evidence, it was found to be high in 80%, moderate in 10% and low in another 10%, depending on the country where the investigations were carried out, they were 30% in Brazil, 30% in the United States, 30% India and 10% Turkey. To find these studies, the following scientific research search engines were used: Epistemonikos, Researchgate, Pubmed and Google academic. Results: Of the 10 investigations analyzed, it is evident that 70% (7/10) demonstrate the Effectiveness of cleaning and disinfecting the laryngoscope in the Surgical Center to reduce complications due to infections. Conclusions: According to the research analyzed, it was obtained that 70% (n = 7/10) of the 10 articles demonstrate that there is effectiveness of cleaning and disinfection of the laryngoscope in the Surgical Center for the reduction of complications due to infections and 30% ( n=3/10) show the cleaning and disinfection of the laryngoscope in the Surgical Center as ineffective in reducing complications due to infections, therefore, it is recommends using disposable laryngoscope blades to help reduce the spread of nosocomial infections, thus avoiding complications.

Objective: Determine the effectiveness of continuous versus intermittent enteral nutrition in intensive care patients. Materials and methods: The present study is quantitative with a Systematic Review, observational and retrospective design. Results: in the final selection, 6 articles were chosen, we found that 33.3% (2) correspond to Brazil, with 16.6% (1) we found the USA, China, Turkey respectively, most of them have been studied clinical trials with 50%, 33.6% experimental and among the systematic review we find Spain with 16.6%(1), of the total articles analyzed, 50% mention that continuous enteral nutrition is not more effective than intermittent enteral nutrition nor vice versa. Conclusions: according to the studies reviewed, it could not be defined which of them is the most effective, with 50% (3/6) mentioning that there are no statistically significant differences. 33.3% (2/6) conclude that in intermittent enteral nutrition they achieve better caloric intake and in less time compared to continuous enteral nutrition; Another study that corresponds to 16.6% (1/6) mentions that diarrhea and lung infections are more frequent with intermittent enteral tube feeding compared to continuous nutrition and finally in the review (18), concludes that enteral nutrition Intermittent therapy obtains better results both in nutritional status and in reducing bronchial aspiration.

They were analyzed according to the Grade system to determine the strength and quality of the evidence. Of the 10 articles, 40% (4/10) correspond to the methodological design of randomized controlled trials, 20% (2/10) systematic reviews, 10% (1/10) experimental, 30% (3/10 ) are cases and control. 30% of articles come from the US and 10% are found in Nigeria, Canada, China, Spain, Türkiye, the Republic of Macedonia and Ethiopia. Results: According to the evidence found, 100% of the authors demonstrate that the use of double gloves is effective as a surgical barrier in the surgical center team, avoiding the perforation of the gloves during surgical procedures, thus reducing contact with blood and fluids. body fluids. Conclusions: Of the 10 articles reviewed, 10 indicate that the use of double gloves is effective as a surgical barrier for surgical center equipment, preventing perforation of the gloves during surgical procedures and thus reducing proximity to fluids.

Efficacy of a closed suction system compared with open suction system on hemodynamic changes in patients with mechanical ventilation
(Universidad Privada Norbert Wiener, 2017-12-03) Ganto Yarasca, Rosario; Bonilla Asalde, César Antonio
Objectives: Systematize the evidence on hemodynamic changes in patients with mechanical ventilation as a consequence of the use of the closed aspiration system and the open aspiration system. Methodology: Observational and retrospective systematic review, the research has been restricted to articles with full text, the selected articles were subjected to a critical reading, using Jover's evaluation to identify their degree of evidence. Results: After the review, very diverse related research was obtained. In the final selection, 10 articles were selected corresponding to Brazil and Iran with 3 articles each, Turkey with 2 and Australia and Holland with 1 article each comparatively. Conclusions: It was shown that in the studies analyzed there is evidence on the effectiveness of a system closed suction system compared to the open suction system in the hemodynamic changes of patients with mechanical ventilation, 60% (6/10) demonstrated an incidence in hemodynamic changes, while 40% (4/10) do not accept hemodynamic changes or no significant changes were recorded in the use of both systems. Recommendations from consensus and specialists suggest the importance of monitoring hemodynamic changes that may occur in patients during the aspiration procedure, regardless of whether it is performed with the closed system or the open system to prevent complications. They recommend the closed aspiration system as safer. And they clarify that nursing staff should not perform aspiration as part of a scheduled routine due to possible adverse effects.
