To determine the relationship between self-medication practices and treatment adherence in patients visiting a private pharmacy service in Lima, 2024. Materials and Methods: This was an observational, cross-sectional study involving 364 patients at a private pharmacy in Lima. Sociodemographic data and self-medication practices were collected through a structured questionnaire. Treatment adherence was assessed using a validated scoring method for this population. The Shapiro-Wilk test was applied to assess the normality of variables, and Spearman’s correlation was used to analyze the relationship between self-medication and adherence, considering a significance level of p < 0.05. Results: Most patients were women (64.84%), adults aged 31 to 49 years (50%), and of Peruvian nationality (82.97%), with basic education predominating (62.91%) and a large percentage employed (71.43%). A regular self medication practice was reported by 96.43% of patients. The median adherence score was 35 points (range: 18–45). A significant positive correlation was found between self-medication and adherence (rho = 0.218; p < 0.001), suggesting a slight trend towards lower adherence among those practicing self-medication. Conclusions: A significant positive relationship was observed between self-medication and treatment adherence in this population, though the correlation was mild. Educational strategies are recommended in pharmacies to promote safe medication use and improve treatment adherence.
