(Universidad Privada Norbert Wiener, 2015) Arnao Rojas, Verónica; Ramírez Mendoza, Tania Alejandrina; Herencia Torres, Victor Reynaldo
The present thesis, entitled Characteristics of Adverse Drug Reactions in Patients Under Highly Active Antiretroviral Therapy at the San José Maternal and Child Center Program in Villa El Salvador, 2006–2010, is an observational, descriptive, retrospective, and cross-sectional study. This thesis considered the antiretroviral drugs available at the Center for the Care and Treatment of Sexually Transmitted Infections and HIV/AIDS (CATITSS), including nucleoside reverse transcriptase inhibitors, non-nucleoside reverse transcriptase inhibitors, and protease inhibitors. A total of 420 clinical records of HIV-positive patients who were admitted to CATITSS at the San José Maternal and Child Center and met the inclusion criteria were reviewed. Among these, 268 patients presented one or more adverse reactions to antiretroviral drugs. The method used involved data collection from patient medical records; for each patient, a report form for suspected adverse reactions to antiretroviral drugs was completed. These reports were subsequently evaluated using the Peruvian pharmacovigilance algorithm. Regarding the results, the characteristics of adverse reactions to antiretroviral drugs by sex were 56.3% male and 43.7% female. By age group, 29.5% were 18–25 years, 36.6% were 26–32 years, 36.6% were 33–39 years, 7.1% were 47–53 years, and 7.8% were 54 years and older. By organs and systems affected, 44.78% involved the peripheral nervous system, 42.16% were related to metabolic and nutritional disorders, and 32.09% affected the skin. By type, the most frequent was type C at 89.93%, followed by type A at 61.19%. According to causality, 95.9% were classified as probable cases. Regarding severity, 52.10% were non-serious, 47.41% were serious, and only 0.49% were severe. According to the ATC classification of antiretrovirals, 97.28% of nucleoside reverse transcriptase inhibitors produced adverse drug reactions.

Identificación de problemas relacionados con los medicamentos en pacientes hospitalizados en el servicio de Medicina Interna: Julián Arce y San Andrés del Hospital Nacional Dos de Mayo (junio–julio 2013)
(Universidad Privada Norbert Wiener, 2013) Quispe Hurtado, Virginia Milagros; Vega Aquino, Luis Martín; Herencia Torres, Victor Reynaldo
The National Hospital Dos de Mayo has five Internal Medicine Services. Patients admitted to these services usually present with acute and chronic pathologies, comorbidities, are of variable ages, and are often polymedicated. Many of these patients may develop Drug-Related Problems (DRPs) during treatment, which may be due to various causes such as subtherapeutic or toxic doses, adverse reactions, or drug interactions. These situations predispose patients to the occurrence of DRPs, which must be identified through validated methods, as they may affect the achievement of therapeutic goals, prolong hospital stays, increase morbidity, and raise the costs associated with treatment. The general objective of this study was to determine drug-related problems (DRPs) in hospitalized patients in the Internal Medicine Services Julián Arce and San Andrés of the National Hospital Dos de Mayo (June–July 2013). The specific objectives were to determine potential and actual DRPs in hospitalized patients in the aforementioned services; to describe actual DRPs related to need, effectiveness, and safety; and to identify the medications involved according to the Anatomical Therapeutic Chemical (ATC) classification. The study determined 164 (71%) actual DRPs and 67 (29%) potential DRPs. Among the actual DRPs, 50 (30.5%) were related to need, 49 (29.9%) to effectiveness—the lowest proportion—and 65 (39.6%) to safety, representing the highest proportion. In total, 151 medications were identified as being implicated in actual DRPs, with the most frequent therapeutic groups being systemic anti-infectives (29%) and cardiovascular therapy (27%). The findings highlight the importance of recognizing DRPs, defined as health problems understood as negative clinical outcomes derived from pharmacotherapy, which, caused by various factors, lead either to the failure to achieve therapeutic goals or to the occurrence of undesirable effects (Second Granada Consensus, 2002). In conclusion, the study established that actual DRPs predominate over potential ones, with safety-related DRPs being the most frequent. The large number of medications implicated, particularly systemic anti-infectives and cardiovascular drugs, underscores the need for closer monitoring. It is therefore recommended that medication reconciliation and patient education regarding medication use at the time of discharge be implemented, not only to develop the concept of pharmaceutical care but also to address a crucial need in hospitalized patients. Access to reliable and independent information sources, as well as critical evaluation of the literature on medicines, is essential to improve the quality of pharmacotherapeutic follow-up, particularly in relation to pharmaceutical interventions. Furthermore, pharmaceutical care should include interventions related to diet, physical activity, prevention of medication errors, and primary and secondary prevention through auxiliary diagnostic tests. Finally, the active participation of pharmacists in medical rounds fosters the recognition of their role within the healthcare team and encourages ongoing professional development, ensuring that pharmaceutical interventions are both timely and effective.

Identificación de reacciones adversas medicamentosas por el uso de antibióticos en pacientes hospitalizados en el Hospital Nacional María Auxiliadora, periodo 2017–2018
(Universidad Privada Norbert Wiener, 2019-03-06) Luján Curahua, Cristy Sadith; Herencia Torres, Victor Reynaldo
The objective of this research was to determine the percentage of adverse drug reactions (ADRs) from antibiotic use in hospitalized patients at the María Auxiliadora National Hospital in Lima, Peru. For this purpose, a descriptive, cross-sectional, and prospective study was designed, and the sample consisted of all 123 event notification forms related to ADRs from the period of 2017–2018. The results showed that 40.6% of reported ADRs were dermal. Of these, 70% occurred in women, 43.1% were attributed to antibiotics, and 74% were mild. Furthermore, 72% of these dermal ADRs were reported by a pharmaceutical chemist, with the largest proportion occurring in the gynecology-obstetrics department. Within the group of ADRs associated with antibiotics, 19% were related to Beta-lactams. The conclusions were that 40% of all reported ADRs in the hospital were dermal, and the highest percentage of reactions associated with antibiotics occurred in the medicine service. Additionally, 14.6% were dermal reactions associated with antibiotics. It was observed that the pharmaceutical chemist was the professional who reported most frequently, accounting for 77% of notifications. Half of the notifications were mild, followed by moderate reactions at 42%. Dermal drug reactions are frequently observed in daily medical practice, and a significant number of patients require hospitalization, with some associated variables such as age and severity being noted.
