The majority have been analyzed: 40% descriptive studies, 20% experimental studies and 10% systematic reviews. Results: 90% of the studies show that peritoneal dialysis is a good option in the management of refractory heart failure, and 60% show benefits in functional class, improvement in costs and effectiveness, decrease in mortality, decrease in hospital admissions, it also achieves a positive impact on the quality of life reported by the patient. Conclusions: peritoneal dialysis benefits the patient in functional class in 60% of patients with heart failure.Ítem Acceso abierto Effective interventions for the control of congestive heart failure in emergency units(Universidad Privada Norbert Wiener, 1905-07-09) Avila Salazar, Jesela; Huaman Grandez, Maria Liseth; Gamarra Bustillos, CarlosObjective: Establish effective interventions for the control of congestive heart failure in emergency units. Materials and methods: A systematic review was carried out, which synthesizes the results of multiple primary investigations. They are an essential part of evidence-based nursing due to their rigorous methodology, identifying relevant studies to answer specific questions of clinical practice. Results: 10 systematically reviewed scientific articles on effective interventions for the control of congestive heart failure in emergency units were included in the review. The articles were located in the Lilacs, Medline, Ebsco and Pubmed databases; 70% (n = 07/10) of the articles reviewed agree and show that the administration of ASA diuretics, which act on the medullary portion of the loop of Henle, inhibiting sodium reabsorption in the thick ascending portion of the loop of Henle, Henle; belonging to Group II such as: furosemide, bumetanide, torsemide, ethacrynic acid, in bolus or infusion, are the most used; on the other hand, 30% of the articles reviewed (03/10) of the authors agree that educational interventions based on formative education is effective in improving the control of congestive heart failure. Conclusion: The articles reviewed show that the most effective intervention for heart failure in older adults is ASA diuretics and educational interventions based on training education are effective in improving the control of congestive heart failure in emergency units. .Ítem Acceso abierto Effectiveness of continuous versus bolus injection of diuretics in congestive heart failure(Universidad Privada Norbert Wiener, 2018-07-08) Arauco Cuyutupa, Felix Jose; Perea Laguna, Jonatan Josue; Pretell Aguilar, Rosa MaríaMaterials and Methods: The present study is a Systematic review, the search has been restricted to articles with full text and the selected articles were subjected to a critical reading, using the GRADE evaluation to identify their level of evidence. 10 articles were chosen, which correspond to 100% (10/10) of the articles, 40% (4/10) correspond to the United States, 30% (3/10) correspond to Italy and 10% (1 /10) correspond to Taiwan, Portugal and Malaysia respectively; Likewise, 10 pieces of evidence are determined (100%): 2 systematic reviews (20%), 2 meta-analysis (20%), 3 randomized controlled trial (30%), 2 open randomized trial (20), 1 cross-sectional descriptive trial (10 %). Results: 100% (7/10) of the articles showed the effectiveness of the continuous administration of diuretics because it was associated with a high reduction in body mass, an increase in total urine production in 24 hours, and a significant increase in of additional hospital treatment, was the choice in patients with worse clinical status such as low systolic blood pressure, more severe kidney injury, where the kidney alteration was gradually worse, and the incidence of hydroelectrolyte imbalance decreased. Materials and methods: A search was carried out for systematic reviews that are the reference standard for the synthesis of health evidence. The population was made up of 20 articles, of which through selection criteria such as: Evidence no older than 10 years, the degree of evidence of medium or high quality of which 10 articles were chosen that contain the main premises of the problem. in study. Results: The present systematic review consists of 10 scientific evidence linked to the effectiveness of exercise-based cardiac rehabilitation in reducing hospital readmissions and/or mortality in adult patients with heart failure. Of the 10 articles reviewed, 70% are systematic reviews. and 30% is a systematic review and meta - analysis. 100% of the evidence is of high quality. According to the country of origin, 70% of the evidence comes from the United Kingdom and 30% from Spain. In this sense, 80% demonstrate the effectiveness of exercise-based cardiac rehabilitation in reducing hospital readmissions as well as long-term mortality in adult patients with HF. 20% do not show effectiveness of exercise-based cardiac rehabilitation in reducing hospital readmissions and/or mortality in adult patients with HF. Relevant studies were identified in the following databases: LIPECS, MEDLINE, LILACS, EBSCO, PUBMED, and COCHRANE PLUS. The research was limited to full-text studies, and the selected articles were subjected to an exhaustive reading, using the GRADE system to assess the quality of the evidence. Results: In the final selection, 10 articles were chosen, of which the majority were systematic review studies (70%), followed by clinical trials (20%). Additionally, 10% of the studies were meta-analyses. On the other hand, 90% (N=09/10) show that telemonitoring in patients with heart failure is effective in reducing hospital readmissions, decreasing mortality, and improving the quality of life of patients with heart failure. Conclusions: The studies considered in this research demonstrate the effectiveness of home telemonitoring in patients with heart failure. Materials and Methods: Systematic review of 8 articles which allow us to analyze the positive impact of the educational intervention provided by the nursing professional to patients with heart failure. In the final selection, 8 articles were chosen, we found that 37.5% (3) correspond to Colombia, 25% (02) to Brazil, 25% (02) to Spain, while with 12.5% (01) we find Mexico . Results: In this systematic review of the total number of articles analyzed, 100% affirm that nursing educational interventions are effective for the self-care of patients with heart failure. The nursing educational intervention must be individualized and comprehensive when addressing the patient with Heart Failure, promoting self-care and containing aspects such as weight control, salt and fluid restriction, medications, exercise, diet and symptoms of worsening of the disease. Materials and Methods: The design was observational, retrospective, a systematic bibliographic search was carried out in the Virtual Health Library, Scielo, Pub Med, Medline, Cochrane Plus. 10 articles were identified, of which 50% (5) correspond to the United States. , 30% (3) corresponds to Germany, 10% (1) corresponds to Australia and 10% (1) corresponds to the United Kingdom with no more than ten years of experience, the search has been limited to complete scientific texts, which were subjected to a critical interpretation, for this purpose the Grade system is used to indicate the recommendation value. Results: 60% (6) reviewed articles show that the ventricular assist device as a replacement for cardiac function in adults is effective and 40% (4) of the reviewed articles show that there are no favorable data in relation to the assist device ventricular assist device as a replacement for cardiac function in intensive care patients, since future studies still need to be carried out Conclusions: It is evident that the articles reviewed demonstrate the effectiveness of the ventricular assist device as a replacement for cardiac function, improving quality of life and prolongation of its existence.Ítem Acceso abierto Effectiveness of ultrafiltration compared to diuretic use for reducing weight loss and fluid elimination in patients with decompensated heart failure(Universidad Privada Norbert Wiener, 2020-02-01) Herrera Córdova, Jorge Victor; Mejia Fernandez, Rojsana; Matta Solis, Eduardo PercyObjective: Systematize the evidence on the effectiveness of ultrafiltration compared to the use of diuretics to reduce weight loss and fluid elimination in patients with decompensated heart failure. Materials and Methods: the design used was a systematic review. 29 articles were reviewed and the population of 10 international scientific articles published in the Database was taken. Elsevier, Sciencedirect, PubMed, Researchgate and Scielo. For the analysis of the Systematic Review, a summary table was prepared (Table Nº1). To determine the quality of the evidence and the strength of the recommendation, the GRADE table was prepared (Table Nº2). The scientific evidence reviewed was evaluated critically and rigorously, respecting and applying ethical and bioethical principles. Results: 100% (10/10) indicate that ultrafiltration is more effective than the use of diuretics to reduce weight loss and fluid elimination in patients with decompensated heart failure. Material and methods: Systematic observational and retrospective review, subjected to critical reading and the GRADE evaluation system was used to identify the degree of evidence of the articles published in the following databases: Epistemonikos, Infomed and Pubmed. Of the 11 articles systematically reviewed, 100% were of high quality as described below: 82% (n= 9/11) are Systematic Review, 9% (n=1/11) are experimental and 9% ( n=1/11) is a meta-analysis and comes from the following countries: England (27.2%), Australia (18.2%), United States (18.2%), Japan (9.1%), Cuba (9.1%) and Netherlands (9.1%) and Taiwan (9.1%). Results: 100% (11/11) (10/12) indicate that outpatient exercise is effective in improving the quality of life of patients with Chronic Heart Failure. Materials and Methods: The design used was a retrospective and observational systematic review, which uses a specific method to summarize and synthesize the results of scientific articles. The population was made up of 40 scientific articles of which 10 articles that represent the sample were selected. Results: In the 10 selected articles, 40% (n=2) were carried out in Brazil, 20% (n=2) in Germany, 10% (n=1) in the United Kingdom, 10% (n=1) in the United States. United States, 10%(n=1) in Colombia and 10%(n=1) in Canada. Most of them were Systematic Review studies in 50% (n=6), Randomized Controlled Trial 30% (n=3), Meta-analysis 10% (n=1), quasi-experimental 10% (n=1). Of 10 pieces of evidence, 90% (n=9) affirm that self-care improves quality of life in patients with heart failure and 10% (n=1) does not show improvement in quality of life. Materials and Methods: A systematic review of international research studies was conducted, obtained through searches in databases such as Epistemonikos, NCBI, PubMed, Elsevier, Google Scholar, and Scielo. These studies were selected and analyzed using the GRADE system to determine their respective levels of evidence. Articles were identified using DECS and MESH descriptors for searches in Spanish and English, respectively. Among the reviewed articles, 50% (5/10) are from the USA, 30% (3/10) from China, and 20% (2/10) from England and Japan. Results: Seventy percent of the studies reviewed indicate that CPAP treatment is effective in patients with obstructive sleep apnea and heart failure, while 30% show that CPAP treatment is ineffective in such patients. Methodology/Methods: Observational and retrospective systematic review, the search has been restricted to articles with full text, and the selected articles were subjected to a critical reading, using Jover's evaluation to identify their level of evidence. Results: In the final selection, 11 articles were chosen, we found that 73% (08) correspond to Spain, with 9% (01) are from Brazil, Colombia and Argentina respectively. Of which 50% are cross-sectional analytical studies of the total number of articles analyzed, 80% state that the risk factors in the evolution and mortality of heart failure in the elderly are high blood pressure, diabetes mellitus, smoking and obesity, while 20% % are lifestyle, dyslipidemia, pharmacological, age, sex and anxiety that negatively affects patients.
