The possibility of uterine rupture in an intact uterus is rare, but prevalent risk factors such as multiparity, dystocia, fetal macrosomia, use of oxytocin, breech deliveries, obstetric trauma, internal and external fetal versions, uterine abnormalities, misuse of prostaglandins, and instrumental deliveries are described. This clinical case involves a 44-year-old multiparous patient at 38 weeks of gestation with uterine hypokinesia and a third-degree facial presentation. Fetal bradycardia was noted, and an immediate cesarean section revealed uterine rupture with posterior uterine segment rupture, followed by a cesarean hysterectomy and unilateral right salpingo-oophorectomy. Determine the effectiveness of the nursing intervention plan to reduce anxiety in pre-surgical patients with total abdominal hysterectomy at the National Maternal Perinatal Institute. Materials and methods. The approach of this research corresponds to the quantitative one, and the design to be used will be the quasi-experimental pretest and posttest design with a single group. The population will be made up of 80 pre-surgical patients who are about to undergo total abdominal hysterectomy in the gynecology service of the National Maternal Perinatal Institute, located in Cercado Lima, and the sample will consist of 50 patients from the aforementioned Institute. The sampling will be non-probabilistic of an intentional type. The technique will consist of the survey, with the application of the Zung Anxiety Assessment Scale (EAA). This test includes 20 statements that quantify 15 somatic and 5 cognitive symptoms, each referring to manifestations and characteristics of anxiety, such as symptoms or signs. Regarding the concurrent validity of the instrument, the correlation values obtained indicate significance between the EAA and the Beck depression inventory. Materials and methods: Quantitative type and systematic review design, with a population of 22 articles, and a sample of 10 articles. Results: Michele Scandola, in his study, concluded that robot-assisted surgery is more effective, with a shorter operative time compared to conventional laparoscopic surgery. Ahmet Gocmen, in his study, concluded that conventional laparoscopic surgery is more effective due to a shorter operative time and shorter hospital stay. Data collection was done through a literature review of national and international research articles that aimed to evaluate the effectiveness of total abdominal hysterectomy versus laparoscopic hysterectomy in patients with cervical cancer and uterine fibroids. Twenty scientific articles were reviewed, with a sample of 10 articles published in scientific databases in Spanish and English, no older than ten years. The selected articles were evaluated using the GRADE method, which allowed for the assessment of the quality of evidence and grading the strength of recommendations in the context of clinical practice guidelines development and systematic reviews. Ten scientific articles were reviewed, of which 30% were retrospective, 20% were quasi-experimental, 20% experimental, 20% systematic review, 10% prospective, and 10% observational. Ninety percent of the reviewed articles concluded that total laparoscopic hysterectomy is more effective than total abdominal hysterectomy. In the systematic review of the 10 scientific articles, it was concluded that 90% of them found that total laparoscopic hysterectomy is more effective than total abdominal hysterectomy in patients with cervical cancer and uterine fibroids.Ítem Acceso abierto Nursing care in post-hysterectomy patients in the post-anesthetic recovery unit of a hospital in Lima, 2021(Universidad Privada Norbert Wiener, 2021-10-31) Leiva Orihuela, Roberto Roggel; Cárdenas de Fernández, María HildaIntroduction: Hysterectomy is a global healthcare issue, and in Peru, it is reported that a higher percentage of women aged 35 to 49 are affected. This condition often requires a surgical intervention, either of moderate or high complexity, where nursing plays a key role in providing continuous care post-operation. Nurses are responsible for providing quality attention and expertise in the medical treatment of the patient, minimizing complications. To achieve this, we use nursing assessments and theories with the support of NANDA, NIC, and NOC. General Objective: Our general objective is to determine the nursing care provided to post-operative hysterectomy patients in the post-anesthesia recovery unit of a hospital in Lima, 2021. Methodology: This is a quantitative, descriptive, and explanatory study, as it measures based on a questionnaire and describes the behavior of the variable. It is a cross-sectional study, conducted within a specific time and space. The sample consists of 123 professional nurses working at the hospital. The techniques used are observation and interviews, with a questionnaire developed by Parasuraman, translated by Miranda Mendoza Nilda Sofía, and a reliability coefficient (Cronbach's alpha) of 0.85. The questionnaire is polytomous and includes dimensions, items, and scores.
