Publicación Acceso abierto Asociación entre tiempo de intubación y estenosis laringotraqueal en pacientes Covid del hospital nacional PNP "Luis N. Sáenz", Lima, 2020-2023(Universidad Privada Norbert Wiener, 2025-07-12) Vilchez Barzola, Adaia Keren-Kalid; Aviles Gonzaga, Roberto CarlosIntroducción: La estenosis traqueal post-intubación, común en casos de COVID 19 y asociada a intubación prolongada, puede causar SDRA y requerir ingreso a UCI. Objetivos: Determinar la estenosis laringotraqueal asociada a los días de intubación en pacientes post intubación por COVID del área de otorrinolaringología del “Hospital Nacional PNP Luis N. Saenz”, Lima, 2020-2023” Materiales y métodos: estudio observacional, transversal y retrospectivo con enfoque cuantitativo. La muestra estuvo conformada por 197 pacientes post COVID intubados en la UCI del Hospital Nacional PNP “Luis N. Saenz”, seleccionados por muestreo probabilístico y según criterios de selección. Se utilizó una ficha de recolección de datos validada por juicio de expertos. El procesamiento de datos incluirá el uso de medidas descriptivas, además de pruebas inferenciales para determinar la significancia estadística, entre ellas: test de Chi2 y t-Student. Resultados: Se analizó a 197 pacientes intubados por neumonía por SARS-CoV 2. La mayoría fueron hombres sin comorbilidades, aunque hipertensión y diabetes fueron frecuentes entre los que sí las presentaron. Un 14,72 % desarrolló estenosis laringotraqueal, diagnosticada principalmente por laringoscopia. Los pacientes intubados por más de 18 días presentaron un riesgo notablemente mayor de desarrollar estenosis moderada a severa. Para estenosis grado II, la razón de prevalencias (RP) fue de 3,6 (IC 95 %: 2,6–5,01; p = 0,0001), mientras que para grado III la RP fue de 4,12 (IC 95 %: 3,18–5,3; p = 0,0001) y para grado IV fue de 4,0 (IC 95 %: 3,03–5,2; p = 0,0001). Conclusiones: Estos hallazgos indican que la intubación prolongada se asocia significativamente con una mayor prevalencia de estenosis traqueal en grados avanzados, con implicancias clínicas relevantes para la prevención y manejo de estas complicaciones.Publicación Acceso abierto Conocimientos sobre aspiración endotraqueal y la práctica del profesional de enfermería en pacientes con ventilación mecánica hospitalizados en una unidad de cuidados intensivos de un hospital del MINSA Lima 2024(Universidad Privada Norbert Wiener, 2024-12-01) Cajas Pío, Renee Lizbeth; Morillo Acasio, Berlina del RosarioObjetivo: Determinar cómo se relacionan el conocimiento sobre aspiración endotraqueal y la práctica del profesional de enfermería en pacientes con ventilación mecánica hospitalizados en una unidad de cuidados intensivos de un hospital del MINSA Lima 2024.Diseño metodológico: será cuantitativa, básica, correlacional, transversal, no experimental. Participarán 40 enfermeros que trabajan en la unidad de cuidados intensivos. The search was restricted to articles with full text subjected to critical reading, using the GRADE system to assign the strength of recommendation. In the final selection, 10 articles were chosen, finding that 40% (04) correspond to Europe (2 from the United Kingdom, 1 from France and 1 from Italy), 30% (03) from America (2 from the USA and 1 from Canada). ) and 30% (03) Asia (2 from Japan and 1 from Taiwan). These are mostly meta-analysis (60%), clinical trials (20%), systematic review (20%). Results: 90% of the studies affirm that videolaryngoscopy is effective, because it reduced the number of failed intubations particularly in patients with a difficult airway, increased the success of the first intubation attempts, improved glottic vision, reduced laryngeal airway trauma, there was less incidence of postoperative hoarseness and showed higher success rate and shorter intubation time and is not effective for adult patients undergoing intubation endotracheal in 10% of the studies because no significant advantage between both was evident. The population consisted of a search for 24 articles, with inclusion criteria established. The final sample included 10 articles from indexed databases. Results: By origin, 20% (2/10) of the articles are from India, 20% (2/10) are from the USA, 10% (1/10) are from Korea, 10% (1/10) are from the United Kingdom, 10% (1/10) are from Iran, 10% (1/10) are from Belgium, 10% (1/10) are from Japan, and 10% (1/10) are from Singapore. According to the research design, 50% (5/10) are randomized clinical trials, 40% (4/10) are experimental designs, and 10% (1/10) are descriptive observational designs. According to the quality of evidence, 90% (9/10) are high and 10% (1/10) are low. Materials and Methods: Quantitative research type, systematic review design, the population was 10 scientific articles, the search instrument used was databases: Cochrane Library, Scielo, British Medical Journal, PubMed, Lancet, Wiley Online Library, and EBSCO. For article evaluation, the GRADE system was used; 10% correspond to Uruguay, 10% to Saudi Arabia, 10% to Iran, 10% to France, 10% to Malaysia, 30% to the USA, 10% to New Zealand, and 10% to the UK. Regarding designs, 40% were prospective observational, 30% case-control, 10% systematic review, 10% cross-sectional, and 10% retrospective. Results: Most of the systematically reviewed articles indicate the effectiveness of premedication for newborn endotracheal intubation in reducing adverse events. Conclusions: 80% of the studies showed that premedication for newborn endotracheal intubation allowed good relaxation of the jaw, suppression of the pharyngeal and laryngeal reflexes, success on the first attempt, and reduction of adverse physiological responses to pain, bradycardia, severe desaturation, systemic hypertension, and intracranial hypertension, while 20% of the studies showed that the use of premedication for endotracheal intubation was not effective, indicating concerns about possible side effects.Ítem Acceso abierto Effectiveness of the control of pneumotaponing pressure for the decrease of complications of patients with endotraqueal tube subjected to mechanical ventilation in the icu(Universidad Privada Norbert Wiener, 2019-08-04) Almeyda Pachas, Milagros Soledad; Cárdenas de Fernández, María HildaObjective: To systematically analyze the evidence from studies conducted on the effectiveness of pneumothorax pressure control to reduce complications in patients with endotracheal tube undergoing mechanical ventilation in the ICU. Material and method: The population is comprised of a review of 20 bibliographic articles, from which a sample of 10 articles was taken. The studies were found in the following databases: Cochrane, Lilacs, Scielo, Medline Pubmed, all of which were analyzed according to the Grade scale to determine the strength and quality of evidence. These studies correspond to the methodological design of systematic reviews 20%; clinical trial with 30%; meta-analysis with 10% and cohort studies with 40%. Results: Of the articles, 20% indicate that pressures greater than 30cm H2O cause morphological changes in the tracheal mucosa, therefore, acute complications such as dysphonia, sore throat, tracheal rupture, among other chronic cases, 20% indicate that the use of the aneroid manometer is essential for its. 30% indicate that continuous monitoring is important in the prevention of bronchial microaspirations and 20% indicate that it did not cause a strong impact in the prevention of VAP. According to 20%, it points out that in the prevention of nosocomial pneumonia, the measurement of pneumothorax is effective and enhances other general measures in the prevention of VAP. According to the articles, 80% of the studies demonstrate the effectiveness of supraglottic laryngeal masks for airway management in adult patients. This results in a higher percentage of successful intubations on the first attempt, less time needed for successful intubations, and the conventional tube can be used for blind tracheal intubation. 20% of the studies show effectiveness because they present less incidence of sore throat, less dysphonia, and hoarseness after the removal of supraglottic devices. In the conclusions, it is determined that the Fastrach supraglottic laryngeal masks are effective for airway management in adult patient intubation. In (8/10) of the studies, a higher percentage of intubations on the first attempt is achieved, less time is needed for successful intubations, and the conventional tube can be used for blind tracheal intubation. Of the 10 articles systematically reviewed, 50% (n= 5/10) are meta-analyses, 20% (n= 2/10) are a systematic review, 20% (n= 2/10) are randomized controlled trials and 10% (n= 1/10) are cohort studies and come from the countries of Australia (30%), followed by the United States (30%), Canada (20%), Brazil (10%) and Italy (10%). We identified 10 scientific pieces of evidence. The search was restricted to full-text articles, and the selected articles underwent a critical reading, using the GRADE system evaluation to identify their level of evidence. Among the 10 evidences, 40% (4/10) were case-control studies, 30% (3/10) were systematic reviews and meta-analyses, 20% (2/10) were cohort studies, and 10% (1/10) was a randomized controlled trial. 70% of the evidence came from the United States, followed by 20% from Japan and 10% from the United Kingdom. Results: In the final selection, 10 articles were chosen, and it was found that 100% (n=10/10) of the evidence indicated: 40% (n=4) report that supraglottic devices have a similar effectiveness to the endotracheal tube; 20% (n=2) report that supraglottic devices have greater effectiveness than the endotracheal tube; 40% (n=4) report that the endotracheal tube is associated with better outcomes, which are not associated with significant clinical consequences. Two instruments with acceptable validity and reliability criteria will be applied. The instrument measuring job satisfaction consists of 27 items and includes seven dimensions: administrative policy, relationship with authorities, physical and material conditions, social relationships, personal development, labor and/or remuneration benefits, and task performance. Meanwhile, the questionnaire measuring job performance consists of 28 items and includes four dimensions: personal development and training, responsibility, motivation and leadership, and teamwork. They are an essential part of evidence-based nursing due to their rigorous methodology, identifying relevant studies to answer specific questions of clinical practice, the search has been restricted to articles with full text, and the selected articles were subjected to critical reading. using the grade system to assign recommendation strength. In the final selection, 10 articles were chosen, we found that 30% (03) correspond to Spain, with 20% (02) to the Country of Italy, while 50% (05) to Australia, Denmark, Mexico, Colombia and Iran. Of the total number of articles analyzed, 100% affirm that nursing care in neonates with an endotracheal tube is effective when performed on newborns with mechanical ventilation in intensive care units. Results The prone position favors oxygenation in neonates with an endotracheal tube. It is important to apply preventive pain measures before any procedure. Care for neurodevelopment includes interventions aimed at improving both the light and noise environment, as well as the microenvironment in which the child develops. Materials and methods: Indirect reasoning with a research approach is quantitative, descriptive, simple correlation and transversal since it allows obtaining information about the understanding of nursing staff in the intensive care unit and the daily work of endotracheal aspiration of patients in a hospital in Lima, Peru, therefore, this study is not experimental. It should be noted that the population is made up of 100% of nurses who worked in the intensive care unit of a hospital in Lima, Peru in 2021, with a total of 40 nurses corresponding to the total population under investigation. These studies are an essential part of evidence-based nursing due to their rigorous methodology, identifying relevant studies to answer specific clinical practice questions. The search was limited to full-text articles, and the selected articles were subjected to a critical review using the GRADE system to assign the strength of recommendations. Results: In the final selection, 10 articles were chosen, with 30% (3) from Spain, 20% (2) from Italy, and 50% (5) from countries such as Australia, Denmark, Mexico, Colombia, and Iran. Most of the studies were systematic reviews (60%), primarily from Italy, Spain, Australia, Mexico, and Colombia. Clinical trials were found in 20% (2) from Spain, and 10% (1) from Denmark. One analytical study (10%) was from Iran. From the total analyzed articles, 40% affirm that endotracheal suctioning is effective when performed as needed for neonates with mechanical ventilation in intensive care units. 10% (1) state that the prone position improves oxygenation in neonates with endotracheal tubes. 10% (1) emphasize the importance of applying preventive pain measures before any procedure. 10% (1) highlight that neurodevelopmental care includes interventions aimed at improving both the lighting, noise, and microenvironment in which the neonate develops. Conclusions: The evidence-based nursing care for neonates with endotracheal tubes was found to be effective in the reviewed studies.
