    Advanced maternal age as a risk factor associated with obstetric morbidity

    Objective: To identify scientific publications on advanced maternal age as a risk factor associated with obstetric morbidity. Methodology: The review was conducted through an electronic search of articles related to advanced maternal age as a risk factor associated with obstetric morbidity. Literature and publications from LATINDEX, SCIELO, LILACS, PUBMED, REDALYC, SCOPUS, and the Repository of the Norbert Wiener University were used. Original articles published in journals from Peru and abroad from 2012-2020 were included. Results: 87.5% of the publications show that women with advanced maternal age (≥35 years) are more likely to experience adverse obstetric outcomes such as preeclampsia, hypertension, gestational diabetes, preterm birth, and cesarean delivery. 12.5% of the publications indicate that very advanced maternal age (≥45 years), often a result of assisted reproductive technology, is associated with an increased risk of various obstetric pathologies, mainly preeclampsia and a higher rate of cesareans. Eight studies report adjusted odds ratios (ORa) for preeclampsia ranging from ORa=1.25-3.05; gestational diabetes ORa=1.83-5.96; preterm birth ORa=1.10-3.2; and cesarean ORa=1.33-2.8. Additionally, two studies show adjusted relative risks (RRa) for preeclampsia ORa=1.3-1.86; preterm birth RRa=1.20; and cesarean RRa=1.7. Conclusion: Advanced maternal age is mainly associated with preeclampsia, gestational diabetes, preterm birth, and a high cesarean rate.

    Breastfeeding self-efficacy of pregnant women with an effect on the interruption of exclusive breastfeeding in the tablada de Lurín maternal and child center 2020

    Exclusive breastfeeding is the fundamental pillar for infant nutrition; in the world a large group of infants enjoy the benefits of EBF; Among the most important benefits are saving their lives, avoiding morbidity, promoting optimal physical and cognitive development, in addition to reducing the risk of contracting chronic diseases. It is worth mentioning that there are studies that affirm that the lack of this practice affects sustainable development and poverty reduction in the long term. Objective: Determine the breastfeeding self-efficacy of pregnant women with an effect on the interruption of exclusive breastfeeding at the Tablada De Lurín 2020 Maternal and Child Center. Methodology The present research will have a quantitative approach since the variable will be measured numerically, the design of the The study will be non-experimental, the method will be descriptive and cross-sectional. The sample population will be made up of 70 pregnant women who attend the health facility. The data collection technique is the survey and the instrument will be the LIKER type called “Self-efficacy for breastfeeding”, an instrument created in Mexico in 2018, which is found to have validity and reliability.

    Effectiveness of inhaled antibiotic treatment to reduce morbidity in adult patients with chronic pulmonary infections

    Objective: To systematically analyze the evidence on the effectiveness of inhaled antibiotic treatment in reducing morbidity in adult patients with chronic pulmonary infection. Material and Methods: The systematic review of 10 scientific articles found on the effectiveness of inhaled antibiotic treatment to reduce morbidity in adult patients with chronic pulmonary infection were sourced from the following databases: Cochrane, Lilacs, Scielo, Medline Pubmed. All articles were analyzed according to the GRADE scale to determine the strength and quality of the evidence. Of the 10 articles reviewed, 80% (8/10) correspond to the methodological design of systematic reviews, and 20% (2/10) to meta-analyses. Results: Of the 10 articles reviewed, 100% (10/10) demonstrate the efficacy of inhaled antibiotic treatment. Conclusions: The 10 articles reviewed demonstrate the effectiveness and efficacy of inhaled antibiotic treatment in reducing morbidity in adult patients with chronic pulmonary infection.

    Effectiveness of the influenza vaccine for the prevention of respiratory diseases and reduction of morbidity in children and older adults

    Objective: to systematize the evidence on the effectiveness of the influenza vaccine for the prevention of respiratory diseases and the reduction of morbidity in children and the elderly. Materials and methods: A retrospective observational systematic review was used. In the search, full-text articles were restricted. The selected articles were subjected to a critical reading. The Jover evaluation was used to verify the degree of evidence. Results: For the final selection, 9 articles were chosen, where it is evident that 50% (05) are articles carried out in Spain, 20% (02) carried out in Brazil, while 10% (01) were in Colombia and 10% (1) in the United States. Among systematic reviews, we found the countries of Spain, the United States and Brazil with 10%. And 40% observational studies carried out in the United States. Of the total number of articles reviewed, 90% corroborate that the influenza vaccine is effective. Conclusions: finally, it is evident that there is immunological effectiveness in the vaccine of influenza, as prevention of respiratory diseases in children under 5 years of age and older adults, thus providing the population with the benefit of reduced morbidity

    Effectiveness of tympanoplasty in patients with otitis media

    Objective: analyze and systematize evidence on the effectiveness of tympanoplasty in patients with otitis media. Materials and methods: the research was quantitative, its design being a systematic review whose population was made up of 15 scientific articles having as a sample 10 studies that were published in different scientific databases: Mediciego, Sciencedirect, Scielo, Pubmed, Elsevier, Epistemónikos, Ebsco and for its analysis the grade method was used where the quality of evidence and the strength of recommendation was evaluated. Results: we have that of 100% of the scientific articles, 20% correspond to Chile, 10% Colombia, 10% USA, 10% Mexico, 10% Cuba, 10% Qatar, 10% Cyprus, 10% to the United Kingdom and finally 10% to China respectively. Regarding the study designs, we can mention that 70% correspond to retrospective, 10% meta-analysis, 10% prospective and 10% cohort respectively. Conclusions: it was concluded that 100% of the studies show the effectiveness of tympanoplasty in patients with otitis media; the correction of tympanic perforations in both children and adults caused by this pathology, reduction in postoperative morbidity, hearing improvement, anatomical recovery, comfort for the patient and their self-esteem.

    Efficacy of early extubation vs. conventional intubation in reducing the hospital stay of older adults undergoing cardiac surgery

    Objective: To establish the effectiveness of early extubation vs. conventional extubation in reducing the hospital stay of older adults undergoing cardiac surgery. Materials and Methods: The systematic bibliographic review is Observational and retrospective in Spanish, with a history of no more than 5 years, the search has been restricted to full-text articles and which have been selected through critical reading. Results: In the final selection, 10 articles were chosen, 10% being Chile and 90% of other Latin American countries, no attempt was made to calculate the sample sizes required to determine if the mortality rates and major morbidities after cardiac surgery They change depending on the moment chosen to perform the extubation. 30% are Quasi-experimental studies, another 30% are Random Studies applying conventional extubation with early extubation and prospective studies of the study criteria considering risk factors and patient age in search of evaluating the morbidity and mortality of the patients. patients undergoing cardiac surgery. Conclusions: It was evident in scientific research and systematic reviews of the different types of randomized, quasi-randomized and prospective studies that consider Early Extubation effective in cardiac surgery patients, which reduces the risk of infection and the hospital stay of the post-surgery patient. cardiac surgery, therefore, reduces institutional costs. In addition, the nurse plays important roles in standardizing the care of the intubated patient through timely care in the patency of secretions, position of the patient, well-being and maintenance of the physical and mental comfort of the patient undergoing cardiac surgery.

    Efficacy of the kangaroo mother method in the reduction of morbidity and mortality of low birth weight neonates.

    Objectives: To analyze the evidence on the effectiveness of seroprotection of the tetravalent vaccine compared to the bivalent vaccine in the prevention of cervical cancer. Materials and Methods: The study design was qualitative, observational, and prospective. The search was conducted in articles with complete texts, and the study population consisted of 24 articles, with a sample of 10 from the period 2013 to 2017. Included articles included randomized controlled trials, systematic reviews, meta-analyses, and cohort studies regarding the seroprotection of the vaccine in preventing cervical cancer. Results: Of the 10 articles reviewed, 100% showed significant efficacy in the seroprotection of the tetravalent vaccine compared to the bivalent vaccine. Conclusions: 90% of the studies related to the tetravalent vaccine compared to the bivalent vaccine in the prevention of cervical cancer were effective due to the reduction in the incidence of HPV, the development of prophylactic vaccines, an acceptable safety profile, monitoring, and a positive impact in achieving useful coverage. The remaining 10% concluded that the vaccine only covers some high-risk types of HPV.

    Efficacy of the use of probiotics for the reduction of morbidity in very low birth weight preterm neonates

    Objective: To systematize the evidence of the efficacy of probiotic use for the reduction of morbidity in very low birth weight preterm neonates. Materials and Methods: A systematic observational and retrospective review of a quantitative type, subjected to critical selection, using the GRADE evaluation system for identifying the level of evidence, found in the following databases: Cochrane, LILACS, Scielo, Medline Pubmed, Epistemonikos, Sciencedirect, Elsevier, Epistemonikos. Of the 10 articles systematically reviewed, 60% (n= 6/10) are meta-analyses, 20% (n= 2/10) are systematic reviews, 10% (n= 1/10) is a systematic review/meta-analysis, and 10% (n= 1/10) is experimental, so 100% of the evidence is of high quality. According to the results obtained from the systematic review conducted in this study, they come from the following countries: China (30%), followed by Taiwan (20%), USA (20%), Spain (10%), Brazil (10%), and Scotland (10%). Results: Of the evidence found, 90% (n=9/10) indicate that probiotics are effective for the prevention and reduction of morbidity in very low birth weight preterm neonates. Conclusion: 9 out of 10 articles conclude that probiotics are effective for the reduction of morbidity in very low birth weight preterm neonates.

    Estado nutricional al ingreso y su relación con la morbimortalidad y la estancia hospitalaria en pacientes de la unidad de cuidados intensivos. Hospital Nacional categoría III-1, Lima 2023

    El objetivo del estudio fue determinar la relación del estado nutricional al ingreso con la morbimortalidad y la estancia hospitalaria en pacientes del servicio de la Unidad de Cuidados Intensivos de un Hospital Nacional categoría III-1, Lima 2023. Estudio prospectivo, observacional, descriptivo con característica correlacional. Se estudió historias clínicas de pacientes críticos de la Unidad de Cuidados Intensivos de un Hospital Nacional categoría III-1, en el período febrero – mayo del año 2023, se recopiló datos mediante un instrumento validado, los datos recolectados se codificaron por medio del programa SPSS, se utilizó la prueba estadística de correlación de Spearman rho para la prueba de hipótesis. Se encontró relación estadística entre el estado nutricional con la mortalidad, la morbilidad y la estancia hospitalaria en su parámetro bioquímico: nivel de albúmina, presentando un p-valor < 0,05; situación que no ocurre en su parámetro hematológico: nivel de hemoglobina, ya que presenta un p-valor > 0,05. Se llegó a la conclusión de que existe relación significativa entre el estado nutricional al ingreso con la morbimortalidad y la estancia hospitalaria en pacientes del servicio de la Unidad de Cuidados Intensivos en un Hospital Nacional categoría III-1, Lima 2023.

    El síndrome de dificultad respiratoria aguda ha sido estudiada desde hace 50 años con mayor énfasis por considerarse un problema recurrente en niños nacidos prematuros que por consiguiente traen consecuencias llegando hasta la muerte en algunos casos. Determinar la relación que existe entre factores del distrés respiratorio y el impacto en la morbimortalidad en prematuros en el hospital Hermilio Valdizan Medrano 2023.El presente estudio es de tipo básico, cuantitativo, correlacional y de corte transversal. La muestra será censal y estará constituida por 104 Historias clínicas de neonatos prematuros nacidos en el Hospital Hermilio Valdizan Medrano. Se procederá a hacer el uso de la técnica de análisis documental para aplicar la ficha de recolección de datos previamente de a ver sido validado y realizado la confiabilidad para medir las variables a estudiar.Se realizara la prueba de hipótesis haciendo uso de la prueba estadística del coeficiente de correlación de Pearson dado el carácter cuantitativa de las variables.

    Impact of using the surgical checklist on reducing perioperative morbidity and mortality

    Materials and Methods: The study type is qualitative, with a systematic review design. The population consisted of 70 articles, and the sample was 10 scientific articles. They originated from Australia (10%), Switzerland (10%), Germany (10%), the Netherlands (10%), the United States (40%), Iran (10%), and Chile (10%). The instrument was obtained from the following databases: PubMed, Wolters Kluwer, Wiley One Library. The GRADE system was used for article evaluation. The studies included 40% (4/10) qualitative studies and 60% (6/10) quantitative studies. Results: According to the level of evidence based on the methodological design, 40% of studies showed high evidence, and 60% showed moderate evidence. Of these, 40% (4/10) were systematic reviews, 40% (4/10) were cohort studies, and 20% (2/10) were case-control studies. Conclusion: After the systematic review, 90% (9/10) of the articles show the impact of the surgical checklist on reducing perioperative morbidity and mortality, while 10% (1/10) were inconclusive regarding the topic.

    Knowledge and practices of the kangaroo mother method in mothers of premature newborns in the Intensive Care, Intermediate Care and Neonatal Joint Accommodation Unit of a National Hospital

    Introduction: Since the kangaroo method was first applied in a hospital, much has been written about its approach to maternal care. It was created in Peru as an alternative to incubator care to guarantee the well-being of the premature infant. Objective: To determine the relationship between the level of knowledge about the mother kangaroo method and the practice in mothers of premature newborns in the Intensive Care Unit, Intermediate Care Unit and Neonatal Joint Lodging of a National Hospital, Lima 2023. Methods: Hypothetical-deductive, applied, quantitative, non-experimental, descriptive, correlational and cross-sectional research. The census sample will be constituted by 70 mothers of premature newborns in a National Hospital. The survey and two Likert-type questionnaires will be used, which have been validated and are reliable to quantify the relevant variables. The data will be processed in Excel and the hypotheses will be verified using the Spearman correlation coefficient statistical test.

    Risk factors associated with high blood pressure in adult patients

    Objective: Determine the risk factors associated with high blood pressure in adult patients. Materials and methods: Systematic investigations are an observational and retrospective research design that synthesizes the results of multiple primary investigations, which are an essential part of evidence-based nursing due to its rigorous methodology, identifying relevant studies to answer specific questions of the clinical practice. Results: The scientific articles included in the systematic review on the risk factors associated with hypertension in adults were found in the following databases Lipecs, Lilacs Scielo and Medline, all of them correspond to the type and design of cross-sectional studies. The studies reviewed show that the risk factors associated with high blood pressure are obesity, hypercholesterolemia, age, overweight, low HDL concentrations and an inadequate lifestyle. Conclusions: The risk factors associated with high blood pressure are obesity, hypercholesterolemia, age, overweight, low HDL concentrations and inadequate lifestyle. Interventions to control high blood pressure are to achieve the maximum reduction in the total long-term risk of cardiovascular morbidity and mortality. This process should be based on primary prevention and early detection of all identified reversible risk factors and management. adequate of the associated clinical conditions such as the increase in blood pressure itself.
