    Adherence factors involved in the treatment of cancer patients

    Objective: Determine the adherence factors that intervene in the treatment of cancer patients. Methodology: It is a systematic review in different Medline databases - Pubmed, Lilacs, Scielo and Science direct, following the PICO format, the inclusion criteria were: scientific articles published between 2006 and 2016 in Spanish and English, with selection final of 12 articles. Results: Of the 12 articles reviewed, 100% (12/12 articles) that intervened were the sociocultural factor, which had 59% (7/12), while the factor of adverse effects to treatment had 50% (6/12) and the economic factor has 17% (2/12). The quality of the articles was mostly moderate at 75% (9/12). In the most searched articles it was in the country of Nigeria (3/12) with 25.6%, in which the dimension that most intervened for non-adherence to treatment was the economic factor. Conclusions: The associated adherence factor was, according to this review, multifactorial and involves sociocultural factors, adverse effects of treatment, and economic factors. Strategies and alternatives must be generated to develop early interventions that allow for better therapeutic adherence in cancer patients.

    Critical review effect of oral nutritional supplementation in patients receiving chemotherapy

    Cancer patients who receive chemotherapy manifest various secondary nutritional effects that contribute to the deterioration of their nutritional status. The present research titled as a critical review: Effect of oral nutritional supplementation in patients receiving chemotherapy aimed to identify the possible effects of nutritional supplementation in patients undergoing chemotherapy. The clinical question was: What is the effect of oral nutritional supplementation in patients receiving chemotherapy? The Evidence-Based Nutrition (NuBE) methodology was used. The investigation and selection of information was carried out from the Google Scholar search engines and databases such as Science Direct, Pubmed, HINARI, Scielo, Dialnet, and 42 articles were found, selecting 12 that have been evaluated by the instrument for critical reading. CASPE, finally choosing “Systematic review and meta-analysis of the evidence of oral nutritional intervention on nutritional and clinical outcomes during chemotherapy (radiotherapy): current evidence and guidance for the design of future trials, which It has a Level of evidence: AII, Grade of recommendation: Strong, as evaluated by the researcher. The critical comment allowed us to conclude that said meta-analysis suggests a general effect that could be found on body weight after nutritional interventions, with oral supplementation during chemo and/or radiotherapy, in addition to providing a benefit with respect to the metabolic alterations that we could found in cancer patients.

    Critical review: diet risk factors associated with gastric cancer.

    In our country, gastric cancer continues to be the second malignant neoplasm with the worst prognosis and the highest number of deaths. Therefore, the objective of this review was to know the risk factors that are associated with the development of gastric cancer, for this purpose the evidence-based nutrition (NUBE) methodology was used. The search strategy was developed with the keywords, and it was carried out in the Scopus, Embase, Pubmed and Cochrane Library databases. Of those, a total of 60 were obtained, of which 10 were selected for analysis through the CASPe tool, to be able to classify them according to degree of evidence and level of recommendation. The selected article was classified with an AI level of evidence and a strong recommendation; salt in the diet is an independent risk factor for the development of gastric cancer, just as alcohol consumption is more likely to be a risk factor. risk of excessive consumption. Likewise, red meat and a high-fat diet increase the risk, while fresh fruits, vegetables and certain micronutrients such as selenium and vitamin C have protective effects.

    Critical review: effect of a vegetarian regimen in the management of chronic kidney disease (CKD)

    The nutritional intervention is to evaluate the effect of a vegetarian diet on chronic kidney disease. The present secondary research titled as a critical review: “Association of Vegetarian Diet with Kidney Disease”, aimed to provide professional critical commentary regarding scientific articles of clinical studies related to the association of vegetarian diet on kidney disease. The clinical question was: What are the effects of a vegetarian diet on the progression of kidney disease in patients with chronic kidney disease? The methodology used was Evidence-Based Nutrition (NuBE). An information search was carried out in PUBMED, REDALYC, SCIELO, SCIENCE DIRECT, DIALNET; 48 articles were found, 11 were chosen that have been evaluated by the CASPE critical reading tool, finally choosing the Retrospective Cohort Study entitled "Association of Vegetarian Diet with Chronic Kidney Disease", said study has a level of evidence C II and Strong Recommendation Degree, according to the researcher's review. The review of the study gave rise to a critical comment which allowed us to conclude that the vegetarian diet is significantly associated with a lower prevalence of chronic kidney disease, obtaining multiple benefits, including better blood pressure control, lower production of uremic toxins, and preventing progression. of kidney disease, however more large-scale randomized controlled trials are needed.

    Critical Review: Effect of Covid-19 Confinement on Adult Weight Gain.

    Social isolation and the use of other measures, such as quarantine or confinement during the development of the Covid-19 pandemic, have been shown to affect different aspects of the lives of the global population. Therefore, the present review, is a secondary investigation titled as a systematic critical review, aimed to identify scientific articles for the critical review of the scientific literature that answers clinical questions. In accordance with evidence-based nutrition, a search for information was carried out in SCOPUS, EMBASE, PUBMED, SCIENCE DIRECT, with 151 initial articles, of which 31 were finally selected and evaluated by the reading tool. CASPE review. After the evaluation, the article that best answered the research question was selected, regarding the impact of confinement on lifestyles that may lead to weight gain. COVID-19 had both a negative and positive impact on practices. across Europe and around the world, and negative dietary habits were associated with other poor lifestyle outcomes, such as weight gain, mental health problems and limited physical activity.

    Critical review: effect of omega 3 supplementation in children with nonalcoholic fatty liver disease.

    An information search was carried out in the databases PUBMED, SCIENCE DIRECT, NCBI, COCHRANE LIBRARY, THE JOURNAL OF PEDIATRICS, finding 43 articles, 10 being selected, which have been evaluated by the CASPE critical reading tool, finally the selected article allowed us to conclude that, when omega-3 fatty acids (DHA and EPA; in a ratio of 3:2 (450-1300 mg / day)) were supplemented during 6 months did not show a significant effect on the reduction of liver fat according to the study. Supplementation with omega-3 showed significant changes in the levels of aspartate aminotransferase (AST) and gamma-glutamyl transpeptidase (GGTP) in children with NAFLD compared to placebo and suggested that omega-3 may be useful in patients. with NAFLD to prevent other complications, such as cardiovascular diseases.

    Critical review: effect of physical exercise on glycosylated hemoglobin levels in patients with type 2 diabetes mellitus

    Diabetes is one of the biggest health emergencies of the 21st century and one of the four non-communicable diseases (NCDs) selected by world leaders to intervene as a priority. This research seeks to understand the scientific literature on the effect of physical exercise on glycosylated hemoglobin levels in patients with type 2 diabetes mellitus. The type of research is secondary. The methodology according to Evidence-Based Nutrition (NuBE) is used. The 5 phases of the NuBE will be described and used for the development of critical reading. A synthesis of the literature found is described and the selected scientific literature is evaluated through the “Critical Appraisal Skills Program Spain” (CASPe). According to the chosen article, combined exercise showed a more pronounced improvement in HbA1c than supervised aerobic exercise or exercise. of resistance supervised alone. The prescription of physical exercise is recommended for the treatment and/or prevention of type 2 diabetes mellitus so that, together with medication and dietary intervention, adequate glycemic control can be achieved and thus avoid or delay the complications that this disease entails.

    Critical review: effect of the low Fodmap diet in the treatment of Irritable Bowel Syndrome.

    Irritable Bowel Syndrome (IBS) is a functional digestive disorder characterized by abdominal pain and changes in stool frequency or consistency. The etiology is unknown. There are four IBS subtypes, resulting from the interaction of various factors. There are no direct diagnostic tests for IBS, and treatment focuses only on symptom management. Methodology was described based on Evidence-Based Nutrition (EBN), using its five phases for critical reading. Articles from systematic reviews and clinical case studies of IBS patients were selected, utilizing global search engines such as Google Scholar, PubMed, and ScienceDirect. A review article titled "Efficacy of Low FODMAP Diet Compared to Low Lactose Diet in IBS Patients: A Randomized Cross-Over Trial" by Krieger et al. was reviewed. IBS patients, regardless of lactase deficiency, showed significantly reduced scores after both diets, with no significant differences between them. Both diets resulted in less impact on daily life for patients compared to before the study. The daily evaluation showed that IBS patients on the low FODMAP diet reported significantly less abdominal pain and bloating than those on the low lactose diet.

    Critical review: effect of vitamin d on multiple sclerosis management.

    Recent studies indicate that multiple sclerosis has been increasing worldwide, being the main cause of neurological disability in young adults. The objective of this critical review was to know the effect of vitamin D supplementation in the management of patients with multiple sclerosis. Through the principles of evidence-based nutrition (NuBE), a bibliographic search was carried out with MeSH and DeCS descriptors and related terms, in the Scopus, Embase, Web of Science, Pubmed and Cochrane databases. After the CASPE critical reading analysis, the article was selected: “Effectiveness of vitamin D supplementation in the management of multiple sclerosis: A systematic review”, which presents a level of IA evidence and a strong recommendation grade. The analysis allows us to conclude that, although the vitamin presents positive effects as part of the treatment of multiple sclerosis, it is still not conclusive given the heterogeneity of the studies; however, it is observed that supplementation is very favorable in those who have the vitamin D levels at the lower limit, therefore, it is important to analyze vitamin D levels in patients with multiple sclerosis.

    Critical review: effect of zinc and supplementation on immune function and response

    The immune system is an integral part of the inflammatory response with a protective function in order to counteract the invasion of pathogenic microorganisms. Likewise, all cells in the body have the potential to participate in the immune response process. On the other hand, zinc is an essential element for most of the processes that occur in organisms. It has recently been noted that, for humans, food is the main source of zinc. So much so that zinc deficiency can be demonstrated in a significant proportion of the population. For this, the principles of evidence-based nutrition were used as a methodology. For this reason, a systematic search was carried out in 5 databases, Scopus, WOS, Pubmed, Embase and Science Direct, from the last 5 years, which allow us to answer the research question. After the selection of articles, they were evaluated through a critical reading tool to determine the degree of recommendation and level of evidence. Finally, the article with the highest degree of evidence is developed in the critical commentary, in order to extract recommendations for the professional practice of nutritionists and other health professionals.

    Critical review: Influence of flavonoid consumption on liver cancer

    Liver cancer is the sixth type of cancer with the highest mortality rate, with hepatocellular carcinoma being the most common, representing 90% of all cases, which originates in cells called hepatocytes. The present secondary research entitled critical review: Influence of flavonoid consumption on liver cancer, aimed to determine the influence of flavonoid consumption on liver cancer. The clinical question was: How does the consumption of flavonoids influence patients with liver conditions and liver cancer? The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in Pubmed, Scielo, Wiley, Science Direct, Google scholar and Taylor & Francis, finding 52 articles, 23 being selected that have been evaluated by the CASPE critical reading tool, finally selecting the article titled Autophagy: a potential therapeutic target of polyphenols in hepatocellular carcinoma, which has a level of evidence A II and a strong degree of recommendation, according to the researcher's expertise. The critical commentary allowed us to conclude that flavonoids have antioxidant, anti-inflammatory, carcinogen inactivating, antiproliferative, cell cycle arrest inducers, apoptotic, antiangiogenic, autophagy modulators and multiple drug reversal effects, in addition, they help prevent and treat several types of cancer including hepatocellular carcinoma.

    Critical Review: Influence of intestinal microbiota alteration on the development of colorectal cancer.

    According to the World Health Organization (WHO), cancer is a public health problem. Among them, colorectal cancer has the highest incidence. The present investigation, Influence of the alteration of the intestinal microbiota on the development of colorectal cancer, was developed as a critical review. Objective: Evaluate the scientific literature through a critical reading of it, using the Critical Appraisal Skills Program Spanish (CASPE) tool in published studies. The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in PUBMED, SCIELO, SCOPUS, SCIENCE DIRECT, REDALYC, DIALNET, LILACS, finding 87 articles, selecting 29 that were evaluated by the CASPE critical reading tool, selecting the Systematic Review entitled Preclinical and clinical relevance of probiotics and synbiotics in colorectal carcinogenesis: a systematic review, which has a level of evidence AI and Grade of Recommendation Strong, according to the researcher's expertise. The critical commentary concludes that intestinal dysbiosis influences colorectal carcinogenesis. However, more studies are necessary to identify the characteristics of the eubiosis profile. It is important that new prophylactic, diagnostic and therapeutic strategies be carried out based on research into the intestinal microbiota.

    Critical review: influence of moderate-high physical activity for glycemic improvement in patients with type II diabetes mellitus

    Physical activity is considered a method for controlling blood sugar levels in patients with Type 2 diabetes mellitus, contributing to the management of the condition. The objective was to determine the effect of resistance exercise (RE) with varying intensity levels on HbA1c values, biochemical factors like insulin and blood glucose in Type 2 diabetes patients. Evidence-Based Nutrition principles were applied, and scientific articles were searched in PubMed, Scopus, Embase, and Lilacs. After selecting articles, they were evaluated using the Spanish version of the CASPe critical reading tool. The selected article, "Resistance Exercise Intensity is Correlated with Attenuation of HbA1c and Insulin in Patients with Type 2 Diabetes: A Systematic Review and Meta-Analysis," presents evidence level A and a strong recommendation grade. The conclusion is that high-intensity resistance exercise has greater beneficial effects than low to moderate intensity on controlling and reducing HbA1c and insulin levels in Type 2 diabetes patients.

    Critical Review: Mediterranean diet in the reduction of colorectal cancer

    Globally, colorectal cancer (CRC) occupies fourth place and reaches an incidence of more than 20% in men and around 16% in women. On the other hand, adherence to a Mediterranean diet is associated with a reduced risk of metabolic diseases and cancer. Therefore, the objective of this review was to determine whether the consumption of a Mediterranean diet can reduce the risk of colorectal cancer. For the development of this review, the principles of evidence-based nutrition were used as a methodology. For this reason, a systematic search was carried out in 5 databases, Scopus, WOS, Pubmed, Embase and Science Direct, from the last 5 years, which allow us to answer the research question related to the consumption of the Mediterranean diet and the risk of cancer. colorectal in adults. After the selection of articles, they were evaluated through a critical reading tool to determine the degree of recommendation and level of evidence. Finally, the article with the highest degree of evidence is developed in the critical commentary, in order to extract recommendations for the professional practice of nutritionists and other health professionals. It was found that greater adherence to a Mediterranean diet is associated with a reduction in the incidence of colorectal cancer, which is detailed in the critical commentary.

    Critical review: nutritional intervention with probiotics in patients with dyslipidemia

    The present secondary research titled as a critical review: Nutritional intervention with probiotics in patients with dyslipidemia, aimed to identify the effects of probiotics on the lipid profile in patients with dyslipidemia. The clinical question was: Will nutritional intervention with probiotics improve the lipid profile in adults suffering from dyslipidemia? The Evidence-Based Nutrition (NuBE) methodology was used. An information search was carried out in PubMed, Lilacs, Scielo, Dialnet, Science Direct and 37 articles were found, of which 11 were selected and evaluated by the CASPE critical reading tool, finally selecting the Systematic Review and Meta-analysis titled “The impact of probiotic yogurt consumption on lipid profiles in subjects with mild to moderate hypercholesterolemia: A systematic review and meta-analysis of randomized controlled trials”, which has a level of evidence A II and Grade of Recommendation Strong, according to the researcher's expertise. The critical comment allowed us to conclude that daily consumption of 300 mg of probiotic yogurt, with a maximum dose of 109 cfu and for more than 4 weeks reduces total cholesterol and LDL-C.

The clinical question was: Will nutritional intervention (nutritional supplements) have an effect on the risk of anemia in anemic children aged 6 to 36 months? The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in PUBMED, MEDLINE, SCIELO, SCOPUS, SCIENCE DIRECT, finding 43 articles, 10 being selected that have been evaluated by the CASPE critical reading tool, finally selecting the Randomized Clinical Trial entitled Effect of Fortification with Multiple Micronutrient Powder in the Prevention and Treatment of Iron Deficiency and Anemia in Brazilian Children: A Trial Randomized Clinical, which has a level of evidence A I and Grade of Recommendation I, according to the expertise of the researcher. The critical comment allowed us to conclude that supplementation is effective in preventing anemia in children from 6 to 42 months of age.

    Critical review: Protective effect of vitamins c, d, e and selenium in thyroid cancer

    Globally, thyroid cancer ranks sixth in incidence for 2018, occurring in 3.7% of the population. Likewise, it is located in ninth place in relation to the appearance of new cases, and the increase in them has been observed constantly in the last forty years. Although thyroid cancer is not the most common cancer, it is one of the most diagnosed among endocrine cancers. Because cancer is characterized by an increase in oxidative stress and the formation of free radicals, the possibility has been seen that antioxidant micronutrients may play an important role in the prevention and management of thyroid cancer. It has been shown that the intake or supplementation of vitamin D can generate a favorable change seen from a thyroid cancer prevention perspective. The objective was to find in the scientific literature an association of vitamins C, D and E and selenium with the risk of thyroid cancer or to find a protective effect of vitamins C, D and E and selenium in the treatment of thyroid cancer. Using the NUBE methodology, scientific literature on the relationship of vitamin C, vitamin D, vitamin E and selenium in the risk and treatment of thyroid cancer was integrated from systematic reviews and clinical trials. This research guides health professionals to know a possibility or finding of the nutritional interventions available to be applied for the prevention and nutritional treatment of thyroid cancer.

    Critical review: Spirulina supplementation in adult patients with dyslipidemia

Currently there are various studies of supplements that lower cholesterol, among which spirulina stands out. This secondary research titled: “Critical review: Spirulina supplementation in adult patients with dyslipidemia” aimed to identify the benefits of oral supplementation with spirulina algae in adult patients suffering from dyslipidemia. The clinical question was: Does oral supplementation with spirulina improve the lipid profile of patients suffering from some type of dyslipidemia? The Evidence-Based Nutrition (NuBE) methodology was used. The information search was carried out in PUBMED, REDALYC, SCIELO, SCIENCE DIRECT, finding 32 articles, of which only 11 were evaluated by the CASPE critical reading tool, finally selecting the Systematic Review titled “Meta-analyses A systematic review and meta-analysis of the impact of Spirulina supplementation on plasma lipid concentrations” which has a level of evidence AII and Grade of Strong Recommendation, according to the expertise of the researcher. The critical commentary allowed us to reach the conclusion that there is sufficient evidence that spirulina supplementation helps reduce high concentrations of lipids in the blood.

The clinical question was: Does parenteral glutamine supplementation improve clinical outcomes (hospital infections, mechanical ventilation days, and ICU length of stay) in critically ill adult patients? The Evidence-Based Nutrition (NuBE) methodology was used. Information was searched in Science Direct, PubMed, HINARI, Lilacs, Latindex, Scielo, ERIHPLUS, Dialnet, Redalyc, and Cochrane, finding 62 articles, of which 21 were selected and evaluated using the CASPe critical appraisal tool. Ultimately, the systematic review titled 'Parenteral glutamine supplementation in critical illness: a systematic review' was selected, with a level II evidence and a Strong Recommendation Grade according to the researcher's expertise. The critical commentary concluded that there is sufficient evidence to affirm the beneficial effect of intravenous glutamine supplementation in reducing hospital mortality, infectious complications, mechanical ventilation days, and ICU length of stay.
