Controles rutinarios de nutriciones parenterales elaborados en la Unidad de Mezclas de Nutrición Parenteral del Hospital María Auxiliadora de Lima, octubre–noviembre 2021

Abstract

Objective: To identify errors in the preparation of parenteral nutrition through routine controls. Methodology: This was a prospective, descriptive study conducted during October and November 2022. The parenteral nutrition solutions were weighed, and their pH was measured. The theoretical weight was calculated based on the density and volume of each medication, with the acceptable percentage error for gravimetric accuracy being ±5% for preparations (≥300 ml) and ±3% for preparations (<300 ml). The medication control was conducted by comparing production sheets with medication consumption sheets. Results: A total of 384 parenteral nutrition solutions were analyzed, and 15 (3.9%) had gravimetric errors outside the acceptable range. Among the 221 preparations <300 ml, 11 (5.0%) were incorrect; and among the 163 preparations ≥300 ml, 4 (2.5%) were incorrect. All parenteral nutrition solutions had a pH within the acceptable range. In the medication control, 15 errors (1.3%) were detected: adding more volume of a medication in 7 cases (46.7%), adding less volume in 5 cases (33.3%), and using the wrong product in 3 cases (20%). Conclusions: Preparation errors were identified through routine controls, preventing them from being administered to patients. The most frequent gravimetric error occurred in preparations with a volume <300 ml, and all parenteral nutrition solutions had an appropriate pH.

Objetivo: Identificar errores en la elaboración de nutriciones parenterales mediante controles rutinarios. Metodología: Estudio prospectivo, descriptivo, durante los meses de octubre y noviembre 2022, se pesó y midió el pH de las nutriciones parenterales; para el peso teórico se consideró densidad y volumen de cada medicamento, aceptándose % error gravimétrico correcto dentro del intervalo ± 5% para preparados (≥300 ml) y ± 3% para (<300 ml), el control de medicamentos utilizados se obtuvo comparando las hojas de producción versus hojas de consumo de medicamentos. Resultados: Se analizaron 384 nutriciones parenterales, 15 (3.9%) presentaron error gravimétrico fuera del rango. De 221 preparados < 300 ml, fueron incorrectas 11 (5.0%); y 163 preparados ≥ 300 ml, fueron incorrectas 4 (2.5%). Todas las nutriciones parenterales presentaron un pH dentro del rango aceptable. En el control de medicamentos se detectaron 15 errores (1.3%), los errores fueron añadir más volumen de un medicamento 7 (46.7%), añadir menos volumen 5 (33.3%) y producto erróneo 3 (20%). Conclusiones: Se identificó errores de elaboración mediante controles rutinarios evitándose administración al paciente. El error gravimétrico más frecuente fue en preparados de volumen < 300 ml, todas las nutriciones parenterales tuvieron un pH adecuado
