Adherencia al tratamiento e independencia funcional en pacientes post accidente cerebrovascular de un hospital de Lima, 2025

Abstract

El Ictus o accidente cerebrovascular (ACV) se reconoce entre las más significantes fuentes de limitación funcional a nivel global y un problema sanitario de gran relevancia en el contexto peruano. La recuperación funcional depende en gran medida de la adherencia al tratamiento y a la neurorrehabilitación, factores determinantes para lograr independencia funcional incrementando el bienestar, aun así, existen barreras que limitan una adecuada adherencia, lo que evidencia la necesidad de estudiarla en el contexto peruano. En este marco, el presente estudio busca evaluar el vínculo entre adherencia a la neurorrehabilitación y independencia funcional en personas post-ACV atendidos dentro del Hospital de San Juan de Lurigancho, con ello generar información que permita implementar acciones preventivas y de mejora en la atención. El objetivo principal es determinar la relación entre la adherencia al tratamiento y independencia funcional en pacientes post-ACV atendidos en un hospital de Lima en 2025. Específicamente, se busca caracterizar las variables sociodemográficas y clínicas, evaluar el nivel de adherencia mediante la escala SMAN 1.0, medir la independencia funcional con el Índice de Barthel. Esta investigación es cuantitativa, de tipo observacional y diseño transversal, que se desarrollará en un hospital de Lima en 2025. La población estará conformada por pacientes adultos de entre 20 y 75 años que hayan sufrido un ACV en los últimos meses, seleccionados mediante muestreo probabilístico aleatorio simple. Para la recolección de datos se aplicarán instrumentos validados, como la escala SMAN 1.0 y el Índice de Barthel, a través de encuestas y revisión de historias clínicas. La validez y confiabilidad estarán aseguradas mediante validación previa y prueba piloto. El análisis estadístico incluirá chi-cuadrado y correlaciones, con un nivel de significancia del 95%.

Stroke or cerebrovascular accident (CVA) is recognised as one of the most significant sources of functional limitation globally and a major health problem in Perú. Functional recovery depends largely on adherence to treatment and neurorehabilitation, which are determining factors in achieving functional independence and increasing well-being. However, there are barriers that limit adequate adherence, highlighting the need to study this issue in the Peruvian context. Within this framework, the present study seeks to evaluate the link between adherence to neurorehabilitation and functional independence in post-stroke patients treated at the Hospital de San Juan de Lurigancho, thereby generating information that will enable the implementation of preventive and improvement measures in care. The main objective is to determine the relationship between treatment adherence and functional independence in post-stroke patients treated at a hospital in Lima in 2025. Specifically, the aim is to characterise sociodemographic and clinical variables, assess the level of adherence using the SMAN 1.0 scale, and measure functional independence using the Barthel Index. This research is quantitative, observational, and cross-sectional in design, and will be conducted in a hospital in Lima in 2025. The population will consist of adult patients between the ages of 20 and 75 who have suffered a stroke in recent months, selected through simple random sampling. Validated instruments, such as the SMAN 1.0 scale and the Barthel Index, will be applied for data collection through surveys and review of medical records. Validity and reliability will be ensured through prior validation and pilot testing. Statistical analysis will include chi-square and correlations, with a significance level of 95%.
