Recuperación de la guía anterior mediante un modelo biomecánico asistido por anclaje esquelético extra-alveolar: reporte de caso

Abstract

Se presenta el caso de un paciente de 29 años, sexo masculino, sin antecedentes médicos, que acudió para tratamiento de ortodoncia. Al examen clínico y radiográfico, se evidenció maloclusión Clase I con relación esquelética Clase III por protrusión mandibular, vestibularización de incisivos, relación molar Clase III, relación canina Clase III del lado izquierdo, apiñamiento severo (discrepancias alveolodentarias superior de −2,5 mm e inferior de −8 mm), línea media inferior desviada y ausencia de guía incisiva. Se registró una apertura bucal máxima aumentada (60 mm), sin sintomatología asociada. El objetivo del tratamiento fue corregir relaciones oclusales, la línea media inferior, la discrepancia dentoalveolar y restablecer la guía anterior, manteniendo el perfil facial. El plan de tratamiento consistió en la instalación de aparatología fija con brackets prescripción Roth y anclaje esquelético extra-alveolar en la zona del buccal shelf. El manejo ortodóncico se centró en la nivelación y alineación con arcos de Níquel-Titanio, seguido de retracción en masa de la dentición mandibular mediante mecánica asistida por anclaje esquelético sobre arcos de acero, y fases de acabado e intercuspidación. Como resultado, se logró la corrección del apiñamiento, la distalización mandibular, el establecimiento de relaciones oclusales estables y la recuperación de la guía incisiva, con mejoría funcional y estética. El tratamiento finalizó con la colocación de contención fija inferior y contención removible de arco continuo superior. En conclusión, este caso demuestra la eficacia del anclaje esquelético extra-alveolar para lograr la distalización en masa, oclusión mutuamente protegida y la corrección de relaciones oclusales.

This report presents the case of a 29-year-old male patient with no relevant medical history who sought orthodontic treatment. Clinical and radiographic examination revealed Class I malocclusion with a skeletal Class III relationship due to mandibular prognathism, incisor proclination, Class III molar relationship, left Class III canine relationship, severe crowding (maxillary and mandibular dentoalveolar discrepancies of −2.5 mm and −8 mm, respectively), deviation of the lower dental midline, and absence of incisal guidance. An increased maximum mouth opening of 60 mm was recorded, without associated symptoms. The treatment objectives were to correct occlusal relationships, lower midline deviation, dentoalveolar discrepancy, and to restore anterior guidance while maintaining the facial profile. The treatment plan consisted of fixed orthodontic appliances with Roth prescription brackets and extra-alveolar skeletal anchorage in the mandibular buccal shelf region. Orthodontic management focused on leveling and alignment with nickel-titanium archwires, followed by in masse retraction of the mandibular dentition using skeletal anchorage-assisted mechanics on stainless steel archwires, and finishing and intercuspation phases. As a result, crowding correction, mandibular distalization, stable occlusal relationships, and recovery of incisal guidance were achieved, with functional and esthetic improvement. Treatment was completed with the placement of a fixed mandibular retainer and a removable maxillary retainer with continuous arch design. In conclusion, this case demonstrates the effectiveness of extra-alveolar skeletal anchorage for in masse distalization, establishment of mutually protected occlusion, and correction of occlusal relationships.
