The overall objective of this study was to determine the position of the lower third molars in relation to the distance from the inferior alveolar canal in panoramic radiographs taken in Lima during the 2022–2025 period. The study was conducted using a deductive method with a quantitative approach; it was a basic, observational, non experimental, descriptive, cross-sectional, and retrospective study. The population consisted of 900 digital panoramic radiographs of patients over 20 years of age treated at a radiology center; from this, a sample of 270 images was determined using the formula for finite populations, considering a 95% confidence level and a 5% margin of error. Selection was performed using simple random probability sampling. The results showed that the mesioangular position was the most frequent (52.0%), followed by the vertical (24.3%) and horizontal (23.7%) positions. Likewise, the Langlais classification showed a predominance of the category not close to the inferior alveolar canal (50.4%); however, 49.6% presented some radiographic sign of proximity, with thinning being the most common finding (19.3%), followed by a dark band (14.6%) and discontinuity (11.5%). In conclusion, the mesioangular position predominated, and although most cases did not show proximity to the inferior alveolar canal, a significant proportion exhibited radiographic signs of proximity, highlighting the importance of a thorough panoramic evaluation to optimize surgical planning and reduce the risk of neurosensory complications.en_US
