Dolor neuropático y discapacidad por dolor lumbar en pacientes de un policlínico, 2024

Abstract

En el contexto de atención primaria en policlínicos, existe una falta de comprensión profunda sobre la magnitud y la relación entre estos tipos de dolor y su impacto funcional. El objetivo del estudio fue determinar la relación entre el dolor neuropático y la discapacidad por dolor lumbar en pacientes de un policlínico durante el año 2024. En cuanto a la metodología, se empleó un enfoque cuantitativo y la metodología hipotético deductiva. La investigación es aplicada, con un diseño observacional y de corte transversal. La muestra fue de 87 pacientes con dolor lumbar. Los datos fueron recolectados a través de encuestas, utilizando el cuestionario DN4 para diagnóstico de dolor neuropático y el cuestionario de discapacidad por dolor lumbar Oswestry. Resultados: Se observó que el 58,6% de los pacientes con dolor lumbar también experimentaban dolor neuropático. En relación con la discapacidad por dolor lumbar, la mayoría presentó discapacidad moderada (42,5%). El análisis de las dimensiones del dolor neuropático mostró que las descargas eléctricas y la sensación de quemazón fueron los síntomas más frecuentes. El análisis de hipótesis reveló que existe una relación estadísticamente significativa entre el dolor neuropático y la discapacidad por dolor lumbar, tanto en las dimensiones de síntomas como de signos del dolor neuropático. Las conclusiones indican que los síntomas como las descargas eléctricas y la quemazón, así como los signos como la alodinia y la hipoestesia, están estrechamente relacionados con una mayor discapacidad por dolor lumbar.

In the context of primary care in polyclinics, there is a lack of deep understanding about the magnitude and relationship between these types of pain and their functional impact. The aim of the study was to determine the relationship between neuropathic pain and disability due to low back pain in patients in a polyclinic during the year 2024. Regarding the methodology, a quantitative approach and the hypothetical-deductive methodology were used. The research is applied, with an observational and cross-sectional design. The sample consisted of 87 patients with low back pain. The data were collected through surveys, using the DN4 questionnaire for the diagnosis of neuropathic pain and the Oswestry low back pain disability questionnaire. Results: It was observed that 58.6% of patients with low back pain also experienced neuropathic pain. In relation to disability due to low back pain, the majority presented moderate disability (42.5%). The analysis of the dimensions of neuropathic pain showed that electric shocks and burning sensation were the most frequent symptoms. Hypothesis analysis revealed that there is a statistically significant relationship between neuropathic pain and disability from low back pain, both in the symptom and sign dimensions of neuropathic pain. The findings indicate that symptoms such as electric shock and burning, as well as signs such as allodynia and hypoesthesia, are closely related to greater disability from low back pain.
