Capacidad resolutiva y características clínicas de los pacientes en el servicio de emergencia del Hospital Sergio Bernales, Lima 2023

Abstract

El hacinamiento en los servicios de Emergencia y la problemática que se presenta por ella, son evidenciados en los sistemas nacionales e internacionales de salud, así también, se demuestra que las características de los pacientes que acuden a este servicio son diversas y esto afecta la atención oportuna y eficaz. El objetivo de este estudio fue establecer la existencia de relación entre la Capacidad resolutiva y características clínicas de los pacientes en el servicio de emergencia del hospital Sergio Bernales, Lima 2023, mediante la utilización de la Escala NEDOCS, que fue un instrumento desarrollado en los Centros Académicos de Estados Unidos y ha sido validada en el resto del mundo. El presente fue un estudio Correlacional, No experimentales, de corte transversal realizado en el mes de abril del 2023, con mediciones en 3 momentos del día (6am, 12m y 6pm) con un total de 90 mediciones para 1034 datos, la población estudiada fueron los pacientes que asistían al servicio de emergencia en estos horarios y que no estén incluidos dentro de los criterios de exclusión. Se realizó la medición no solo de la escala de NEDOCS, también se recolectó el nivel de prioridad por el que ingresaban los pacientes en emergencia determinado por el área de triaje. Los resultados fueron que el servicio de emergencia presenta un nivel peligrosamente sobreocupado con un 85.6%, seguido del nivel severamente sobreocupado con un 14.4%, con mayor congestión en el horario de medio día, según las características por nivel de prioridad se determinó que hubo una mayor afluencia de pacientes con prioridad II con 65.2% seguido de los de prioridad III con 34.5% y un mínimo ingreso de prioridad I con 0.3%, existiendo una relación entre la capacidad resolutiva y las características clínicas de los pacientes que ingresan al servicio de emergencia

The overcrowding in the Emergency services and the problems derived from it are evidenced in the national and international health systems, likewise, it is shown that the characteristics of the patients who attend this service are diverse and this affects timely and effective care. The objective of this study was to establish if there is a relationship between the resolution capacity and clinical characteristics of patients in the emergency service of the Sergio Bernales Hospital, Lima 2023, by using the NEDOCS Scale, which is a tool previously developed in Academic Centers in the United States and widely validated in the rest of the world. It was a Correlational, Non-experimental, cross-sectional study carried out in April 2023, with measurements at 3 times of the day (6am, 12 noon and 6pm) with a total of 90 measurements for 1034 data, the study population was the patients admitted to the emergency service at these times, except for patients who met the exclusion criteria. In addition to calculating the NEDOCS scale scores, the priority level for which patients were admitted to the emergency room, determined by the triage area, was collected. The results were that the emergency service presents a dangerously overcrowded level with 85.6%, followed by the severely overcrowded level with 14.4%, with greater congestion at noon, according to the characteristics by priority level, it was determined that there was a greater influx of patients with priority II with 65.2% followed by those with priority III with 34.5% and a minimum admission of priority I with 0.3%, there being a relationship between the resolution capacity and the clinical characteristics of the patients admitted to the emergency service.
