Utilidad clínica del índice NLR en población pediátrica con diagnóstico de amigdalitis en un policlínico de Lima norte, Perú 2024

Abstract

Introducción: La amigdalitis es la inflamación de las anginas que suele extenderse hasta los adenoides y las amígdalas linguales1. El diagnóstico se realiza según una puntuación clínica que permite además guiar el tratamiento de elección, pero existen pruebas de laboratorio que pueden usarse en la evaluación de pacientes con sospecha de amigdalitis. Durante la rutina se conoce que el diagnóstico de amigdalitis se realiza utilizando los signos clínicos observados por el médico tratante y su etiología suele ser atribuida a origen viral, debido a prevalencia.; sin embargo, a veces la clínica del paciente no es concluyente. En base a ello, se hace necesaria la evaluación de otros parámetros que podrían estar vinculados a la afección de las amígdalas. Por ello, estudiar el comportamiento del índice NLR es una alternativa fácil y rápida que permita conocer su vinculación con el curso de este proceso inflamatorio. Es entonces que el objetivo de este estudio es fue determinar la utilidad clínica del índice NLR en población pediátrica con diagnóstico de amigdalitis. Materiales y métodos: Se realizó una recopilación de una base de datos de Policlínico ROAL Laboratorios S.A.C., de pacientes atendidos en el año 2024, que cumplían los criterios de inclusión y exclusión. Posteriormente, esta recopilación fue sometida a pruebas estadísticas. Resultados: Se determinó que el índice tiene relación directamente proporcional con la severidad del cuadro de amigdalitis; sin embargo, el AUC de 0.536 muestra que tiene baja capacidad de discriminar, por sí mismo, a pacientes que requieren o no hospitalización. Conclusiones: Se demostró la utilidad clínica del índice NLR en población pediátrica con diagnóstico de amigdalitis.

Introduction: Tonsillitis is the inflammation of the tonsils that usually extends to the adenoids and lingual tonsils. 1 The diagnosis is made based on a clinical score that also helps guide the treatment of choice, but there are laboratory tests that can be used in the evaluation of patients with suspected tonsillitis. Routinely, it is known that the diagnosis of tonsillitis is made using the clinical signs observed by the treating physician, and its etiology is usually attributed to a viral origin, due to a prevalence. However, sometimes the patient's clinical symptoms are inconclusive. Based on this, it is necessary to evaluate other parameters that could be linked to tonsil involvement. Therefore, studying the behavior of the NLR index is a simple and rapid alternative that allows us to understand its relationship with the course of this inflammatory process. Therefore, the objective of this study is to determine the clinical usefulness of the NLR index in the pediatric population diagnosed with tonsillitis. Materials and methods: A database of patients treated in 2024 who met the inclusion and exclusion criteria was compiled from the ROAL Laboratorios S.A.C. Polyclinic. This database was subsequently subjected to statistical testing. Results: The index was determined to be directly related to the severity of tonsillitis; however, the AUC of 0.536 indicates that it has a low ability to discriminate, on its own, between patients who require hospitalization and those who do not. Conclusions: The clinical utility of the NLR index was demonstrated in the pediatric population diagnosed with tonsillitis.
