Valor diagnóstico de la prueba de esfuerzo en la detección de enfermedad coronaria crónica en un hospital de Lima, 2023

Abstract

Objetivo: Determinar el valor diagnóstico de la prueba de esfuerzo en la detección de enfermedad coronaria crónica en pacientes sometidos a cateterismo cardíaco en el Hospital Nacional Edgardo Rebagliati Martins, durante el año 2023. Métodos: Se realizó un estudio observacional, transversal, con enfoque cuantitativo, en 143 pacientes que fueron sometidos a prueba de esfuerzo y cateterismo cardíaco. La información fue recolectada de manera retrospectiva mediante revisión de historias clínicas. Se calcularon la sensibilidad, especificidad, valor predictivo positivo (VPP) y valor predictivo negativo (VPN) de prueba de esfuerzo utilizando el cateterismo como prueba de referencia. Resultados: La edad promedio fue de 65 ± 14.6 años, con predominio del sexo masculino (69%). La prueba de esfuerzo fue positiva en el 31.5% de los casos, mientras que el cateterismo evidenció lesiones coronarias significativas en el 23.8%. La sensibilidad fue del 91%, especificidad del 87%, VPP del 68% y VPN del 96%. La arteria descendente anterior fue la más frecuentemente afectada (64.7%). Conclusión: La prueba de esfuerzo demostró ser una herramienta diagnóstica útil destacando especialmente su elevado valor predictivo negativo lo cual refuerza su utilidad como herramienta de cribado para descartar enfermedad coronaria significativa. Estos hallazgos son relevantes para la optimización de recursos diagnósticos en contextos con limitaciones tecnológicas como el sistema de salud peruano.

Objective: To determine the diagnostic value of the exercise stress test in detecting chronic coronary artery disease in patients undergoing cardiac catheterization at the Hospital Edgardo Rebagliati Martins National during the period of 2023. Methods: An observational, cross-sectional study with a quantitative approach was conducted in 143 patients who underwent both exercise stress testing and cardiac catheterization. Data were collected retrospectively from medical records. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the stress test were calculated using catheterization as gold standard. Results: The mean age was 65 ± 14.6 years, with a predominance of male patients (69%). The exercise stress test was positive in 31.5% while significant coronary lesions were found in 23.8% through catheterization. Sensitivity was 91%, specificity 87%, PPV 68%, and NPV 96%. The most frequently affected artery was the left anterior descending artery (64.7%). Conclusion: The exercise stress test demonstrated high diagnostic value particularly due to its elevated negative predictive value supporting its use as a screening tool for detecting significant coronary artery disease. These findings are relevant for optimizing diagnostic resources in healthcare systems with limited access such as in Peru.
