| dc.description.abstract | Los afectados por la diabetes mellitus tipo 2 tienden a relacionarse con infecciones de significancia clínica como ocurre con los procesos infecciosos del aparato urinario. Esto se debe a que dichos pacientes presentan un estado inmunocomprometido a causa de la hiperglicemia mal controlada, produciendo en ellos una inmunosupresión. Se tuvo como objetivo analizar los factores de riesgo para desarrollar infecciones urinarias resistentes a BLEE en pacientes diabéticos tipo 2 en un Hospital Lima Centro. Por otro lado, se empleó el método hipotético deductivo, enfoque cuantitativo, tipo de investigación aplicada, diseño observacional analítico retrospectivo de corte transversal. La muestra estuvo conformada por 360 pacientes con diabetes mellitus tipo 2 e infección del tracto urinario, el instrumento aplicado es la ficha de recolección de datos de elaboración propia validada por juico de expertos y el análisis estadístico se realizó con el software estadístico SPSS v26. Se evidenció que el 68.6% de los pacientes presentó urocultivo positivo para Escherichia coli BLEE. Además, se encontró asociación estadísticamente significativa con el sexo femenino (p = 0.002), antecedente de infección del tracto urinario (p<0.001; OR = 2.49), tratamiento irregular (p = 0.001; OR = 0.46) y hemoglobina glicosilada ≥7.0% (p<0.001; OR = 2.58). Escherichia coli fue el principal microorganismo aislado (86.4%). El patrón farmacológico mostró alta resistencia a aminopenicilinas, cefalosporinas, fluoroquinolonas y glucopéptidos, con sensibilidad conservada para carbapenems. En conjunto, estos hallazgos evidencian la presencia de factores clínicos y laboratoriales asociados al desarrollo de infecciones urinarias resistentes a BLEE en pacientes diabéticos tipo 2, resaltando el antecedente de infección del tracto urinaria, el tratamiento irregular y el mal control glicémico. | es_ES |
| dc.description.abstract | Patients with type 2 diabetes mellitus tend to experience clinically significant infections, such as urinary tract infections. This is because these patients are immunocompromised due to poorly controlled hyperglycemia, resulting in immunosuppression. The objective of this study was to analyze the risk factors for developing extended-spectrum beta-lactamase (ESBL)-resistant urinary tract infections in type 2 diabetic patients at a hospital in central Lima. The hypothetico-deductive method was used, with a quantitative approach, applied research design, and a retrospective, cross-sectional, analytical observational study. The sample consisted of 360 patients with type 2 diabetes mellitus and urinary tract infections. The data collection instrument was a self-developed form validated by expert review, and statistical analysis was performed using SPSS v26 software. The results showed that 68.6% of the patients had positive urine cultures for ESBL-producing Escherichia coli. Furthermore, a statistically significant association was found with female sex (p = 0.002), a history of urinary tract infection (p < 0.001; OR = 2.49), irregular treatment (p = 0.001; OR = 0.46), and glycated hemoglobin ≥7.0% (p < 0.001; OR = 2.58). Escherichia coli was the most frequently isolated microorganism (86.4%). The drug pattern showed high resistance to aminopenicillins, cephalosporins, fluoroquinolones, and glycopeptides, with preserved sensitivity to carbapenems. Taken together, these findings demonstrate the presence of clinical and laboratory factors associated with the development of ESBL-resistant urinary tract infections in patients with type 2 diabetes, highlighting a history of urinary tract infection, irregular treatment, and poor glycemic control. | en_US |
